Physical training maintains or improves gait ability in long-term nursing home residents: A systematic review of randomized controlled trials.

Numerous studies have reported the benefits of physical exercise in older adults. However, studies performed in long-term nursing home (LTNH) residents are scarce. A literature search was conducted to identify physical exercise intervention studies that were randomized and controlled and that assessed gait ability in older LTNH residents using both walking speed and timed up-and-go (TUG) tests simultaneously. Together, these tests have been defined under the term "gait ability"; they are widely used to screen for impaired physical function, and can predict accelerated functional decline, diffi

Zinc supplementation for the treatment of measles in children.

BACKGROUND: Measles is an important cause of childhood morbidity and mortality globally, despite increasing vaccine coverage. Zinc plays a significant role in the maintenance of normal immunological functions, therefore supplements given to zinc-deficient children will increase the availability of zinc and could reduce measles-related morbidity and mortality. This is an update of a review first published in 2015. OBJECTIVES: To assess the effects of zinc supplementation in reducing morbidity and mortality in children with measles. SEARCH METHODS: We searched CENTRAL (03 February 2017, Issue

The Schistosomiasis Clinical Trials Landscape: A Systematic Review of Antischistosomal Treatment Efficacy Studies and a Case for Sharing Individual Participant-Level Data (IPD).

BACKGROUND: Schistosomiasis control mainly relies on preventive chemotherapy with praziquantel (PZQ) distributed through mass drug administration. With a target of 260 million treatments yearly, reliably assessing and monitoring efficacy is all-important. Recommendations for treatment and control of schistosomiasis are supported by systematic reviews and meta-analyses of aggregated data, which however also point to limitations due to heterogeneity in trial design, analyses and reporting. Some such limitations could be corrected through access to individual participant-level data (IPD), which f

C-reactive protein concentrations across the mood spectrum in bipolar disorder: a systematic review and meta-analysis.

BACKGROUND: Inflammatory processes and neural-immune interactions have been implicated in the pathogenesis of psychiatric conditions, but studies in bipolar disorder are inconclusive so far. We aimed to investigate whether peripheral concentrations of C-reactive protein (CRP), an acute-phase response protein of inflammatory activity, are increased in bipolar disorder across the mood spectrum. METHODS: In this systematic review and meta-analysis, we searched MEDLINE, the Cochrane Library, Scopus, and Web of Knowledge from database inception to Aug 14, 2016, for studies that measured serum and

Applicability of Clinical Trials in an Unselected Cohort of Patients With Intracerebral Hemorrhage.

BACKGROUND AND PURPOSE: Patient selection in clinical trials on intracerebral hemorrhage (ICH) affects overall applicability of results. We estimated eligibility for completed, ongoing, and planned clinical trials in an unselected cohort of patients with ICH. METHODS: Large clinical ICH trials were identified using trial registration databases. Each trial's inclusion criteria were applied to a consecutive group of patients with ICH from the prospective hospital-based Lund Stroke Register. Survival status was obtained from the National Census Office and 90-day poor functional outcome (modified

Noninvasive ventilation for acute lung injury a meta-analysis of randomized controlled trials.

PURPOSE: To compare the effect of noninvasive ventilation (NIV) and standard oxygen therapy on treating acute lung injury (ALI). METHODS: A search on PubMed, Embase, Springer, Cochrane Central Register of Controlled Trials and Clinical Trials was carried out up to Nov 2015 for randomized controlled trials (RCTs) with NIV as cases and standard oxygen therapy as controls. Risk ratios and weight mean difference were used for estimation. RESULTS: This meta-analysis included seventeen RCTs. Results showed NIV significantly reduced the intubation rate, length of ICU stay and hospital mortality. Th

Psychological therapies for panic disorder with or without agoraphobia in adults: a network meta-analysis.

BACKGROUND: Panic disorder is characterised by the presence of recurrent unexpected panic attacks, discrete periods of fear or anxiety that have a rapid onset and include symptoms such as racing heart, chest pain, sweating and shaking. Panic disorder is common in the general population, with a lifetime prevalence of 1% to 4%. A previous Cochrane meta-analysis suggested that psychological therapy (either alone or combined with pharmacotherapy) can be chosen as a first-line treatment for panic disorder with or without agoraphobia. However, it is not yet clear whether certain psychological therap

Cardiac Rehabilitation in Patients with Established Atherosclerotic Vascular Disease: New Directions in the Era of Value-Based Healthcare.

Exercise-based cardiac rehabilitation (CR) is associated with significant improvements in coronary disease outcomes, but has been underutilized. However, new developments within the field, some spurred by healthcare reform and the transition to more accountable and coordinated care, offer hope for closing the large CR treatment gap. This review presents new CR-related research, policy, and analyses, and discusses how evolving eligibility criteria, referral processes, performance measures, care models, and delivery and payment options could increase CR utilization over the next decade and enabl

Meta-analysis of randomized controlled trials on the efficacy and safety of ondansetron in preventing postanesthesia shivering.

