Trauma and medically unexplained symptoms towards an integration of cognitive and neuro-biological accounts.

Medically unexplained symptoms (MUS) are frequently associated with a history of traumatization. The first purpose of the present review paper was to investigate systematically the evidence for such relation in a subset of clinical samples with MUS presenting with functional somatization: chronic pelvic pain, irritable bowel syndrome and conversion and somatization disorder. The second purpose was to critically review three dominant models explaining the relation between trauma and MUS (i.e. dissociation, conversion and hierarchical cognitive models). The latter model in particular adequately

Are patients with somatization disorder highly suggestible?

OBJECTIVE: High suggestibility is widely regarded as an important feature of patients with medically unexplained symptoms (MUS), particularly those with multiple MUS [i.e. somatization disorder (SD)], although there are few empirical data attesting to this assumption. A study was therefore conducted to compare levels of non-hypnotic suggestibility in patients with SD and medical controls. METHOD: A modified version of the Barber Suggestibility Scale was administered to 19 patients with SD and 17 controls with an established organic dystonia. RESULTS: Patients with SD were no more suggestible

[Autonomic neuropathy in somatization disorders].

AIM: We conducted a study to investigate whether patients with somatization disorder show abnormal values in autonomic testing, especially in the central baroreceptor sensitivity. PATIENTS AND METHODS: Seventy-one patients were included. All had a diagnosis of somatization disorder (ICD-10, F45.0). Psychometric testing was performed by means of validated questionnaires (STAI, STAXI, FPI, GBB, ADS, SOMS, SCL-90-R). Autonomic regulation was analyzed by international standards using frequency spectral calculation by fast Fourier transformation. Thereby 3 different groups were detected: 12 patien

The efficacy of cognitive-behavioral therapy on the core symptoms of bulimia nervosa.

Cognitive behavioral therapy (CBT) is widely regarded as the treatment of choice for bulimia nervosa (BN), with previous reviews of the CBT outcome literature claiming an approximate 40%-50% recovery rate. Most of these reviews have focused on reductions of binge eating and purging; however, the cognitive model of BN that underlies the CBT approach identifies three additional symptoms as central to the disorder: restrictive eating, concerns with shape and weight, and self-esteem. The purpose of this review was to determine the effect of CBT on the five core symptoms of BN, particularly those n

Diagnosis of functional neurological disease.

Functional disease is a common and difficult problem in clinical neurology. A thorough appraisal should be undertaken before concluding that a patient's problem is not physically based. This article describes symptoms and signs of potential value in the diagnosis of such disorder.

Abrupt onset of severe pain at term. A case report.

This case report involves an adolescent primigravida at term who was admitted with urinary complaints to the labor and delivery unit of a medical center. Within an hour, she suddenly began screaming and complaining of severe pain running from her anterior pelvis through her vagina and up her spine. Three days of very challenging co-management of the patient, with several recurrences of acute pain, followed. Differential diagnoses that could explain this patient's symptoms are reviewed and discussed. Difficult management issues, including the stress of clinical management in the face of unident

Comprehensive management of persistent somatization: an innovative inpatient program.

Psychiatry has been largely unsuccessful in developing effective treatment strategies for patients with chronic somatization. They continue to utilize a disproportionate share of health care resources and experience personal distress and significant social and occupational dysfunction. The Psychosomatic Medicine Unit at the Clarke Institute of Psychiatry is attempting to treat these patients with a comprehensive treatment program that takes into account the multifactorial etiology of the disturbance. The unit is in a tertiary care facility and the patient population is heterogeneous with respe

Income in epidemiological research: a guide to measurement and analytical treatment with a case study on mental disorders and mortality.

BACKGROUND: Income is one of the most widely used indicators of socioeconomic position in studies of health inequalities. Despite its frequent use, no empirical-driven guidelines exist on how to operationalise and analytically handle income. In this study, we develop straightforward step-by-step graphical guidelines based on a comprehensive comparative examination. METHODS: We followed the entire population aged 30-65 residing in Denmark in 2010 (n=2 699 296) until death, diagnosis of either severe mental disorder or neurotic, stress-related somatoform or depressive disorders, respectively, o

Unexpected symptoms after concussion: Potential links to functional neurological and somatic symptom disorders.

