Retrospective analysis of 514 cases of tibial plateau fractures based on morphology and injury mechanism.

BACKGROUND: Tibial plateau fractures remain a clinical challenge due to the complexity of the fracture patterns which have been repeatedly categorized by many researchers. However, limitations do exist in some respects. So we aimed to reclassify tibial plateau fractures based on injury mechanism and morphological characteristics. METHODS: Five hundred fourteen cases of tibial plateau fractures were enrolled. The X-rays and CT scans were analyzed. RESULTS: According to our observation and analysis, tibial plateau fractures can be categorized into the following six types: (1) Lateral condylar

Outcome following subluxation of mobile articulating spacers in two-stage revision total knee arthroplasty.

INTRODUCTION: Infection after total knee arthroplasty (TKA) is a severe complication. It is usually treated with two-stage revision and implantation of a cement spacer. Few studies describe the complications associated with a mobile articulating spacer. This study examined the subluxation of articulating antibiotic spacers in knees and correlated it with prospectively collected early outcome scores after implantation of a revision prosthesis. METHODS: Staged revisions for 72 infected primary total knee arthroplasties between 2004 and 2012 were examined. The mean age of the patients was 70.2 ±

[Larsen syndrome: multicenter study of 12 new cases. Diagnosis, planning and results of treatment].

Larsen's syndrome is characterized by the association of congenital knee, hip and elbow dislocations, characteristic facial abnormalities, joint hyperlaxity, and other inconstant malformations. A review is made after description of 12 new cases from a multicenter study. A partial modification of diagnostic criteria is proposed, based on the analysis of the frequency and localization of the deformities. Radiological aspects, surgical indications, clinical evolution, complications and late results are presented, with emphasis on lower extremity problems. Good clinical results are more remarkable

Patellar morphology is different in patellofemoral instability: An MRI comparative case-control study.

PURPOSE: Although patellofemoral instability (PFI) affects both femoral and patellar compartments, literature provided little attention for the patellar morphology contribution on PFI. This study evaluates the patellar morphology patterns on MRI to establish their contribution in PFI. METHODS: This study retrospectively analyzes patellar MRI and X-ray measurements performed between 2018 and 2022. 50 knees with recurrent patellar dislocation were matched with 50 matched knees of ACL-reconstruction candidates with no history of patellar dislocation based on age and gender. Surgical treatment for recurrent patellar dislocation with severe torsional deformities: Double-level derotational osteotomy may not have a clear advantage over single-level derotational osteotomy in improving clinical and radiological outcomes.

PURPOSE: The purpose of this study was to investigate whether double-level (femur + tibia) derotational osteotomy is superior to single-level femoral derotational osteotomy for recurrent patellar dislocation with severe femoral and tibial rotational deformities (femoral anteversion >30° and external tibial torsion >30°). METHODS: Between January 2015 and June 2020, a total of 115 knees with recurrent patellar dislocation treated with combined medial patellofemoral ligament reconstruction (MPFL-R) and derotational osteotomies were evaluated after a minimum follow-up of 2 years. A combined procedure with Bereiter-type trochleoplasty leads to a stable patellofemoral joint at 5-year follow-up.

PURPOSE: Trochlear dysplasia is a major risk factor predisposing to patellar instability and has been reported to occur in up to 85% of patients with recurrent patellar dislocation. Different operative techniques have been described to correct trochlear dysplasia, including the Bereiter technique, in which subchondral bone is removed and the cartilage can be compressed into a U-shaped groove. The hypothesis was that after a Bereiter-type trochleoplasty combined with medialisation of the tibial tubercle and MPFL reconstruction, patellar pain and instability decreased and anatomic reconstruction

The Relationship of the Femoral Physis and the Medial Patellofemoral Ligament in Children: A Cadaveric Study.

BACKGROUND: Young athletes sustain patellar dislocations in a variety of sports. The medial patellofemoral ligament (MPFL) is a critical structure that functions as an anatomic checkrein to help prevent lateral patellar dislocation. Reconstruction of this ligament is challenging in patients with open physes because of concerns about iatrogenic damage to the femoral physis. PURPOSE: To evaluate the relationship of the distal femoral physis and the MPFL. STUDY DESIGN: Descriptive laboratory study. METHODS: In 15 cadaveric, pediatric knees (age, 7-11 years), markers were placed at the proximal

Arthroscopic Treatment of Combined Anterior and Posterior Cruciate Ligament Avulsion Fracture: A Case Report.

Anterior cruciate ligament (ACL) avulsion fractures without any concomitant injuries are extremely rare in skeletally mature patients and sporadically reported in the literature. Such injuries are more likely to be associated with tibial plateau fractures, other knee ligament ruptures, distal femoral fractures, or knee dislocations. In this article, a case of a 55-year-old male, who suffered a combined displaced ACL-posterior cruciate ligament (PCL) avulsion fracture, without any related injuries, is reported. The fracture was treated all arthroscopically with both an internal fixation and a s…

Mid term results of LCS knee: The Indian experience.

BACKGROUND: The low contact stress rotating platform (LCS RP) knee (DePuy Orthopedics, Inc, Warsaw, Indiana), in use for last four decades in Western population, is reported to have a survival of more than 95% at 15 to 20 years. The reported Indian experience of this knee is limited to 5 years. Our aim was to report the clinical and radiological results of the LCS RP TKA design in the Indian population with a minimum followup of 10 years. MATERIALS AND METHODS: Fifty-five LCS knees (45 patients) operated between February 1997 and October 2001 were evaluated retrospectively. LCS design was gen

Current concepts in the treatment of first-time patella dislocation in children and adolescents.

BACKGROUND: Lateral dislocation of the patella is a common injury in children. It can occur in previously healthy and anatomically normal knees, but there are several abnormalities that predispose to patellar instability. Magnetic resonance imaging is an essential part of assessing for associated injuries and risk of further instability. Treatment aims to prevent redislocation, residual instability, osteoarthritis, and allow return to previous activities. The purpose of this review was to assess evidence for management of first-time patella dislocation in children and adolescents. METHODS: Li

Complications of high tibial osteotomy and internal fixation with staples.

Osteotomy for osteoarthritis of the knee has established itself well since its first description by Jackson and Waugh [8, 9]. Internal fixation with staples allows early functional treatment with only a minimum of operative intervention. Removal of the metal is optional. In the 5-year period from 1986 to 1990, 182 high tibial osteotomies were performed at the Orthopaedic Department of the University Hospital of Freiburg. In four cases internal fixation was done with plates; in 178 cases two or more staples were used; in 3 cases a screw was additionally inserted for better hold. The staples bec

[Use of the Pavlik harness before the age of 3 months in congenital hip dislocation].

The authors report 128 cases of reduction of congenital dislocation of the hip by the Pavlik harness with more than one year of follow-up. They studied 60 patients who had less than 3 months of age at the beginning of the treatment. They obtain 59 reductions (98%) without necrosis of femoral heads. The only case of unsuccess, treated after by continuous extension, has doubtful femoral heads at 15 months of age. The femoral heads appear in normal delay and walk was normal between 10 and 18 months. Only a strict watchfulness, as strict as a continuous extension, can obtain good results with prog
