Cough-induced vertebral artery dissection: A case report and literature review.

Spontaneous vertebral artery dissection (VAD) is common after minor neck trauma or abrupt neck manipulation, which may result in stroke. However, violent cough is a less recognised cause of VAD. Only a few cases are reported in the literature. We review the literature and report a case of a 62-year-old man who presented with a headache and neck pain following a period of intermittent coughing, which led to VAD and a cerebellar ischaemic stroke. He responded well to medical treatment and made a full recovery. Therefore, VAD should be considered in the differential diagnosis for patients present

The Effect of Tumor Characteristics and Location on the Extent of Lymph Node Metastases of Head and Neck Cutaneous Squamous Cell Carcinoma.

BACKGROUND: The extent of a neck dissection for patients with metastasis of cutaneous squamous cell carcinoma of the head and neck (HNcSCC) is still subject to debate and clear guidelines are lacking. Tumor characteristics like size, differentiation and tumor location are known risk factors for lymph node metastasis (LNM). There is some evidence that, depending on tumor location, LNM follows a specific pattern. This study aims to identify which tumor characteristics can predict the pattern and extent of LNM. METHOD: In this cohort study 80 patients were included, who underwent a primary neck …

Internal Jugular Vein Fenestration: An Intraoperative Finding Without a Radiological Clue.

A comprehensive understanding of the anatomical variations of the internal jugular vein (IJV) is essential to prevent inadvertent injuries during neck procedures, particularly neck dissection. In addition, its relationship with the spinal accessory nerve in the upper part of the neck is relatively variable. IJV fenestration refers to bifurcation of the vein with reunion proximal to the subclavian vein, whereas IJV duplication refers to continued branching till joining the subclavian vein separately. We report a case of a fenestrated IJV identified intraoperatively with the spinal accessory ner…

Horner's Syndrome Incidental to Medullary Thyroid Carcinoma Excision: Case Report and Brief Literature Review.

Horner's syndrome is characterized by a combination of ipsilateral miosis, blepharoptosis, enophthalmos, facial anhidrosis, and iris heterochromia in existence of congenital lesions. The syndrome results from a disruption of the ipsilateral sympathetic innervation of the eye and ocular adnexa at different levels. Though rare, thyroid and neck surgery could be considered as possible causes of this clinical entity. We present a case of Horner's syndrome in a patient after total thyroidectomy and neck dissection for medullary thyroid cancer with neck nodal disease and attempt a brief review of th…

Variations in the course of the hypoglossal nerve: a case report and literature review.

The extracranial hypoglossal nerve has a well described course as it traverses the neck, and is frequently identified during neck dissection, carotid endarterectomy and other procedures involving the deep spaces of the neck. A case is presented which demonstrates aberrancy of its anatomic position. Variations of its course are well documented, however none have demonstrated a course lateral to the internal jugular vein. This serves to alert the surgeon to be mindful of such atypical presentations as the consequences of iatrogenic injury to such structures may be significant.

Squamous dedifferentiation and differentiated high-grade transformation of papillary thyroid carcinoma in metastatic lymph nodes: Two cases and literature review.

Although it is extremely rare, papillary thyroid carcinoma (PTC) can exhibit unexpected behavior, with the potential to transform into more lethal forms in metastatic sites. This report describes 2 cases in which PTC transformed within metastatic lymph nodes. Case 1: A 57-year-old woman with a progressively enlarging right neck mass was found on imaging to have cervical lymphadenopathy and small right thyroid nodules. She underwent total thyroidectomy with neck dissection. Histopathology revealed metastatic PTC with squamous dedifferentiation in cervical nodes. Immunohistochemistry confirmed t…

Dog bites or knife wounds? A case report of atypical neck injuries.

A 24-year-old woman was brought to the emergency department after suffering dog bites, as reported by a witness. Autopsy revealed 21 deep wounds of the neck, along with unclear injuries to both carotid arteries, extensive damage to neck muscles, a wound to the larynx, and the right lobe of the thyroid gland had been avulsed. The forensic pathologist initially concluded that the cause of death was asphyxia and haemorrhagic syndrome. Furthermore, due to the nature of some wounds resembling stab wounds, as well as facial injuries and marks consistent with gripping, the death was initially conside

The surgical management of carotid artery stenosis in patients with previous neck irradiation.