OBJECTIVE: Reporting of unexpected symptoms after concussion might, in some people, reflect a Functional Neurological Disorder (FND), Somatic Symptom Disorder (SSD), or exaggeration (feigning). This study aimed to determine whether reporting unexpected symptoms after concussion was associated with risk factors for FND/SSD, exaggeration, or both. METHOD: Adults with persistent symptoms following concussion (N = 77; 61% women) rated the presence and severity of unexpected neurological symptoms (from the Screening for Somatoform Symptoms scale, e.g., paralysis) and somatic symptoms (from the Pat

Sustained effects of left-behind experience during childhood on mental health in Chinese university undergraduates.

Rapid industrialization and urbanization in China have resulted in labor migrants leaving children behind. For left-behind children (LBC), disrupted parental attachment may increase the risk of psychiatric morbidity in adulthood. To investigate psychopathological consequences for university students who were LBC and to estimate the effects of one or both parents being migrants, the duration of left-behind experience, and parental absence during critical periods of growth on psychiatric morbidity. We conducted an annual survey of all freshmen at a Chinese university from 2014 to 2018. The quest

Biopsychosocial correlations in patients with chronic oro-facial pain. Part II. Experiences of pain and dramatic events before the 16th year of life.

Before the sixteenth year of life, biopsychosocial influences such as self-experienced and observed accidents/illnesses, adverse medical/dental treatments, dangerous events and self-experienced or observed force seem to affect the manifestation of chronic pain in the oro-facial area in the forms of temporomandibular disorder (TMD) and psychogenic denture-intolerance (PDI)/somatoform oro-facial pain (SOP). We evaluated the occurrence of these types of events before the age of 16 years in 50 TMD patients, 55 PDI/SOP patients and 55 randomly selected pain-free control group (CG) using the medical

Teaching the psychosocial aspects of care in the clinical setting: practical recommendations.

Communication skills and the psychosocial dimensions of patient care are increasingly taught in medical schools and generalist residency programs. Evidence suggests they are not reinforced or optimally implemented in clinical training. The authors present the product of an iterative process that was part of a national faculty development program and involved both experts and generalist teachers concerning teaching psychosocial medicine while precepting medical students and residents in clinical settings. Using scientific evidence, educational theory, and experience, the authors developed recom

Emotional symptoms are secondary to the voice disorder in patients with spasmodic dysphonia.

The aims of this study were to evaluate the emotional status and life quality of the patients with spasmodic dysphonia (SD) before and after botulinum toxin treatment, and to ascertain whether SD is a somatoform disorder. Ten patients with spasmodic dysphonia were injected unilaterally into the vocal cord with botulinum toxin. Before botulinum toxin treatment, two clinician's rating scales--Hamilton Depression Rating Scale (HDRS) and Hamilton Anxiety Rating Scale (HARS), and three self-rating psychometrics--Zung's Self-Rating Depression Scale (SDS), Life Quality Scale (GHQ/QL-12), and Symptom

Change Patterns of Posttraumatic and Grief Disorders and Their Somatic Variables in Ongoing Trauma.

IMPORTANCE: This study provides insight into change patterns of posttraumatic stress disorder (PTSD), complex PTSD (CPTSD), and prolonged grief disorder (PGD) under conditions of continuous trauma. Clarification of how these disorders are associated with somatization and pain is essential for the assessment and development of integrated care models for continuous trauma-exposed populations. OBJECTIVE: To examine longitudinal change patterns of PTSD, CPTSD, and PGD and assess their associations with somatization and pain. DESIGN, SETTING, AND PARTICIPANTS: A population-based cohort study was

The role of alexithymia and maladaptive coping in long-term Trauma: Insights from the aftermath of the October 7th attacks.

The study investigates the psychological effects of alexithymia and maladaptive coping strategies on individuals exposed indirectly to trauma through media coverage. Focusing the October 7th, 2023, terrorist attacks in Israel, we explore how alexithymia characterized by difficulty in processing emotions, exacerbates trauma-related symptoms. The research examines 213 participants using the Toronto Alexithymia Scale (TAS-20) as well as trauma, somatization and coping questionnaires. Results indicate that individuals with alexithymia experience more severe and prolonged trauma symptoms, including
