In-bed vital signs monitoring system based on unobtrusive magnetic induction method with a concentric planar gradiometer.

SIGNIFICANCE: Unobtrusive vital signs monitoring is of major importance for various medical areas such as detection and treatment of sleep disorders, monitoring neonates and burned victims, home health care and smart home applications and wearables among others. Such applications call for monitoring methods in which the patient's natural state is less interfered with. An ideal device would be non-invasive, minimally restrictive, robust enough to compensate movements of the patients, and would operate without relying on the patient's full cooperation. OBJECTIVE: This paper focuses on the desig

Cardiopulmonary monitoring of shock.

PURPOSE OF REVIEW: We will briefly review the classification of shock and the hallmark features of each subtype. Available modalities for monitoring shock patients will be discussed, along with evidence supporting the use, common pitfalls, and practical considerations of each method. RECENT FINDINGS: As older, invasive monitoring methods such as the pulmonary artery catheter have fallen out of favor, newer technologies for cardiac output estimation, echocardiography, and noninvasive tests such as passive leg raising have gained popularity. Newer forms of minimally invasive or noninvasive moni

Circulating Tumor Cells as Cancer Biomarkers in the Clinic.

It is believed that the development of metastatic cancer requires the presence of circulating tumor cells (CTCs) , which are found in a patient's circulation as rare abnormal cells comingled with billions of the normal red and white blood cells. The systems developed for detection of CTCs have brought progress to cancer treatment. The molecular characterization of CTCs can aid in the development of new drugs, and their presence during treatment can help clinicians determine the prognosis of the patient. Studies have been carried out in patients early in the disease course, with only primary tu

Minimally invasive or noninvasive cardiac output measurement: an update.

Although cardiac output (CO) by pulmonary artery catheterization (PAC) has been an important guideline in clinical management for more than four decades, some studies have questioned the clinical efficacy of CO in certain patient populations. Further, the use of CO by PAC has been linked to numerous complications including dysrhythmia, infection, rupture of pulmonary artery, injury to adjacent arteries, embolization, pulmonary infarction, cardiac valvular damage, pericardial effusion, and intracardiac catheter knotting. The use of PAC has been steadily declining over the past two decades. Mini

Characterisation of Supra- and Infratentorial ICP Profiles.

In pathophysiology and clinical practice, the intracranial pressure (ICP) profiles in the supratentorial and infratentorial compartments are unclear. We know that the pressure within the skull is unevenly distributed, with demonstrated ICP gradients. We recorded and characterised the supra- and infratentorial ICP patterns to understand what drives the transtentorial ICP gradient.A 70-year-old man was operated on for acute cerebellar infarction. One supratentorial probe and one cerebellar probe were implanted. Both signals were recorded concurrently and analysed off-line. We calculated mean ICP

In vivo calibration of esophageal pressure in the mechanically ventilated patient makes measurements reliable.

BACKGROUND: Esophageal pressure (Pes) can provide information to guide mechanical ventilation in acute respiratory failure. However, both relative changes and absolute values of Pes can be affected by inappropriate filling of the esophageal balloon and by the elastance of the esophagus wall. We evaluated the feasibility and effectiveness of a calibration procedure consisting in optimization of balloon filling and subtraction of the pressure generated by the esophagus wall (Pew). METHODS: An esophageal balloon was progressively filled in 36 patients under controlled mechanical ventilation. VBE

Circadian rhythms in bed rest: Monitoring core body temperature via heat-flux approach is superior to skin surface temperature.

Continuous recordings of core body temperature (CBT) are a well-established approach in describing circadian rhythms. Given the discomfort of invasive CBT measurement techniques, the use of skin temperature recordings has been proposed as a surrogate. More recently, we proposed a heat-flux approach (the so-called Double Sensor) for monitoring CBT. Studies investigating the reliability of the heat-flux approach over a 24-hour period, as well as comparisons with skin temperature recordings, are however lacking. The first aim of the study was therefore to compare rectal, skin, and heat-flux tempe

Using a Sequence of Earcons to Monitor Multiple Simulated Patients.

OBJECTIVE: The aim of this study was to determine whether a sequence of earcons can effectively convey the status of multiple processes, such as the status of multiple patients in a clinical setting. BACKGROUND: Clinicians often monitor multiple patients. An auditory display that intermittently conveys the status of multiple patients may help. METHOD: Nonclinician participants listened to sequences of 500-ms earcons that each represented the heart rate (HR) and oxygen saturation (SpO2) levels of a different simulated patient. In each sequence, one, two, or three patients had an abnormal leve

Walking cycle control for an active ankle prosthesis with one degree of freedom monitored from a personal computer.

