Ultrasound Features of the Proximal Hamstring Muscle-Tendon-Bone Unit.

The hamstring muscle complex is made by a group of posterior biarticular thigh muscles, originating at the ischial tuberosity, which extend the hip and flex the knee joint. Proximal hamstring injuries are frequent among athletes, commonly involving their long myotendinous junction during an eccentric contraction. In this pictorial essay, we describe the ultrasound technique to visualize the normal anatomy of the proximal hamstring muscle-tendon-bone complex and present ultrasound findings in patients with traumatic injuries and tendinopathies.

Special segment: soft tissue matrices--one form of acellular human dermis for use in tendon and ligament repairs in the foot and ankle.

Trauma to tendons and ligaments represents a significant part of orthopaedic and podiatric injuries. To improve the outcome of repair, complex or chronic injuries may require reinforcement of the primary repair with a graft. Biologic grafts currently available provide different handling, strength, and biological responses. One of the currently available grafts, an acellular human dermal membrane with specific properties, has been successfully used in the augmentation of tendon and ligament repairs for more than 6 years.

A dynamic traction splint for the management of extrinsic tendon tightness.

The dynamic traction splint designed by therapists at Walter Reed Army Medical Center is used for the management of extrinsic extensor tendon tightness commonly seen in brachial plexus injuries and traumatic soft tissue injuries of the upper extremity. The two components of the splint allow for simultaneous maximum flexion of the MCP and IP joints. This simple and economical splint provides an additional modality to any occupational therapy service involved in the management of upper extremity disorders.

Cyclic Stretch Bioreactor Enhances Tenogenic Differentiation of MSCs in Biomimicked PCL-tdECM Membranes through Dynamic Mechanical Loading.

Tendon injuries are widespread, often leading to tendinopathy due to a lack of early recognition, resulting in discomfort and reduced mobility. Despite their mechanically active nature, tendons possess limited self-healing capacity, and current clinical interventions fall short in fully regenerating the tendon structure. To address this challenge, we propose an in vitro model to study disease progression and develop an effective tissue regeneration strategy. Here, we show that an electrospun bioactive polymeric membrane comprising poly(ε-caprolactone) (PCL) and goat tendon decellularized extra…

Patellar tendon reconstruction using a medial gastrocnemius flap: review of the literature and an illustrative case report, including some technical Tipps.

Patients with chronic patellar tendon ruptures, failed primary repair, injuries with significant loss of tendon tissue or skin coverage require a complex reconstruction. Several reconstructive options are available, but in the case of a revision surgery with an associated infection, most of them seems contraindicated. The use of a vascularized gastrocnemius tendon graft to reconstruct the knee extensor apparatus, is in our opinion, the most appropriate treatment option. We will report a complex case of chronical patellar tendon rupture (after failed allograft reconstruction) in the context of

Augmentation of Patellar Tendon Repair With Autologous Semitendinosus Graft-Porto Technique.

Patellar tendon ruptures can lead to significant functional deficiency of the extensor mechanism of the knee. These injuries, because of their inherent nature and associated complications, may require a complex treatment and remains a challenge for orthopaedic surgeons. Current surgical techniques present significant complications, including patellar fracture, damage to patellar articular cartilage, and abnormal patella height. This note describes a surgical technique to provide an additional reinforcement to the patellar tendon repair with a semitendinous autograft, without the necessity to p

Chronic peroneus brevis tendon lesions.

Changes can occur in the peroneus brevis tendon following ankle injuries or sprains. A series of 14 tendon lesions is reported in the ankles of 13 patients. The duration of symptoms ranged from 8 months to 20 years. The predominant symptom in 12 ankles was lateral pain. In 11 ankles, lateral ankle instability was treated by a reconstruction with the split peroneus brevis graft, and in one ankle, by direct repair. The defects were found during harvest of the graft. One patient had previous fractures with bony impingement and one had a chronic tear of the tibialis posterior tendon with pes planu

Chronic rotator cuff injury and repair model in sheep.

BACKGROUND: Most rotator cuff surgery is performed on chronic tears. As there is no animal model in which to examine the physiology of muscle and tendon injury and repair in this setting, we developed a chronic rotator cuff injury model in sheep. METHODS: The infraspinatus tendon was released in thirty-six female sheep. Biopsy specimens were obtained from the muscle and were analyzed for fat content. The force generated by the muscle with supramaximal stimulation was recorded intraoperatively. A control group (twelve sheep) underwent an immediate tendon repair. In the remaining twenty-four sh

Ultrasound features of a pulley strain in a sport climber: a discussion based on a case report.

Ruptures of the annular pulleys of the finger flexor tendons are not common in the general population. In sport climbing, these structures can be abnormally stressed, mainly because of the so-called crimped position, an extreme flexion of the proximal interphalangeal joint, levering an abnormal tension by flexor tendons. Complete pulley tears manifest with explicit pain and an inability, but strains or minor tears might only be perceived by individuals like professional climbers, since they can bring total disability to crucial grip positions. Complete tears of one or more pulleys have already

Extensor Carpi Ulnaris Subsheath Reconstruction.