OBJECTIVES: Considerable controversy exists regarding the efficacy of ondansetron in preventing postanesthesia shivering (PAS). We performed a meta-analysis of randomized controlled trials to examine the controversy. MATERIALS AND METHODS: Randomized controlled trials assessing the effect of ondansetron on the prevention of PAS were identified from electronic databases (PubMed and EMBASE). The meta-analysis was performed with the fixed-effect model or random-effect model according to heterogeneity. RESULTS: Twelve trials randomized clinical trials met the inclusion criteria including 1205 su

Realizing Informed Consent in Times of Controversy: Lessons from the SUPPORT Study.

This Essay examines the elegantly simple idea that consent to medical treatment or participation in human research must be "informed" to be valid. It does so by using as a case study the controversial clinical research trial known as the Surfactant, Positive Pressure, and Oxygenation Randomized Trial ("SUPPORT"). The Essay begins by charting, through case law and the adoption of the common rule, the evolution of duties to secure fully informed consent in both research and treatment. The Essay then utilizes the SUPPORT study, which sought to pinpoint the level of saturated oxygen that should be

Single incision laparoscopic appendicectomy versus conventional three-port laparoscopic appendicectomy: A systematic review and meta-analysis.

BACKGROUND: Appendicectomy is a well-established surgical procedure used in the management of acute appendicitis. The operation can be performed with minimally invasive surgery or as an open procedure. A further development in the minimally invasive appendicectomy technique has been the introduction of single incision laparoscopic surgery (SILA). AIM: To ascertain any differences in outcomes from available trials comparing SILA with conventional multi-incision laparoscopic appendicectomy (CLA). METHODS: A literature search of MEDLINE/PubMed, EMBASE/Ovid and CENTRAL for articles from Jan1990

Spinal cord stimulation in refractory angina. A systematic review of randomized controlled trials.

OBJECTIVE: Spinal cord stimulation (SCS) is an invasive method used for treating patients with refractory angina. Although SCS is recommended by the European Society of Cardiology and American Heart Association (class lib, level of evidence B and C, respectively), the method is used very rarely and data on its effectiveness is limited. The aim of this work was to perform a systematic review of all randomized controlled trials (RCTs) that investigated the effectiveness of SCS as a treatment for refractory angina. METHODS: Medline and Cochrane library databases were searched from their inceptio

Effect of physical activity on sex hormones in women: a systematic review and meta-analysis of randomized controlled trials.

INTRODUCTION: Exposure to high levels of endogenous estrogens is a main risk factor for breast cancer in women, and in observational studies was found to be inversely associated with physical activity. The objective of the present study is to determine the effect of physical activity interventions on sex hormone levels in healthy women. METHODS: Electronic databases (MEDLINE, EMBASE, CENTRAL), from inception to December 2014, and reference lists of relevant reviews and clinical trials were searched, with no language restrictions applied. Randomized controlled trials (RCTs) were included if th

Evaluation of vaccine seroresponse rates and adverse event rates through Bayesian and frequentist methods.

In the evaluation of vaccine seroresponse rates and adverse reaction rates, extreme test results often occur, with substantial adverse event rates of 0% and/or seroresponse rates of 100%, which has produced several data challenges. Few studies have used both the Bayesian and frequentist methods on the same sets of data that contain extreme test cases to evaluate vaccine safety and immunogenicity. In this study, Bayesian methods were introduced, and the comparison with frequentist methods was made based on practical cases from randomized controlled vaccine trials and a simulation experiment to

Efficacy and safety of acupuncture in children: an overview of systematic reviews.

In recent years, acupuncture has increasingly being integrated into pediatric health care. It was used on ~150,000 children (0.2%). We aim to update the evidence for the efficacy and safety of acupuncture for children and evaluate the methodological qualities of these studies to improve future research in this area. We included 24 systematic reviews, comprising 142 randomized controlled trials (RCTs) with 12,787 participants. Only 25% (6/24) reviews were considered to be high quality (10.00 ± 0.63). High-quality systematic reviews and Cochrane systematic reviews tend to yield neutral or negati

Alzheimer's disease and vascular dementia: one potentially preventable and modifiable disease? Part II: Management, prevention and future perspective.

The management of dementia and mild cognitive impairment (MCI) includes pharmacological, nonpharmacological and caregiver interventions. Acetyl-cholinesterase inhibitors and memantine have a small beneficial effect in mild-to-moderate dementia. Attention is increasingly focused on long-term measures that may prevent, delay or minimize MCI and dementia, including Mediterranean diet, exercise, early active treatment of hypercholesterolaemia hypertension, and diabetes starting in midlife and earlier. High cognitive activity and a high cognitive reserve may prevent or delay the onset of aging-rela
