[Endoscopic Resection for GEP-NET].

Neuroendocrine tumours (NET) are rare tumours of the gastrointestinal tract. Many of these are diagnosed incidentally during routine upper and lower GI endoscopy. Complete surgical resection is the treatment of choice for localised tumours. For small tumours with no associated risk factors for metastases, endoscopic resection can be performed with curative intent. This endoscopic approach is standard of care for gastric, duodenal and rectal NET. In contrast, NET of the jejunum and ileum should not be treated endoscopically, as they exhibit a high rate of metastases independent of tumour size.

Intracorporeal reinforcement with barbed suture is associated with low anastomotic leakage rates after laparoscopic low anterior resection for rectal cancer: a retrospective study.

BACKGROUND: Anastomotic leakage (AL) is one of most severe postoperative complications following low anterior resection (LAR) for rectal cancer, and has an adverse impact on postoperative recovery. The occurence of AL is associated with several factors, while few studies explored the role of intracorporeal barbed suture reinforcement in it. METHODS: Consecutive cases underwent laparoscopic LAR for rectal cancer from Mar. 2018 to Feb. 2021 in our center were retrospectively collected. Cases were classified into the intracorporeal barbed suture reinforcement group and the control group accordin

International survey among surgeons on the perioperative management of rectal cancer.

BACKGROUND: Several pivotal studies and international guidelines on the perioperative management of rectal cancer have been published. However, little is known about the current state of perioperative management of rectal cancer patients in clinical practice worldwide. METHODS: An online survey including 13 questions focusing on key topics related to the perioperative management of patients with rectal cancer was conducted among colorectal surgeons registered within the database of the Research Institute Against Digestive Cancer (IRCAD). RESULTS: A total of 535 respondents from 89 countries

Comparison of flexible endoscopy and magnetic resonance imaging in determining the tumor height in rectal cancer.

BACKGROUND: Several modalities are available for the diagnosis of rectal cancer, including conventional gold standard rigid endoscopy and recent flexible endoscopy and magnetic resonance imaging (MRI). Each modality affects the management of these patients. AIM: To compare the accuracy of flexible endoscopy and MRI in the measurement of tumor height in patients with rectal cancer. METHODS AND RESULTS: This study included 174 patients with rectal cancer who underwent flexible endoscopy and MRI for the measurement of tumor height. Data on patient demographics, comorbidities, treatment, and his

Anatomical features of inferior mesenteric and left colic arteries and surgery in colorectal cancer patients with persistent descending mesocolon.

BACKGROUND: Persistent descending mesocolon (PDM) is a fetal abnormality in which the left-sided colon is not fused to the retroperitoneum, and it is often accompanied by the adhesion between the mesocolon and small bowel mesentery. Due to its rarity, whether PDM exhibits anatomical characteristics of the inferior mesenteric artery (IMA) and left colic artery (LCA), and how the anomaly affects laparoscopic surgery are largely unknown. We investigated the branches of these arteries and outcomes of patients who underwent laparoscopic surgery. METHODS: Based on computed tomography (CT) and three

Implementation of robot-assisted curative resection for rare anorectal tumours on the basis of individualised treatment.

PURPOSE: To evaluate the validity of robot-assisted curative operation for rare anorectal tumours, characterised by biological heterogeneity and anatomical complexity. METHODS: The present study evaluated 16 consecutive patients including three with anorectal squamous cell carcinoma (ARSCC), four with anorectal mucosal melanoma (ARMM), seven with anorectal neuroendocrine tumour (ARNET), and two with other types of anorectal tumours. RESULTS: Of the three patients with ARSCC after chemoradiotherapy, two underwent abdominoperineal resection (APR), and one underwent ultralow anterior resection

Bone metaplasia in a rectal polyp.

A 77-year-old male underwent a colonoscopy because of a positive fecal occult blood test. A polyp was removed from the rectum, 12 cm from the anal margin, with a hyperplastic appearance, covered by a cap of whitish fibrinoid exudate. The pathological report reported a hyperplastic polyp with foci of bone metaplasia in the lamina propria.

Electro-thermal bipolar vessel sealing versus clipping of the inferior mesenteric vessels during minimally invasive proctectomy.

INTRODUCTION: The introduction of new energy vessel sealing devices in minimally invasive proctectomy led to better hemostatic effect, less blood loss, and shorter operating time. At present, the available evidence from literature about the use of electro-thermal bipolar vessel sealers (EBVS) in laparoscopic rectal cancer surgery is weak where most studies are retrospective with non-homogenous patient groups. METHODS: This is a retrospective cohort study where 40 rectal cancer patients operated by laparoscopic TME or laparoscopic assisted transanal total mesorectal excision were classified in

Sigmoid colon cancer presenting as complete prolapse.

BACKROUND: The association of rectal prolapse and colorectal cancer is quite rare and only a few cases have been reported previously in the literature. It is unclear whether colorectal cancer triggers rectal prolapse. CASE PRESENTATION: A 77-year-old male patient presented to our emergency department with complete rectal prolapse, and an anterior resection was performed after rectal digital examination revealed a mass. The pathology result came back as mucinous adenocarcinoma in the sigmoid colon and the postoperative period was uneventful. CONCLUSIONS: Considering the age group in which rec

Rectal adenocarcinoma coexisting with incidentally found microscopic gastrointestinal stromal tumor: A case report.

RATIONALE: Adenocarcinoma coexists with adjacent microscopic gastrointestinal stromal tumor (micro-GIST) is rare, especially in the rectum, where the gastrointestinal stromal tumors (GISTs) have the lower incidence rate. It is easy to ignore the concurrent micro-GIST due to the untypical symptoms. PATIENT CONCERNS: A 77-year-old male patient suffered from lower abdominal pain for 20 days and presented with per rectal bleeding for 10 days. He had the medical history of hypertension and diabetes for more than 25 years. DIAGNOSES: Endoscopy revealed that the patient had rectum adenocarcinoma an

Modified double-barrelled wet colostomy after total pelvic exenteration.

Pelvic exenteration is a radical surgery that can require urinary and faecal diversions. Double-barreled wet colostomy, a viable alternative to the traditional ileal conduit with separate colostomy technique, is not always applicable due to the shortness of an ureter. To overcome this problem, I modified the original technique, replacing the two uretero-colic anastomoses with an ureteroureterostomy and an uretero-colic anastomosis.