Acute extensor carpi ulnaris (ECU) subsheath injury and chronic subsheath insufficiency may result in symptomatic ECU instability at the level of the distal ulna osseous sulcus. Associated ulnocarpal (ie, triangular fibrocartilage complex) and ECU intrinsic tendinopathic changes may accompany subsheath pathologies and require concomitant treatment. Surgical treatment is indicated in refractory cases despite nonoperative treatment and may consist of repair of the torn edge of the ECU subsheath or, more frequently, reconstruction utilizing a radially based extensor retinacular sling. An ECU subs

Avulsion fracture of the hamulus from clay gunshot sport: a case report.

We report a case of hamulus (hook of hamate) fracture nonunion and secondary flexor digitorum profundus tendon rupture caused by repetitive wrist hyperextension sustained during clay shotgun shooting. The hyperextension caused avulsion of the hamulus by the pisohamate ligament. The hamulus was excised and a flexor digitorum profundus tendon transfer of the ring to small finger side to side was performed with satisfactory outcome.

Traumatic rupture of both peroneal longus and brevis tendons.

Injuries of peroneal tendons are rare. Diagnosis of traumatic rupture is often late and presents as chronic ankle instability. A case of a complete traumatic rupture of both peroneal longus and brevis tendons with acute clinical and radiological diagnosis is presented. Surgical repair was performed by direct end-to-end suture on the 4th day after trauma, with excellent functional outcome at 1-year follow-up.

Functional anatomy of the thumb sagittal band.

PURPOSE: To study the anatomy and kinematics of the thumb sagittal band (SB) and the influence of simulated injury on the stability of the extensor pollicis longus (EPL) tendon. METHODS: Twenty-four below-elbow, cadaveric amputation specimens were used. The study had 3 components: SB anatomy, SB kinematics, and assessment of SB influence on extensor stability through simulated injury by serial sectioning. RESULTS: The radial and ulnar components of the SB originated on their respective sides of the thumb metacarpophalangeal joint from a soft tissue confluence and inserted into the margins of

Simultaneous Bilateral Quadriceps Tendon Rupture Secondary to Parathyroid Carcinoma: A Case Report.

Simultaneous bilateral quadriceps tendon ruptures are a rare occurrence commonly associated with a traumatic event or systemic disease. A 31-year-old man presented with simultaneous bilateral quadriceps tendon ruptures with associated hyperparathyroidism secondary to parathyroid carcinoma. The injury occurred after the patient attempted to lift a small wooden log from the ground. We discussed the multidisciplinary management of this patient resulting in bilateral quadriceps tendon repairs, tumor resection, and oncological and endocrinological restoration. Clinical follow-up is reported at 15 y

Simultaneous Bilateral Rupture of the Triceps Tendon in a Renal Transplant Patient.

The unilateral rupture of the triceps brachii tendon is a rare lesion representing 1% of all tendon injuries. The most common causes are the result of a contraction against resistance (especially weightlifters) and direct trauma. It has also been associated with systemic diseases such as diabetes mellitus, chronic renal failure, secondary hyperparathyroidism, and use of systemic corticosteroids. Simultaneous bilateral rupture of the triceps tendons is less frequent and has been described in association with chronic metabolic disorders, especially in those patients on hemodialysis. This paper p…

Quadriceps tendinopathy: a review-part 1: epidemiology and diagnosis.

Overuse injuries of the extensor mechanism of the knee are common in both athletes and non-athletes and usually occur during activities associated with repetitive loading, stress, and knee extension. Historically, they have been labeled as Jumper's knee due to the high prevalence seen in the athletic community. In many published reports, the name "patellar tendinopathy" is used to describe this disorder of the quadriceps tendon at the patellar insertion, and the names are often used interchangeably. Numerous reports have been published describing extensor mechanism injuries in athletes, but th

A Rare Case of Triceps Brachii Injury after Electrocution.

Injuries of the triceps brachii muscle are a rare entity and mostly concern its distal tendon. These represent the least common of all muscle and tendons injuries. The most common reported causes are repeated strong physical efforts, a fall on an outstretched forearm when a sudden deceleration is put on contract triceps, or a direct trauma. High-dosed and prolonged corticosteroid therapies, repeated local steroid injections, chronic renal failure, diabetes, rheumatoid arthritis, hyperparathyroidism, and osteogenesis imperfecta are reported as systemic causes. Even rarer are lesions of muscle f

Commonly missed orthopedic problems.

When not diagnosed early and managed appropriately, common musculoskeletal injuries may result in long-term disabling conditions. Anterior cruciate ligament tears are some of the most common knee ligament injuries. Slipped capital femoral epiphysis may present with little or no hip pain, and subtle or absent physical and radiographic findings. Femoral neck stress fractures, if left untreated, may result in avascular necrosis, refractures and pseudoarthrosis. A delay in diagnosis of scaphoid fractures may cause early wrist arthrosis if nonunion results. Ulnar collateral ligament tears are a fre