BACKGROUND: A history of therapeutic irradiation to the neck complicates the management of carotid artery occlusive disease. Serious surgica … r radiation included laryngeal carcinoma and lymphoma. Five patients had undergone previous radical neck dissections, and four patients had permanent tracheostomies. The surgical indications were asymptomatic high-grade stenosis in 7 cases, transient ischemic attack in 4 cases, stroke in 2 cases, and a pseudoaneurysm in 1 case. Traumatic bilateral dissections of the internal carotid artery: an infrequent diagnosis not to be missed.

We report the case of two patients involved in a car crash and victims of multiple injuries. The diagnosis of bilateral dissection of the internal carotid arteries was only evoked belatedly in view of the increase of the neurologic symptoms. They benefitted from anticoagulant treatment. The clinical evolution was good. Dissection of both carotid arteries from blunt head or neck trauma has been reported but is unusual. Diagnosis and treatment are discussed.

Diagnostic dilemma of an atraumatic clavicle fracture following radical treatment for laryngeal carcinoma.

An atraumatic clavicular fracture presented after radical treatment for laryngeal carcinoma. This presented a diagnostic dilemma. The differential diagnosis included metastatic bone disease and osteomyelitis as well as post-radiotherapy complications. After investigation, the cause was thought to be a post-radiation fracture of the clavicle and to the best knowledge of the authors, this is the first ever documented in a patient who had undergone a total laryngectomy with bilateral modified radical neck dissections and post-operative radiotherapy. Cases of a fractured clavicle post-radiation ha…

Mediastinal dissection--1976: indications and newer techniques.

Head and neck neoplasms may invade the mediastinum by direct extension or metastases to the tracheoesophageal or jugulo-subclavian lymph nod … area which usually had prior treatment with radiation and/or surgery. Over 60 transsternal radical neck dissections have been performed in the past 20 years. We have in the past evaluated the morbidity and survivals in stomal cases and present here our revised indications and techniques.

Anatomic relationship between the spinal accessory nerve and the jugular vein: a cadaveric study.

BACKGROUND/PURPOSE: Previous studies of the course of the Spinal Accessory Nerve (SAN) and its relationship to the Internal Jugular Vein (IJV) have yielded conflicting results because of the small number of anatomic specimens and anatomic variability. Classic teaching in Head and Neck Surgery is that the SAN almost always crosses the IJV anteriorly in the upper neck. However, because of the morbidity associated with the injury to the IJV during nerve dissection, it is imperative that the surgeon is wary of the posteriorly crossing nerve. In order to further elucidate the anatomy of the SAN in

Trampolines, children, and strokes.

Strokes in children related to sports injuries are rare, but pediatric trampoline injuries are dramatically increasing. Minor trauma to the vulnerable extracranial vertebral arteries as they travel superficially through the dorsum of the neck can begin a cascade of events that results in arterial dissection, thrombus formation, and embolization with cerebral infarction. We present the case of an 11-yr-old boy who developed left vertebral artery dissection subsequent to a trampoline injury.

Spontaneous internal carotid artery dissection presenting as hypoglossal nerve palsy.

A 42-year-old man presented with right temporal headache, dysarthria, and dysphagia. On examination, he had a right hypoglossal nerve palsy. The diagnosis of right internal carotid artery dissection was suggested by magnetic resonance imaging and confirmed by carotid angiography. A dynamic computed tomogram demonstrated enlargement of the carotid artery. In carotid dissection, the hypoglossal nerve may be compromised by local factors as it passes close to the carotid artery in the neck.

[Surgical aspects of the treatment of malignant thyroid gland tumors].

95 cases with primary malignant thyroid tumours underwent surgery from 1965 to 1977. In view of the interdisciplinary character of treatment and independent of histological differentiation, total thyroidectomy should be performed. Differentiated carcinomas should be treated by selective dissection of suspected cervical lymph nodes. Surgical intervention of undifferentiated and medullary carcinomas should be followed by unilateral or bilateral neck-dissection. The survival rate is shown to depend on histological stage and extension of the tumour.

Stroke, cerebral artery dissection, and cervical spine manipulation therapy.

Stroke represents an infrequent adverse reaction associated with cervical spine manipulation therapy. Attempts to identify the patient at risk and the type of manipulation most likely to result in these complications of manipulation have not been successful. A retrospective review of 64 medical legal cases of stroke temporally associated with cervical spine manipulation was performed to evaluate characteristics of the treatment rendered and the presenting complaints in patients reporting these complications. These files included records from the practitioner who administered the manipulation t
