Orbital streptococcal gangrene and AIDS.

Streptococcal gangrene can present to the ophthalmologist as a fulminant orbital cellulitis involving the eyelids, globe and orbit. Compromised vision is likely, as well as serious systemic illness and death. It commonly affects young and healthy hosts and, prior to this presentation, has never been reported in a patient with HIV infection. We studied a case of streptococcal gangrene of the eyelids and orbit in a patient with AIDS. The infection was treated with debridement and intravenous antibiotics, and showed eventual improvement with the exception of visual function. A biopsy of the lesio…

Orbital and optic nerve trauma.

The ability to measure orbital volume expansion and correlate this with later enophthalmos has advanced radiological assessment of patients with orbital floor fractures. Attempts have also been made to classify orbital floor fractures on the basis of their configuration on CT scan and to correlate this with outcome in terms of ocular motility. A small subset of relatively undisplaced floor fractures with tight entrapment of tissues occurring most commonly in young patients has been identified, and early surgical intervention has been recommended for these. The range of alloplastic materials av…

Orbital myiasis: a case report.

Myiasis; a term first introduced by HOPE refers to invasion of living tissue of humans and other mammals by the eggs or larvae of flies of the order of Diptera. Orbital involvement occurs in approximately 5% of all cases of myiasis. A 72 year old patient is presented with orbital myiasis that was successfully treated by enucleation and grafted with split thickness of skin.

Non-traumatic orbital haemorrhage.

PURPOSE: To establish the incidence of underlying orbital vascular anomalies, the presence of systemic associations and predisposing factors, the natural history and appropriate management of patients with non-traumatic orbital haemorrhage presenting in an orbital clinic. METHODS: The records of 115 patients with a diagnosis of non-traumatic orbital haemorrhage were reviewed with regard to clinical findings, investigations, management and outcome. RESULTS: Associated orbital vascular malformations were present in 104 patients (90%). Thirteen (11%) had additional or other predisposing factors…

Intracranial penetrating orbital injury.

The authors report a case of double-penetrating injury of the globe with intracranial involvement from a pellet gun. A 16-year-old boy had a … case demonstrates the potential for significant visual recovery in some patients with a penetrating orbital injury and intracranial involvement. Complete radiographic evaluation with neurosurgical consultation is important in the management of these patients prior to ophthalmologic intervention with possible foreign body removal. There is a need for mor …

Orbital abscess masquerading as a rhabdomyosarcoma.

Although orbital cellulitis is the most common cause of acute-onset proptosis with inflammatory signs in a child, the clinician should always be alert to the possibility of rhabdomyosarcoma. We describe an unusual presentation of acute-onset nonaxial proptosis of the left orbit without sinus disease or systemic toxicity in a 6-year-old boy. Our clinical differential diagnosis included orbital cellulitis, metastatic disease, capillary haemangioma, lymphangioma with cyst, ruptured dermoid cyst, and orbital rhabdomyosarcoma. Only after orbital biopsy and subsequent microbiologic confirmation were…

Sino-orbital aspergillosis in a dog.

Fungal rhinitis-sinusitis with orbital involvement was diagnosed in a dog with profuse unilateral ocular and bilateral nasal discharge, enophthalmos, and a corneal descemetocele. The descemetocele was treated with a conjunctival pedicle graft. Frontal sinusotomy was performed, and clotrimazole was infused through catheters placed in the frontal sinuses and nasal cavities to saturate the tissues for 1 hour. Successful resolution of orbital infection may have been aided by a fistula between the frontal sinus and orbit that allowed delivery of clotrimazole to the orbit. The dog retained sight in …

Unusual orbital foreign bodies.

PURPOSE: To describe the clinical features of patients with unusual orbital foreign bodies. METHODS: The clinical histories, preoperative photographs, imaging studies, and surgical pathologic findings of five patients with unusual orbital foreign bodies are presented. Additionally, published reports pertaining to similar cases were reviewed. RESULTS: The five patients treated by the authors had good outcomes. CONCLUSIONS: An orbital foreign body may be overlooked because a small penetrating wound may be accompanied by minimal or no signs of inflammation early in the clinical course. A caref…

Management of complex orbital fractures.

Fractures of the orbit place the globe and associated structures at significant risk. Management of these fractures requires a thorough ophthalmic evaluation and precise imaging. Contemporary surgical approaches allow the surgeon to repair these injuries with minimal secondary cosmetic deformities and should avoid injury to the globe. A principle goal of therapy is to anatomically reduce fracture segments and to restore a normal orbital volume. Reconstitution of a normal orbital volume and support for the globe will prevent post-injury enophthalmos and globe malposition. The evaluation, imagin…

Growing fracture of the orbital roof.

BACKGROUND: Growing fractures rarely arise in the skull base, and their pathogenesis and treatment are still debated. METHODS: The clinical and radiologic findings of a growing fracture involving the orbital roof in a 5-year-old boy are presented and the relevant literature is reviewed. RESULTS: The clinical picture of growing fracture of the orbital roof is dominated by ocular symptoms such as diplopia, eyelid swelling, and displaced eye globe. Computed tomography scan is excellent for demonstrating the bony defect in the orbital roof while magnetic resonance imaging is more sensitive in sh…

Albendazole therapy in orbital cysticercosis.

A patient with orbital cysticercosis is being presented, where albendazole therapy was successfully used. Serial CT scans of the lesions showed definite signs of resolution. The role of albendazole therapy in cysticercosis has been discussed. The relevant literature has been reviewed in brief.

Orbital fat prolapse in the dog.

Five dogs presented with varying degrees of subconjunctival swelling and enophthalmos. Biopsy revealed the subconjunctival tissue to be fat. Surgical excision in two cases resulted in resolution of the clinical signs and demonstrated that the fat came from the retrobulbar region.

Vascularized fronto-orbital advancement.

Fronto-orbital advancement is the procedure of choice for correction of fronto-orbital recession resulting from coronal synostosis in children. However, long-term follow-up evaluations often reveal "undercorrection" of the deformity with development of recurrent recession of lateral supraorbital regions. One factor that may contribute to this process is devascularization of the fronto-orbital bar during its reshaping and advancement. If subsequent revascularization is suboptimal, excessive resorption or inadequate growth of the bar may lead to supraorbital recession. On the basis of studies of…

Classification of orbital floor fractures.

A retrospective series of orbital, axial, and coronal computed tomography scans from 24 orbital floor fractures was studied to define the anatomic location of the fracture. Floor fractures without rim involvement, which are referred to clinically as blowout fractures, were located medial to the infraorbital nerve or extended on both sides of the nerve. Floor fractures with rim involvement were associated with zygomatico-orbital or Le Fort II or III fractures, and were located either lateral to or on both sides of the infraorbital nerve. No blowout fractures were confined to the lateral half of…

Alloplastic materials for orbital surgery.

Alloplastic materials are used as an adjunct to many types of ophthalmic surgery. This article reviews recent advancements in the use of alloplastic materials for orbital surgery, with emphasis on anophthalmic socket and bony orbital reconstruction. Several new porous materials and resorbable implants offer exciting prospects in the management of anophthalmos and orbital fractures.
