Immune reconstitution inflammatory syndrome associated with PML in AIDS: a treatable disorder.

An HIV-1-infected patient with progressive multifocal leukoencephalopathy presented clinical deterioration and contrast-enhancing lesions on brain nuclear MR after the initiation of highly active antiretroviral therapy (HAART). Brain biopsy identified an inflammatory reaction compatible with immune reconstitution inflammatory syndrome. Treatment with corticosteroids and transient suppression of HAART led to marked neurologic improvement.

[Molecular Pathogenesis of Nasu-Hakola Disease Brain Lesions].

Nasu-Hakola disease (NHD) is a rare intractable autosomal recessive disorder, characterized by pathological bone fractures and progressive dementia owing to multifocal bone cysts and leukoencephalopathy, caused by various genetic mutations of either DAP12 or TREM2. Loss-of-function of TREM2-DAP12, constituting a signaling complex on osteoclasts and microglia, plays a central role in the pathogenesis of NHD. Recently, NHD has been recognized as the disease entity designated "microgliopathy". However, at present, TREM2-specific ligands in microglia and the precise molecular mechanism underlying

Leukoencephalopathy with cognitive impairment following tocilizumab for the treatment of rheumatoid arthritis (RA).

The biological agent tocilizumab, is a humanized, anti-human interleukin-6 receptor antibody. A 72-year-old woman developed cognitive impairment during the Phase III clinical trial of tocilizumab for the treatment of rheumatoid arthritis. MRI demonstrated hyperintense dissemination throughout the white matter on T2WI. An initial diagnosis of possible progressive multifocal leukoencephalopathy was made, but the PCR for JC virus DNA was negative in the CSF. The leukoencephalopathy might have been caused by a mechanism related to tocilizumab itself. It is strongly recommended to perform MRI if a

PARTICLES RESEMBLING PAPOVA VIRUSES IN HUMAN CEREBRAL DEMYELINATING DISEASE.

Degenerated brain tissue obtained from a deceased patient who had progressive multifocal leukoencephalopathy had been fixed in formalin prior to processing for electron microscopy. In ultrathin sections virus-like particles resembling papova virions were frequently observed in glial nuclei. Correlation with light-microscopy findings suggests that demyelination resulted from the cytocidal effect of the virus on oligodendroglia.

[The 518th case: multiple intracranial lesions, fever, rash].

This article reports the case of a 58-year-old male patient who presented with a generalized rash, intermittent fever, and a concomitant perianal abscess, as well as mycoplasma pneumoniae psittacosis. The initial symptom was episodic slurred speech, which progressively developed into symptoms such as delayed responses and left-sided limb weakness. Cranial magnetic resonance imaging revealed gradually progressive intracranial lesions characterized primarily by asymmetric white matter damage, with no enhancement upon contrast administration. Serum HIV antibody testing was positive. Next-generati

[A case of treatment-related leukoencephalopathy during combination therapy with daratumumab, lenalidomide, and low-dose dexamethasone for multiple myeloma].

A 75-year-old woman with multiple myeloma was admitted due to an acute disturbance of consciousness and aphasia two months after administration of daratumumab, lenalidomide and dexamethasone combination therapy (DLd therapy). Brain MRI on admission showed no acute ischemic changes. She was treated with antiepileptic drugs, and DLd therapy was discontinued, but her consciousness deteriorated, and follow-up brain MRI showed progressive FLAIR high-signal intensity lesions in the cerebral deep white matter bilaterally. Though methylprednisolone pulse therapy and high-dose intravenous immunoglobuli

Myocardial infarction in a patient with multiple sclerosis after receiving natalizumab.

Natalizumab is among the most effective drugs for controlling multiple sclerosis (MS). The most important side effect of this drug is progressive multifocal leukoencephalopathy, although some other complications are increasingly reported in literature such as meningitis and encephalitis. Here, a patient with MS who has suffered from myocardial infarction (MI) after the injection of natalizumab is introduced.

Induction of undifferentiated brain tumors in rats by a human polyomavirus (JC virus).

Newborn SD rats were inoculated intracranially with JC virus (Tokyo-1), a human polyomavirus, isolated from the autopsied brain of a patient with progressive multifocal leukoencephalopathy. Twenty-one to 61 weeks later, 20 of 27 rats developed tumors in the cerebrum, but not in the cerebellum. The undifferentiated neuroectodermal nature of the tumors was histologically, immunohistochemically and ultrastructurally confirmed.

[Radiological diagnosis of viral encephalitis].

