Sleep reductions associated with illicit opioid use and clinic-hour changes during opioid agonist treatment for opioid dependence: Measurement by electronic diary and actigraphy.

Sleep problems are commonly reported during opioid agonist treatment (OAT) for opioid use disorders. Inpatient studies have found both sleep disturbances and improved sleep during OAT. Illicit opioids can also disrupt sleep, but it is unclear how they affect sleep in outpatients receiving OAT. Therefore, we used electronic diary entries and actigraphy to measure sleep duration and timing in opioid-dependent participants (n = 37) treated with methadone (n = 15) or buprenorphine (n = 22). For 16 weeks, participants were assigned to attend our clinic under different operating hours in a crossover

A questionnaire-based study of sleep-wake patterns and sleep quality in a TMJ and orofacial pain clinic.

OBJECTIVE: To determine the relationships among sleep quality, perceived pain, and psychological distress among patients with TMJ and orofacial pain. METHODS: The authors examined 3276 patients with temporomandibular disorder (TMD) who visited the Orofacial Pain Clinic at Yonsei University College of Dentistry. The authors conducted a survey using the sleep-quality questionnaire and classified TMD patients into two groups based on Diagnostic Criteria for TMD. For statistical analysis, the authors calculated the correlations between pain intensity as measured using a numeric rating scale (NRS)

Pharmacological therapy for Tourette syndrome: What medicine can do and cannot do.

Tourette syndrome (TS) is a frequently observed developmental neuropsychological disorder occurring in children. The pathophysiology involves both genetic and environmental factors. In this review, clinical characteristics, pathophysiology, and treatment approaches based on the pathophysiology of TS are presented. The pathophysiology is the acceleration of developmental decrement of dopamine (DA) activity at the terminal of nigro-striatal (NS)-DA system causing DA D2 receptor up-ward regulation. Serotonergic neurons involving in development of the biphasic sleep-wake-rhythm, and locomotion may

Nonpeptide orexin type-2 receptor agonist ameliorates narcolepsy-cataplexy symptoms in mouse models.

Narcolepsy-cataplexy is a debilitating disorder of sleep/wakefulness caused by a loss of orexin-producing neurons in the lateroposterior hypothalamus. Genetic or pharmacologic orexin replacement ameliorates symptoms in mouse models of narcolepsy-cataplexy. We have recently discovered a potent, nonpeptide OX2R-selective agonist, YNT-185. This study validates the pharmacological activity of this compound in OX2R-transfected cells and in OX2R-expressing neurons in brain slice preparations. Intraperitoneal, and intracerebroventricular, administration of YNT-185 suppressed cataplexy-like episodes i

Medical and genetic differences in the adverse impact of sleep loss on performance: ethical considerations for the medical profession.

The Institute of Medicine recently concluded that-on average-medical residents make more serious medical errors and have more motor vehicle crashes when they are deprived of sleep. In the interest of public safety, society has required limitations on work hours in many other safety sensitive occupations, including transportation and nuclear power generation. Those who argue in favor of traditional extended duration resident work hours often suggest that there are inter- individual differences in response to acute sleep loss or chronic sleep deprivation, implying that physicians may be more res

Efficacy and safety of lemborexant vs placebo in treating adults with insomnia disorder: a systematic review and meta-analysis of 1976 patients.

Insomnia is a typical sleep disorder in which an individual finds it difficult to fall asleep and stay asleep resulting in poor daytime functioning and decreased health and quality of life. The orexin system, which regulates wakefulness and arousal, is often overactive in individuals with insomnia, disrupting normal sleep patterns. Lemborexant, a dual orexin receptor antagonist, works through the inhibition of the orexin system, thus facilitating increased sleep onset and maintenance of sleep. This systematic review and meta-analysis seek to determine the effectiveness and safety of lemborexan

Blood pressure in sleep disordered breathing.

