Abnormal course of the corticospinal tracts in KIF5C-related encephalopathy.

KIF5C encodes a protein belonging to the kinesin family and involved in cellular transport. Variants in KIF5C were first associated a decade ago with microcephaly and malformations of cortical development, with a phenotypic spectrum ranging from polymicrogyria to pachygyria. Currently, eight patients have been reported so far. Here we describe a new paediatric patient carrying the recurrent p.(Glu237Lys) KIF5C variant associated with a distinctive neuroradiological pattern of abnormal posterior course of the corticospinal tract at the level of the pons with a thickened anterior component of th…

Autoimmune GFAP astrocytopathy presenting with remarkable CNS hyperexcitability and oculogyric crises.

The autoimmune GFAP astrocytopathy has been associated with meningoencephalomyelitis that usually responds to glucocorticoids. We report a 20-year-old man that developed an acute and severe meningoencephalomyelitis with remarkable CNS hyperexcitability and oculogyric crises. CSF analysis showed hypoglycorrhachia, pleocytosis, elevated ADA, and CSF-immunofluorescence characteristic of autoimmune GFAP astrocytopathy. MRI showed lesions at thalamus, corpus-callosum, dorsal pons and dentate nucleus with associated myelitis. Immunotherapy led to a full recovery, although MRI activity was observed a…

Vertebrobasilar dolichoectasia presenting simultaneously with ischemic and hemorrhagic strokes.

BACKGROUND: Vertebrobasilar dolichoectasia (VBDE) can present with brainstem- compressive symptoms, cerebral ischemia, or hemorrhage. Herein we describe a unique patient with VBDE who presented with hemorrhage and cerebral ischemia simultaneously. CASE: Patient is a 76-year-old man presenting with an acute, symptomatic left pontine ischemic stroke heralded by ataxic dysarthria and right ataxic-hemiparesis. MRI scan of the brain demonstrated acute cerebral ischemia in the left pons, a small right thalamic hemorrhage, and evidence of VBDE. CONCLUSION: The perforating vessels branching off of t…

[A case of cerebellar brainstem form of progressive multifocal leukoencephalopathy associated with idiopathic CD4+ lymphocytopenia].

A 77-year-old woman presented with a 6-month history of slowly progressive cerebellar ataxia. T2-weighted MRI showed high signal intensity in the left upper dorsal pons and bilateral middle cerebellar peduncles. JC virus (JCV) DNA was detected in cerebrospinal fluid (CSF). The patient had no HIV infection, collagen disease, or a history of immunosuppressive treatment, but she was found to have CD4+ lymphocytopenia. We made a diagnosis of cerebellar brainstem form of progressive multifocal leukoencephalopathy (PML) presenting as cerebellar ataxia, which was presumably associated with idiopathic…

[About a note published in 1926 on a BCG vaccine trial in leprosy treatment].

By 1926, two French physicians working in Indochina, R. Pons and L. Chastel, have suggested to treat leprosy by subcutaneous injections of bacillus Calmette-Guerin. This treatment appeared efficient since the lepromatous lesions quickly regressed and the Hansen bacillus disappeared from the nasal mucus. These results were quickly confirmed by other authors. They were all more appreciated than, at this time, the Chaulmoogra oil and its products, the only available drugs, were poorly efficient against the disease. Nevertheless, although not a panacea, the BCG therapy allowed to relieve these unf…

WU and KI polyomaviruses in the brains of HIV-positive patients with and without progressive multifocal leukoencephalopathy.

The polyomaviruses KI (KIPyV) and WU (WUPyV) were recently identified mainly in respiratory samples from children and immunosuppressed patie … ns of the central nervous system that were analyzed, that is, the cerebral hemispheres, cerebellum, pons, and medulla oblongata. No specific histopathological findings were found to be associated with the presence of WUPyV and KIPyV.

Oromandibular dystonia and persistent psychiatric symptoms in extra-pontine myelinolysis.