Most of viral encephalitis may demonstrate no specific change on CT and MR images. Brain swelling, edema, abnormal density (CT) and abnormal intensity (MR) can be detected in herpes simplex encephalitis and enterovirus encephalitis (coxsackie, echo, polio). The common finding on CT and MRI in patients with HIV encephalopathy are atrophy, leukomalacia. Progressive multifocal leukoencephalopathy (PML) shows multifocal oval or round white matter T2-hyperintensities on MR images. Subacute sclerosing panencephalitis (SSPE) may present slight changes in the subcortical and periventricular white matt

JC virus PCR assay reporting for PML: promises, perils, and pitfalls.

One of the most devastating central nervous system infections, progressive multifocal leukoencephalopathy (PML), is caused by the JC virus (JCV). In immunosuppressed patients, PML is a well-recognized complication recognized by clinicians across specialties, such as neurology, internal medicine, oncology, and rheumatology. It can be encountered in patients receiving hematopoietic or solid-organ transplants, cytotoxic chemotherapy, immune checkpoint inhibitors, or long-term immunosuppressive therapies. In neurological settings, those treated with natalizumab, rituximab, ocrelizumab, fingolimod,

Complexities of JC Polyomavirus Receptor-Dependent and -Independent Mechanisms of Infection.

JC polyomavirus (JCPyV) is a small non-enveloped virus that establishes lifelong, persistent infection in most of the adult population. Immune-competent patients are generally asymptomatic, but immune-compromised and immune-suppressed patients are at risk for the neurodegenerative disease progressive multifocal leukoencephalopathy (PML). Studies with purified JCPyV found it undergoes receptor-dependent infectious entry requiring both lactoseries tetrasaccharide C (LSTc) attachment and 5-hydroxytryptamine type 2 entry receptors. Subsequent work discovered the major targets of JCPyV infection in

CADASIL: hereditary disease of arteries causing brain infarcts and dementia.

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) begins with migraine with aura in approximately one-third of the patients. More severe symptoms of recurrent strokes usually appear at 30-50 years of age. However, well before the first stroke, CADASIL may be diagnosed on the basis of characteristic hyperintensities in T2-weighted magnetic resonance images. Multiple lacunar infarcts located mainly in the basal ganglia and frontal white matter lead to a cognitive decline and finally to dementia. These infarcts result from a thickening and fibros

JC virus granule cell neuronopathy in a child with CD40 ligand deficiency.

JC virus infection of the brain typically causes progressive multifocal leukoencephalopathy, a demyelinating disease that rarely involves gray matter. This report presents a case of cerebellar degeneration associated with JC virus infection in a male with CD40 ligand deficiency resulting in hyperimmunoglobulin M type 1. This patient exhibited a progressive cerebellar ataxia with progressive atrophy of the cerebellar cortex in association with the presence of JC virus in the spinal fluid. JC virus infection should be considered in the differential diagnosis of ataxia in children with inherited

[Fatal toxic leukoencephalopathy associated with consumption of pasta base of cocaine: Report of three cases].

The prevalence of drug-associated toxic encephalopathy is unknown, but it is an uncommon condition. Toxic leukoencephalopathy was described associated with heroin consumption, it has been less commonly described with the use of cocaine and there are no reports of its association with consumption pasta base of cocaine (PBC). We report two females aged 31 years and a male aged 19 years, consumers of PBC who developed a fatal toxic leukoencephalopathy. They initiated their disease with severe and persistent headache, sequential focal neurologic deficits and a progressive impairment of consciousne

Safety and efficacy of extended versus standard interval dosing of natalizumab in multiple sclerosis patients: a systematic review and meta-analysis.

BACKGROUND: Multiple sclerosis (MS) is a chronic inflammatory, immune-mediated disease affecting the central nervous system. Natalizumab, an FDA-approved monoclonal antibody for MS, has been explored for its off-label extended interval dosing (EID), suggesting a potential reduction in the risk of progressive multifocal leukoencephalopathy (PML) compared to standard interval dosing (SID). Our objective was to assess the efficacy and safety of EID in comparison to SID for natalizumab treatment in patients with MS. METHODS: We searched PubMed, Embase, WOS, Scopus, Ovid, Science Direct, Clinical

Dynamics and Patterning of 5-Hydroxytryptamine 2 Subtype Receptors in JC Polyomavirus Entry.

The organization and dynamics of plasma membrane receptors are a critical link in virus-receptor interactions, which finetune signaling efficiency and determine cellular responses during infection. Characterizing the mechanisms responsible for the active rearrangement and clustering of receptors may aid in developing novel strategies for the therapeutic treatment of viruses. Virus-receptor interactions are poorly understood at the nanoscale, yet they present an attractive target for the design of drugs and for the illumination of viral infection and pathogenesis. This study utilizes super-reso
