Congenital bifid sternum: repair in early infancy and literature review.

Sternal clefting is an unusual congenital anomaly that should be repaired in early infancy. Early surgery is facilitated by a highly compliant bony thorax. The surgical technique is described, including the (1) removal of a wedge at the confluence of the two lateral sternal bands to allow their apposition, (2) intraoperative assessment of pulmonary compliance and central venous pressure, (3) use of bilateral pectoral flaps, and (4) resulting avoidance of major costochondral stair-step osteotomies. The suggested age of repair is 1 to 4 weeks of age.

Objectives for cleft palate repair.

Improved anesthesia and nutrition are permitting earlier safe cleft palate repair, with higher success rates. Three factors that are considered necessary to obtain satisfactory function of the soft palate for speech are (1) adequate length, (2) adequate mobility, which should include consideration of resting tension and elasticity of the levator and depressor muscles, and (3) the need for conformity of the dorsal surface to the pharyngeal wall, which this paper seeks to emphasize. These concepts are examined with reference to the literature and observations made by the author from endoscopic a

Lateral nasal proboscis.

A rare congenital anomaly of the face has been encountered and reported: lateral nasal proboscis. It is characterized by a trunk-like tube arising from an eyelid and is coupled with hypoplasia of the nose, abnormal orbit and maxilla, and ophthalmic anomalies. The pathogenesis and treatment of this disorder are discussed and the original case report is quoted and illustrated.

Techniques in blepharoplasty.

The technique for upper lid blepharoplasty is presented, which emphasizes the creation of a concave, high supratarsal fold by removal of existing preaponeurotic fat pads, excision of a strip of preseptal orbicularis fibers, and portion of the orbital septum. In additon, the skin edges are fixated to the levator aponeurosis just above the tarsus. With this technique, a pleasing result can be obtained, minimizing the amount of skin that is required to excised and introducing a safety factor in the prevention of postoperative lagophthalmos. A skin technique for lower led blepharoplasty is present

[Post-laryngectomy vocal rehabilitation with a tracheophypopharyngeal shunt (author's transl)].

Oesophageal speech is the accepted method of post-laryngectomy speech rehabilitation, but still has a high failure rate. Among the different surgical procedures that create a tracheo-oesophageal fistula the method of Komorn based on a concept of Conley appears to have the greatest theoretical advantages. In this procedure a fistula is created with its inferior opening into the oesophagus and its superior opening into the trachea. Komorn's technique has been utilized in a modified form in 12 patients and in 6, because of wound problems, useful speech was not achieved. Five patients have achieve

Taming the Tongue: The Surgical Approach to Macroglossia.

While traditionally in the realm of otorhinolaryngology or oral maxillofacial surgery, conditions involving the tongue may also be managed by plastic surgeons. The authors present an unusual case of acquired macroglossia resulting from angiotensin-converting enzyme inhibitor-induced angioedema and review the literature to discuss its surgical management from a plastic surgery perspective. A 62-year-old female suffered severe airway obstruction, respiratory arrest, and anoxic brain injury from angioedema-associated macroglossia. After tracheostomy, the patient was managed nonsurgically, with bi
