Interaction of deletional alpha-thalassaemia with sickle cell beta-thalassaemia and its influence on foetal haemoglobin expression.

A rare association of three haemoglobin defects, viz: traits for deletion form of alpha-thalassaemia, beta-thalassaemia, and sickle cell gene, in a family of French West Indies origin, was studied both at phenotype and genotype levels. In this sickle cell beta-thalassaemia, interacting alpha-thalassaemia is shown to influence the foetal haemoglobin expression. A reverse relationship between the foetal haemoglobin level and the number of alpha genes was observed.

Color vision defects in sickle cell anemia.

The Farnsworth-Munsell 100-hue test was used to compare color vision in patients with sickle cell anemia, 20/20 visual acuity, and mild to severe peripheral retinopathy, but without posterior retinal vessel involvement, with matched normal controls. Patients with sickle cell anemia had significantly more blue-yellow and mixed color vision defects and significantly higher Farnsworth-Munsell 100-hue scores than did controls. There were no significant associations between color vision defects and the severity of peripheral retinal findings or the diameter of the foveal avascular zone. These resul

Lymphocyte subpopulations in homozygous sickle cell anaemia.

Patients with sickle cell disease have an increased tendency to develop frequent and severe pyogenic infections. Lymphocyte subpopulations were studied in 14 patients with homozygous sickle cell anaemia (SCA) using the OKT monoclonal antibody. The mean absolute lymphocyte counts observed in patients with SCA were similar to that of controls. The OKT8+ lymphocytes were significantly higher in SCA patients than in controls. The mean OKT3+ and OKT4+ lymphocytes were significantly lower in SCA patients than in controls, and there is a reversal of the normal OKT4+: OKT8+ ratio. The number of B cell

New pulmonary infiltrates in a 19 year-old with sickle cell crisis.

OBJECTIVE: Sickle cell anemia (SCA) is the most common inherited blood disorder. Sickle cell crisis is characterized by episodes of pain, chronic hemolytic anemia and severe infections, usually beginning in early childhood. Sickle cell disease primarily affects those of African descent and Hispanics of Caribbean ancestry, but the trait has also been found in those with Middle Eastern, Indian, Latin American, Native American, and Mediterranean heritage. Recent studies indicate that more than 12,500 people in England have sickle cell disorders. The acute chest syndrome is the leading cause of de

Sickle cell anemia and hematological neoplasias.

Sickle cell disease (SCD) is a congenital hemolytic anemia with various clinical findings. In some cases hematological neoplasias and some solid tumors may accompany this disease but these have rarely been reported. Here we report five cases with SCD and accompanying hematological neoplasias including lymphoblastic lymphoma, multiple myeloma and hairy cell leukemia in four cases with sickle cell trait and one case of Hodgkin' disease in sickle cell anemia. All of the cases except one had no previously diagnosed congenital hemolytic anemia and/or family history. Peripheral blood findings sugges…

Sickle-trait hemoglobin does not influence Anopheles biting rates.

Children with sickle cell trait (HbAS) are protected against severe and symptomatic Plasmodium falciparum malaria. While several within-host resistance mechanisms have been investigated, it is unknown whether this protection may be attributable in part to reductions in exposure to P. falciparum parasites via mosquito bites. In a 15-month cohort in Western Kenya, we matched mosquito bloodmeals to human hosts based on short tandem repeat (STR) genotypes to determine individual mosquito biting rates. Using a multilevel multivariable model, we assessed mosquito biting behavior with respect to huma…

Correction-spontaneous central retinal artery occlusion in hemoglobin SC disease(1).

PURPOSE: To describe a case of spontaneous central retinal artery occlusion in a young man with hemoglobin sickle cell disease.METHOD: Case report.RESULTS: A 31-year-old African-American man with a history of hemoglobin SC (sickle C) disease developed sudden painless loss of vision in the right eye. Medical history was remarkable for the recent history of a mild sickle crisis, but no other systemic illness or contributing factors. Central retinal artery occlusion was diagnosed with retinal whitening, cherry red spot, and delayed arteriovenous transit on fluorescein angiography. Over the ensuin

The possible role of the spleen in the reduction of nitroblue tetrazolium by neutrophils.

Patients with sickle cell anaemia (SCA) and individuals who have undergone splenectomy have an increased susceptibility to bacterial infections. Reports of variable capacity of reduction of nitroblue tetrazolium (NBT) by neutrophils from patients with SCA suggest that there may be a polymorphonuclear dysfunction in this disease. In order to assess the role of the spleen in this neutrophil abnormality we carried out the NBT test on neutrophils from normal or splenectomized individuals and from patients with SCA, sickle cell trait and sickle-cell-beta-thalassaemia (S-thal). Decreased NBT scores …

Traumatic hyphema in children: risk factors for complications.

OBJECTIVE: To identify risk factors associated with higher rates of ocular complications in children with traumatic hyphema. METHODS: Conse … : 20 African American, 1 Asian American, and 19 white. Five of the 20 African American children had sickle cell trait, and 1 had sickle cell anemia. The rate of secondary hemorrhage was statistically higher in the African American population (P =.05), but no statistical difference existed between the rate of secondary hemorrhage in patients with and without sickle …
