The incisor relation and craniofacial morphology, as evaluated from lateral cephalograms, were studied in 14 women and 13 men with osteogenesis imperfecta. A negative overjet was found in one woman and five men, and for the total material a mesial basal jaw relationship was observed. Data for both women and men indicated that skeletal discrepancies had been partially compensated for by the incisor inclination. Probably, persons with osteogenesis imperfecta may be regarded as a potential high-risk group for development of malocclusion.

A pregnancy at risk for a severe deforming type of osteogenesis imperfecta (OI) was monitored by ultrasonography and radiography. Long bone measurements were normal at 15 1/2 weeks gestation, but ultrasound detected an abnormality of one femur that appeared to be a fracture. The ultrasound study at 19 weeks revealed severe shortness of the femora, and radiographs showed only vertebral bodies with no other fetal skeletal parts clearly visible. Surgical stabilization of the scoliotic spine in osteogenesis imperfecta (OI) is technically difficult owing to the mechanical weakness of the bone. Brittle bone makes instrumentation of the spine a procedure all too often associated with complications. Combining the instrumentation of the OI spine, both anteriorly and posteriorly, with the use of methylmethacrylate to augment the fixation may prove valuable as a surgical technique. This usage of methylmethacrylate in correcting severe scoliosis in OI has not been previously reported.

The clinical and radiographic features of 47 cases of neonatally manifest osteogenesis imperfecta were analyzed. A scoring system was devised to code the degree of skeletal changes. A score of 2.7 and more carried a prospective mortality of 88%. Scores of 2.6 and less were associated with a survival rate of 90%. A patient had recurrent pyogenic infections, chronic mucocutaneous candidiasis, and repeated long-bone fractures. Her disorder was diagnosed as hyper-IgE syndrome on the basis of clinical data, elevated levels of serum IgE, increased levels of IgE specific for Staphylococcus aureus, and impaired T-lymphocyte function. Demineralised bone implants were used for cranio-maxillofacial reconstruction and construction in 34 patients, 28 with congenital deformities and 6 with acquired defects. The patients' ages ranged from 15 months to 59 years (mean 18 years). In 33 cases, the implants were obtained from cadaver femurs; in 1 case, the implant was prepared from the patient's own skull. Sulfate (35SO4(-2)) uptake was studied in confluent skin fibroblasts from three patients with osteogenesis imperfecta "congenita," six patients with osteogenesis imperfecta "tarda," three clinically unaffected relatives of an osteogenesis imperfecta tarda patient, and four controls. Nine patients with osteogenesis imperfecta tarda and stapedial fixation (12 ears) are presented. Surgical findings as well as the immediate and long-term results of stapedectomy are described and compared with the results of previous authors. We found firmly fixed footplates in all 12 ears. Only 2 ears had thick and soft footplates. In 10 ears the stapes crura were abnormally slender and brittle, but only 1 had fractured. In 4 ears a troublesome bleeding tendency occurred during the operation. Both the immediate and the long-term hearing results were in general satisfactory.

Immunological and biochemical techniques demonstrated that the non-collagenous proteins of the organic matrix of bone were abnormal in quantity and composition in four children with osteogenesis imperfecta. Twelve families with 29 patients showing various patterns of osteogenesis imperfecta are described, along with the genetic, pathological, and clinical features. No abnormal blochemical features were found. It has been shown that the best treatment is prophylactic (such as intramedullary nailingy or supportive. There was no remarkable improvement from any form of medical treatment. These data are compared to those of other authors.