A 32 years female presented with gradually progressive dysarthria, dysphagia, oromandibular dystonia and mild generalized weakness. She had several episodes of acute psychotic behavior. She had abnormal saccadic eye movements, generalized hypertonia and exaggerated jerks in upper limbs. She was previously treated in a peripheral hospital for severe vomiting and diarrhea. MRI of brain revealed symmetrical T-2 weighted hyperintensities in bilateral putaminal and caudate region along with pons and midbrain suggesting demyelination due to a metabolic insult. Her power improved gradually over days …

Cerebral listeriosis in an adult Freiberger gelding.

Listeria monocytogenes infection, although rare in horses, can lead to septicaemia, gastroenteritis and abortion or stillborn foals. Equine cerebral listeriosis has been reported, but only in newborn animals. This report describes a Freiberger gelding with severe neuronal symptoms of sudden onset. The animal collapsed within 24 h and was humanely killed. Necropsy revealed multiple small brown to reddish foci within the brain stem and pons. Histopathology demonstrated multifocal suppurative meningoencephalitis with microabscesses and occasional intra-lesional, coccoid to rod-shaped, bacteria. T…

Serial MRI findings in a relapsing-remitting form of neuro-Behçet's disease: a case report.

We describe a case of neuro-Behcet's disease (NBD) characterized by recurrent attacks of neurologic deficit. T2-weighted images showed a high signal intensity lesion with extensive edema in the right thalamolenticular region, midbrain, and pons as well as the cerebral white matter. After a relapse of the disease, MRI demonstrated a high signal intensity in the left thalamus, internal capsule, and midbrain. These MRI abnormalities showed marked resolution with steroid treatment. We observed sequential MRI findings in a patient with a relapsing-remitting form of NBD who had parenchymal CNS invol…

[Clinical course and evolution of lesion in MRI in central pontine myelinolysis in a patient abusing alcohol].

The authors report a 44-year-old patient with at least a 10-year history of alcohol abuse in whom an organic brain syndrome developed with cerebellar ataxia, left-side hemiparesis and bulbar symptoms. MR imaging of the head demonstrated a large hyperintense focus in T2-weighted images involving the central part of the pons. Central pontine myelinolysis was diagnosed. Laboratory investigations showed no hyponatraemia. After withdrawal treatment and motor function rehabilitation the neurological symptoms regressed nearly completely. After 18 months control MR imaging demonstrated considerable re…

[Asymmetric crying facies syndrome: report of 16 clinical cases].

The Asymmetric Crying Facies (ACF) is included among the congenital facial palsy in childhood. We report the clinical characteristics, congenital anomalies and other associated disorders in sixteen patients with ACF examined from 1984 to 1995 in the pediatric units of the Hospital Adolfo Pons, Universitario and Clínico of Maracaibo, Venezuela. Statistically the side of lesion and the sex were independent. In seven of sixteen patients (43.75%) we found congenital anomalies and/or neurological disorders, being diaphragmatic hernia and the hyperkinesia a rare association no reported up to now in …

Lyme neuroborreliosis masquerading as a brainstem tumor in a 15-year-old.

A 15-year-old boy had onset of unilateral facial weakness. A few days later, he experienced mild vertigo, double vision, and headache. Examination confirmed a peripheral right seventh nerve weakness in addition to an internuclear ophthalmoplegia. The neurologic features suggested a pontine glioma. A T2-weighted MRI scan revealed demyelinating lesions in the pons and in several areas of the cerebrum, including the periventricular region. Subsequent history revealed that he had been diagnosed with Lyme arthritis 7 years earlier while living in Connecticut. The radiographic studies favored a diag…

Distribution of leptomeningeal glioneuronal heterotopia in alobar holoprosencephaly.

OBJECTIVE: To clarify the pathogenetic significance of the topographic distribution of leptomeningeal glioneuronal heterotopia, a common fin … neuronal heterotopia showed an identical distribution, ranging from the basal prosencephalon to the pons, with the thickest distribution occurring in the basal prosencephalon. CONCLUSION: The constant localization implicates leptomeningeal glioneuronal heterotopia in severe dysgenesis of midline prosencephalon, the basic pathogenesis of the anomaly.

