Organizational risk management and nurses' perceptions of workplace risk associated with sharps including needlestick injuries in nurses in New South Wales, Australia.

This study aimed to determine nurse reported organizational risk management and nurses' perceptions of workplace risk associated with sharps-related injuries. A cross-sectional survey was carried out on a sample of nurses from the New South Wales Nurses' Association, Australia in 2007 (n =7423), and there were 1301 eligible participants. Overall, 73% participants reported that organizational policies were followed in the event of a "sharps including needlestick" injury. Participants reported working in sharps safety oriented organizations, routine hepatitis B vaccination, sharps disposal conta…

A review of quantitative risk-benefit methodologies for assessing drug safety and efficacy-report of the ISPOR risk-benefit management working group.

OBJECTIVE: Although regulatory authorities evaluate the risks and benefits of any new drug therapy during the new drug-approval process, quantitative risk-benefit assessment (RBA) is not typically performed, nor is it presented in a consistent and integrated framework when it is used. Our purpose is to identify and describe published quantitative RBA methods for pharmaceuticals. METHODS: Using MEDLINE and other Internet-based search engines, a systematic literature review was performed to identify quantitative methodologies for RBA. These distinct RBA approaches were summarized to highlight t

Artificial-intelligence-driven governance: addressing emerging risks with a comprehensive risk-prevention-centred model for public health crisis management.

BACKGROUND: In response to the coronavirus disease 2019 (COVID-19) pandemic, an emerging public health crisis with global impact, various artificial intelligence (AI)-enabled devices for pandemic-prevention emerged, highlighting the urgent need to understand public leverage of AI-enabled digital technologies. METHODS: This study constructs a comprehensive model, the Risk Prevention-centred and AI-enabled Anti-pandemic Technology Acceptance Model (RPAA-TAM), to elucidate public adoption of anti-pandemic digital tools, contributing to innovative governance. Integrating TAM, social influence the

[Clinical management of high-risk non-muscle-invasive bladder cancer during BCG shortage: what are the options, what are the risks?].

High-risk NMIBC is challenging since, although not being muscle-invasive, it is characterised by a high propensity for progression. It is important to determine the prognostic group, which should be done by means of nomograms. Following thorough transurethral resection, adjuvant therapy with BCG is a standard treatment, but BCG is partly unavailable at present due to shortage. Hyperthermic chemotherapy has also been studied, but its broad use is not yet justified based on the data currently available, which also holds true for checkpoint blockade inhibition. Therefore, given the high propensit

French clinical practice guidelines on the management of patients on vitamin K antagonists in at-risk situations (overdose, risk of bleeding, and active bleeding).

The present report from several French medical societies in the field and the French National Authority for Health provides an expert consensus for the management of patients on vitamin K antagonists in at-risk situations (overdose, risk of bleeding, and active bleeding). Asymptomatic VKA overdose is defined as an International Normalized Ratio (INR) value above the upper limit of the therapeutic target. In this case, the guidelines describe the rapid reduction of the INR down to the therapeutic range, either by omitting a dose or using vitamin K. Regarding the haemorrhagic complications, the

A qualitative exploration of perceived risks, motivations, and risk management strategies among brothel-based female sex workers in Ibadan, Nigeria.

BACKGROUND: Female sex workers (FSWs) experience a higher burden of Human Immunodeficiency Virus (HIV), and other sexually transmitted infections (STIs) compared to the general population due to the nature of their work. In Nigeria, challenges such as poverty, stigma, and poor access to healthcare continue to heighten their vulnerability, even though prevention programmes are in place. However, limited qualitative evidence exists on how FSWs perceive sexual risks, the motivations driving unsafe practices, and the strategies used to mitigate them. This study draws from the constructs of the Hea

Inferior vena cava filter-related thrombosis: Mechanisms, clinical features, prevention, diagnosis, and risk-stratified management.

Inferior vena cava (IVC) filters can provide mechanical protection from pulmonary embolism in selected patients who cannot receive anticoagulation, but filter-related thrombosis is an important complication. A uniform definition is lacking. Thrombus in the filter region may represent emboli captured by the filter, extension of pre-existing deep vein thrombosis, or local thrombus formation around the device. Filter-induced flow disturbance and vessel-wall responses may contribute to local thrombosis, although direct mechanistic evidence in humans remains limited. Reported occurrence varies subs

Discordance Between Fracture Risk and Bone Health Management After Kidney Transplantation: A Nationwide Cohort Study.