AIMS: To investigate blood pressure (BP) in children with sleep disordered breathing (SDB). METHODS: BP was measured during single night polysomnography in 23 suspected SDB child patients with adenotonsillar hypertrophy, but without respiratory or heart failure, or coma. The age related changes of the observed BP were normalised to the BP index. The BP indices were examined in relation to SDB measures, such as the desaturation time (percentage of time with oxygen saturation (SaO2) <90% against the total sleep time), SaO2 nadir, apnoea-hypopnoea index (AHI), and arousal index, in addition to a

Sleep phenotypes of α-synucleinopathies and tauopathies with Parkinsonism.

INTRODUCTION: In neurodegenerative Parkinsonism, biomarkers of α-synucleinopathy (Syn) or tauopathy (Tau) are an unmet need. Rapid eye movement (REM) sleep behavior disorder (RBD) strongly indicates Syn. However, it remains unknown if sleep features other than RBD could reflect underlying neuropathology. Here we assess sleep phenotypes of Syn or Tau in neurodegenerative Parkinsonism and explore their properties as potential biomarkers. METHODS: We retrospectively analyzed polysomnography recordings from 198 patients with clinically diagnosed Parkinsonism (20 DLB, 100 PD, 45 MSA, 27 PSP, 6 CBS

Neuroinflammation, Sleep, and Circadian Rhythms.

Molecules involved in innate immunity affect sleep and circadian oscillators and vice versa. Sleep-inducing inflammatory molecules are activated by increased waking activity and pathogens. Pathologies that alter inflammatory molecules, such as traumatic brain injury, cancer, cardiovascular disease, and stroke often are associated with disturbed sleep and electroencephalogram power spectra. Moreover, sleep disorders, such as insomnia and sleep disordered breathing, are associated with increased dysregulation of inflammatory processes. Inflammatory molecules in both the central nervous system an

Cardiovascular and respiratory dynamics during normal and pathological sleep.

Sleep is an active and regulated process with restorative functions for physical and mental conditions. Based on recordings of brain waves and the analysis of characteristic patterns and waveforms it is possible to distinguish wakefulness and five sleep stages. Sleep and the sleep stages modulate autonomous nervous system functions such as body temperature, respiration, blood pressure, and heart rate. These functions consist of a sympathetic tone usually related to activation and to parasympathetic (or vagal) tone usually related to inhibition. Methods of statistical physics are used to analyz

Hypersomnia and the perception of sleep-wake states: some preliminary findings.

35 hypersomniacs (20 with obstructive sleep apnea and 15 with narcolepsy) and 15 controls estimated sleep latency during systematic trials of attempting to remain awake during the day. The error in subjective assessment of sleep latency was more variable for both patient groups than for controls. In addition, narcoleptics could not provide a determination of sleep latency or differentiate sleep-wake states on nearly 23% of all trials. Ratings on a subjective sleepiness scale did not covary with objective sleep latency for any hypersomniac. The findings suggested that patients with either sleep

Primary nocturnal enuresis as a risk factor for sleep disorders: an observational questionnaire-based multicenter study.

INTRODUCTION: Primary nocturnal enuresis (PNE) is a common problem in developmental age with an estimated overall prevalence ranging from 1.6% to 15%, and possible persistence during adolescence. There is a growing interest in the sleep habits of children affected by PNE, which is derived from the contradictory data present in clinical literature. The aim of the present study was to evaluate the presence of sleep disturbances in a population of children affected by PNE, and to identify whether PNE could be considered as a risk factor for sleep disturbances among children. MATERIALS AND METHOD

SOMAS - an open-source software for the analysis of muscle activity during sleep.

STUDY OBJECTIVES: While several algorithms exist for analyzing muscle activity during sleep, none provides information on both muscle tone and movements as open-source software. We aimed to overcome this limitation by developing SOMAS (Sleep Open-source Muscle activity Analysis System). METHODS: SOMAS processes European Data Format+ (EDF+) files with wake-sleep state and candidate leg movement annotations without online data sharing, quantifies muscle tone using the atonia index and the distribution of normalized electromyography values (DNE), and calculates leg movement indices based on the