Reversible white matter lesions in a patient with fulminant hepatitis and acute renal failure.

A 36-year-old man developed fulminant hepatitis and acute renal failure with profound hyponatremia (116 mEq/L). Emergent hemodialysis corrected the serum sodium to 136 mEq/L within 24 hours. He developed generalized convulsions 11 days later. Magnetic resonance imaging (MRI) revealed a single large symmetrical lesion in the pons and extensive white matter lesions in the bilateral occipital, temporal, parietal and right frontal regions. These lesions showed marked resolution as the patient recovered. Fulminant hepatitis and acute renal failure could induce extensive edema in the cerebral white …

Corticosteroid-responsive parkinsonism associated with primary Sjögren's syndrome.

A 74-year-old woman with primary Sjögren's syndrome confirmed by salivary gland biopsy presented with parkinsonism. Magnetic resonance imaging (MRI) of the brain revealed multiple small high intensity lesions in the deep white matter, basal ganglia and pons on T2-weighted images. Treatment with L-dopa failed to improve the parkinsonian features. After the initiation of prednisolone 30 mg/day, the parkinsonian signs and symptoms significantly improved. Some lesions on MRI were decreased in size after corticosteroid therapy. These findings suggest that parkinsonism associated with primary Sjögre…

5-Hydroxyindoleacetic acid and 5-hydroxytryptophol levels in rat brain: effects of ethanol, pyrazole, cyanamide and disulfiram treatment.

The two serotonin metabolites 5-hydroxyindoleacetic acid (5HIAA) and 5-hydroxytryptophol (5HTOL) were measured in two regions of rat brain (pons medulla and diencephalon) using a gas chromatographic-mass spectrometric (GC-MS) method. Acute ethanol intoxication effected an elevation of 5-hydroxytryptophol levels, while 1 week of treatment with ethanol appeared to have no effect on either metabolite when measured 24 h after the last dose. Disulfiram and cyanamide treatment produced an approximately 2-fold increase in 5-hydroxytryptophol and a slight reduction in 5-hydroxyindole-acetic acid. Pyra…

[Two autopsy cases of adult central nervous system toxoplasmosis: light and electron microscopic studies (author's transl)].

Two autopsy cases of adult central nervous system toxoplasmosis were reported. Two cases treated with immunosuppressive agents and corticosteroids under a diagnosis of Hodgkin's disease were considered to be an acquired CNS toxoplasmosis associated with the underlying diseases. The first case, a 73 year old woman with Hodgkin's disease, showed toxoplasma lesions in the cerebrum. The second case, a 22 year old man with pulmonary tuberculosis, showed toxoplasma lesions in the cerebrum, basal ganglion, thalamus, cerebellum, pons, and medulla oblongatta. Each toxoplasma lesion showed small hemonec…

Patency of the lenticulostriate and Pontine arteries after stenting of the intracranial arteries.

Interventional neuroradiology methods have a well-established role in the treatment of many cerebrovascular diseases, including ischemic and hemorrhagic stroke, with perforating artery strokes being a complication that limits recovery after intracranial artery stenting. The aim of this study was to investigate the influence of covering the perforating artery origins with stent struts on their patency. Two geometric models of coverage of the perforating artery ostia by stent struts were developed, and calculations were made using data on origin dimensions from a collection of 3D models of twenty

Endoscopic Contralateral Occipital Interhemispheric Transtentorial Superior Transvelar Approach to Pontomesencephalic Cavernous Malformations: Cadaveric Anatomy and Clinical Illustration.

OBJECTIVE: Neurosurgical management of dorsal pontomesencephalic junction lesions is inherently complex and challenging. We introduce a novel endoscopic technique for resection of such lesions: the contralateral occipital interhemispheric transtentorial superior transvelar approach. METHODS: A meticulous dissection of 5 formalin-fixed cadaveric specimens was conducted to simulate the contralateral occipital interhemispheric transtentorial superior transvelar approach, with a focus on exposing the pontomesencephalic junction and the upper fourth ventricle. Each key anatomical landmark was thor