This paper proposes a fuzzy control algorithm for human walking cycle of an active ankle prosthesis for people who have suffered amputation of the lower limb, the system has one degree of freedom in the sagittal plane. Also, a biomechanical analysis of foot and ankle is shown to define the phases of plantar support and swinging. The used actuator is an intelligent servomotor, Dynamixel MX-106T which has torque, current and position feedback, among others, allowing real-time telemetry of the prototype implemented in a microcontroller system.

A novel approach to detect respiratory phases from pulmonary acoustic signals using normalised power spectral density and fuzzy inference system.

BACKGROUND: Monitoring respiration is important in several medical applications. One such application is respiratory rate monitoring in patients with sleep apnoea. The respiratory rate in patients with sleep apnoea disorder is irregular compared with the controls. Respiratory phase detection is required for a proper monitoring of respiration in patients with sleep apnoea. AIMS: To develop a model to detect the respiratory phases present in the pulmonary acoustic signals and to evaluate the performance of the model in detecting the respiratory phases. METHODS: Normalised averaged power spectr

Atrial Fibrillation Monitoring in Cryptogenic Stroke: the Gaps Between Evidence and Practice.

Identifying occult paroxysmal atrial fibrillation as the etiology of cryptogenic stroke has been a top research priority in the past decade. This is because prompt initiation of anticoagulation has significantly decreased subsequent stroke risk. Available evidence suggests that prolonged cardiac monitoring after stroke increases the likelihood of detecting atrial fibrillation. However, further research is required to fill in the gaps in regard to the optimal period of monitoring, candidates for monitoring, etc. Here, we review the current evidence supporting the use of prolonged monitoring for

Monitoring the acute response in severe hypersensitivity reactions to drugs.

PURPOSE OF REVIEW: Severe adverse drug reactions (ADRs) including Stevens-Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN) and drug-induced hypersensitivity syndrome (DiHS)/drug reaction with eosinophilia and systemic symptoms (DRESS) are acute life-threatening conditions. There is the urgent need for reliable, noninvasive and standardized laboratory tests for identifying patients at higher risk of developing severe ADRs. RECENT FINDINGS: Although previous studies demonstrated the pathogenic role of TNF-α, IFN-γ, IL-10, perforin/granzyme B, Fas L and granulysin in the development of se

Clinical assessment of peripheral circulation.

PURPOSE OF REVIEW: Monitoring of the peripheral circulation can be done noninvasively in contrast to the more traditional invasive systemic haemodynamic monitoring in the intensive care unit. Physical examination of peripheral circulation based on clinical assessment has been well emphasized for its convenience, accessibility, and relation to the prognosis of patients with circulatory shock. The purpose of this article is to highlight the main findings according to recent literature into the clinical applications of the peripheral perfusion assessment in patient management. RECENT FINDINGS: C

Pilot evaluation of an unobtrusive system to detect falls at nighttime.

Research shows that older people (aged 65 years and over) suffer many unintentional indoor falls which often lead to severe injuries. As a result of an increasingly aged population in developed countries, a sizable portion of healthcare funding is consumed in the treatment of fall-related injuries and associated long-term care. Detecting falls soon after they occur can be potentially live saving. In addition, early treatment of fall-related injuries can reduce treatment costs by minimizing health deterioration resulting from long periods spent incapacitated on the floor after a fall (a scenari

[Supervision after breast cancer treatment].

Breast cancer screening and therapeutic advances have significantly decreased breast cancer mortality. Nearly 80% of patients with a localized disease will experience a prolonged survival and the organization of their monitoring is critical to early detection of local or metastatic relapse. Annual mammography is recommanded for the follow up and no further imaging examination is routinely necessary. Early diagnosis of metastatic relapse has not demonstrated evidence of survival benefit. However, treatments are not without toxicities and data about long-term complications are missing. The socio

[Usefulness of the new version 3.02 of Vigileo monitor for anesthetic management in anaphylactic shock: a case report].

We studied the utility of Vigileo monitor for grasping hemodynamics with a patient in a state of anaphylactic shock. The stroke volume variation (SVV) predicts fluid responsiveness of normal cases. In the anaphylactic shock resulting from biased blood distribution, not only blood pressure but also pulse pressure markedly decreased within a couple of minutes. SVV is calculated from the standard deviation of pulse pressure. Therefore the SVV could be overestimated during anaphylactic shock. A former version of Vigileo monitor underestimated the arterial pressure-based cardiac output (APCO) due t

Monitoring diaphragm electrical activity and the detection of congenital central hypoventilation syndrome in a newborn.

A full-term newborn infant is described with recurrent episodes of oxygen desaturation and apnea on the day of birth. The apnea did not improve with continuous positive airway pressure (CPAP) and intermittent nasal ventilation, therefore intubation and mechanical ventilation were required. A preliminary diagnosis of congenital central hypoventilation syndrome (CCHS) was made with the use of simultaneous measurements of end-tidal CO2 (EtCO2) and a diaphragm electrical activity waveform that was detected using microsensors placed on the infant's feeding tube. It was observed that during deep sle
