Parathyroids: the forgotten glands.

Symptomatic calcium disorders--most notably, hypocalcemia--are common problems in preterm and certain term infants. Many factors predispose susceptible neonates to hypo- or hypercalcemia, but in many cases, the root of the problem is altered function of the parathyroid glands. Parathyroid gland dysfunction may affect calcium homeostasis and alter physiologic functioning, resulting in significant clinical symptoms. A review of how the parathyroid glands and parathyroid hormone affect calcium balance and of the problems that result from altered function promotes a better appreciation of the impo

Neonatal abstinence syndrome: therapeutic interventions.

Neonatal Abstinence Syndrome (NAS) occurs in infants exposed to opiates or illicit drugs during pregnancy. It can be severe and cause long hospital stays after birth and with symptoms up to 6 months after birth. Pharmacologic interventions are commonly used as treatment for NAS; however, their safety and efficacy are not fully recognized. Pharmacologic treatments for NAS include medications such as methadone, buprenorphine, morphine, and phenobarbital. Nonpharmacologic interventions and complementary therapies have been documented in neonates. However, there are gaps in the literature regardin

Monitoring apnea of prematurity: validity of nursing documentation and bedside cardiorespiratory monitor.

OBJECTIVE: To compare apnea events recorded by bedside cardiorespiratory monitor and nursing documentation with those detected by visual inspection of continuous electronic cardiorespiratory waveform. METHODS: In a prospective observational study, 20 nonventilated infants of 28 to 33 weeks' gestational age were monitored for apnea during the first 2 postnatal weeks. Apnea was defined as a respiratory pause > 20 seconds or > 15 seconds if associated with a heart rate < 80/min or oxygen saturation < 85%. True apnea was defined as one for which visual inspection of continuous electronic cardiore

Obstetric hemorrhage risk assessment tool predicts composite maternal morbidity.

Obstetric hemorrhage is one of the leading preventable causes of maternal mortality in the United States. Although hemorrhage risk-prediction models exist, there remains a gap in literature describing if these risk-prediction tools can identify composite maternal morbidity. We investigate how well an established obstetric hemorrhage risk-assessment tool predicts composite hemorrhage-associated morbidity. We conducted a retrospective cohort analysis of a multicenter database including women admitted to Labor and Delivery from 2016 to 2018, at centers implementing the Association of Women's Heal

Immunization Status of NICU Graduates at a Tertiary Care Children's Hospital.

BACKGROUND: Approximately 500,000 infants are born prematurely each year in the United States. Immunization of infants in a neonatal intensive care unit (NICU) set a precedence for future immunizations. PURPOSES: The objectives of this study were to determine the current rates of immunization and identify variables associated with immunizations of NICU graduates who were aged 60 days or older at time of discharge. METHODS: This descriptive pilot study utilized retrospective paper medical record review in one tertiary children's hospital. The relationships between immunization status and stud

Part 1: early recognition and treatment of birth trauma: injuries to the head and face.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Sentinel Event Alert from July 21, 2004 states that there have been 47 cases of birth trauma-related perinatal death or permanent disability reported for JCAHO review since 1996. This report clearly illustrates the importance of birth trauma in clinical practice for neonatal and perinatal nurses. Estimates suggest that birth trauma occurs in 2% to 7% of all deliveries and is associated with an increase in both mortality and morbidity. Birth trauma to the head may result in minor superficial extracranial injuries, such as

Predicting adjustment to perinatal death.

One hundred and thirty parents who had experienced a perinatal death completed a self-administered questionnaire that examined the demographic characteristics of parents, factors associated with the loss, and their levels of satisfaction with the amount of support they received. In addition, they completed scales measuring current levels of depression, self-esteem and psychological well-being. Comparisons with available norms revealed that, while parents were more depressed than general members of the community, they showed fewer symptoms of depression than did depressed patients. In addition,

Catheter-related bloodstream infections in neonatal intensive care units.

Central venous catheters (CVCs) are regularly used in intensive care units, and catheter-related bloodstream infection (CRBSI) remains a leading cause of healthcare-associated infections, particularly in preterm infants. Increased survival rate of extremely-low-birth-weight infants can be partly attributed to routine practice of CVC placement. The most common types of CVCs used in neonatal intensive care units (NICUs) include umbilical venous catheters, peripherally inserted central catheters, and tunneled catheters. CRBSI is defined as a laboratory-confirmed bloodstream infection (BSI) with e

Electronic fetal monitoring: a brief summary of its development, problems and prospects.

Electronic fetal monitoring (EFM) was introduced into clinical practice 30 years ago and spread very rapidly in most developed countries. The early optimism that EFM would lead to a marked reduction in fetal neurological injury has not been realized; however, it is now recognized that most such damage is unrelated to perinatal events. Clinical trials have shown that although EFM does reduce the incidence of intrapartum asphyxia, its use is also associated with an increase in cesarean sections. Abnormal fetal heart rate (FHR) patterns are poor predictors of fetal depression at birth when used w

Prostanoids and their analogues for the treatment of pulmonary hypertension in neonates.

BACKGROUND: Persistent pulmonary hypertension of the newborn (PPHN) is a disease entity that describes a physiology in which there is persistence of increased pulmonary arterial pressure. PPHN is characterised by failure to adapt to a functional postnatal circulation with a fall in pulmonary vascular resistance. PPHN is responsible for impairment in oxygenation and significant neonatal mortality and morbidity. Prostanoids and their analogues may be useful therapeutic interventions due to their pulmonary vasodilatory and immunomodulatory effects. OBJECTIVES: Primary objective• To determine the