BACKGROUND: Bone loss and fractures are critical post-kidney transplantation (KT) complications. We evaluated age-stratified screening practices, treatment patterns, and fracture incidence in a nationwide KT cohort. METHODS: Using the Korean National Health Insurance Service database (2003-2020), we analyzed 34 758 adult KT recipients. Screening practices (bone mineral density [BMD] testing), medication continuity, and incident fractures were assessed across age groups (< 50, 50-64, ≥ 65 years). RESULTS: During a median follow-up of 8.4 years, 63.8% of recipients underwent BMD testing, but o

Management of Nondiagnostic Thyroid Fine Needle Aspiration Cytology Nodules Based on TI-RADS Risk Assessment.

OBJECTIVE: The high prevalence of thyroid nodules and the frequent occurrence of nondiagnostic (ND) results on fine-needle aspiration (FNA) necessitates malignancy risk stratification to balance cancer detection while avoiding unnecessary repeat biopsies. This study evaluated malignancy rates and management strategies for nodules with ND cytology, stratified by the American College of Radiology Thyroid Imaging Reporting and Data System (TI-RADS) score. METHODS: This retrospective study analyzed 342 consecutive patients with initial ND ultrasound-guided (USG) thyroid FNA from a high volume, tw

Obesityand Gynaecological Cancers, with a Focus on Morbid Obesity: Risk Stratification, Early Diagnosis and Management.

Obesity is a chronic, relapsing disease and a significant oncological risk factor. The correlation is most pronounced and consistent for endometrial cancer. Conversely, evidence linking obesity to ovarian cancer is less robust and varies by histotype, while the association with cervical cancer is influenced by factors related to screening, diagnosis, treatment, and survival. This review examines obesity, particularly class III (morbid) obesity, in relation to the risk of gynaecological cancer, diagnostic approaches, and management strategies. A structured narrative review of PubMed/MEDLINE, Co

Early Childhood Caries: A Family Centered Preventable Disease- Risk Determinants, Clinical Spectrum and Contemporary Management.

Early childhood caries (ECC) is encountered as a serious oral health issue, chiefly in deprived areas. ECC remains a global concern, troubling babies and pre-school-aged children across nations. The widespread nature of ECC is notable and it is reportedly found in approximately 85.0% of individuals from low socioeconomic backgrounds. Due to its origins and solutions that are developed within the family environment, ECC is often referred to as a family-centered disease. ECC is a condition that affects infants and toddlers, characterized by the presence of decayed, missing, or filled teeth in ch

Precision management of medullary thyroid carcinoma: a dynamic framework integrating biomarkers, genotyping, and risk stratification.

Medullary thyroid carcinoma (MTC) is a heterogeneous neuroendocrine malignancy in which outcomes are shaped by tumor burden, locoregional spread, and molecular context. Precision management therefore requires explicit separation of hereditary MTC driven by germline RET variants from presumed sporadic disease, and a structured integration of serum biomarkers, imaging, pathology, and genotype. This review synthesizes actionable evidence on calcitonin (Ctn) and carcinoembryonic antigen (CEA) baseline values and kinetics, universal germline RET testing, and tumor somatic profiling in advanced or p

Management and risk mitigation strategies for FLT3-ITD+ acute myeloid leukemia: clinical utilization of quizartinib.

Acute myeloid leukemia (AML) with FMS-like tyrosine kinase 3 (FLT3) mutations represent a clinically aggressive subtype of AML characterized by a poor prognosis and high frequency of relapse. Although several FLT3 inhibitors exist, they differ in their side effects, potency and clinical use. Quizartinib is a potent next generation FLT3 inhibitor with a risk of QTc-prolongation necessitating a Risk Evaluation and Mitigation Strategies (REMS) program as well as potential drug-drug interactions with strong CYP3A4 inhibitors. Here we detail two patient cases illustrating the real-world use of quiz

Interval Appendicectomy after Conservative Management of Complicated Appendicitis: Balancing Recurrence, Neoplasm Risk, and Surveillance Strategies.

BACKGROUND: Non-operative management (NOM) of complicated appendicitis is increasingly accepted, but the role of interval appendicectomy (IA) remains contentious. Contemporary evidence has shifted decision-making from recurrence risk alone toward age-linked neoplasm risk and radiological features. SUMMARY: Recurrence after successful NOM is commonly reported at 12-24% and is concentrated within the first 6 months. In adults presenting with periappendiceal abscess, appendiceal tumour prevalence rises with age, reaching approximately 5-10% in patients aged 35-39 years and 14-20% in cohorts aged
