AIM: We report the comparative results of functional upper extremity surgery plans in patients with tetraplegia, including restoration of elbow extension and construction of a key grip in a "one-stage" or "two-stage" operation.
MATERIALS AND METHODS: A series of 36 patients with tetraplegia, with a total of 45 operated upper limbs, was analyzed retrospectively with a mean follow-up of 23 months (13-39 months). We evaluated the analytical strength of elbow extension using the MRC (Medical Research Council) score, the strength of the key grip (kg/F) as well as the autonomy of the patients using

We prospectively evaluated the clinical and functional outcomes of pronator teres to extensor carpi radialis brevis transfer in children with cerebral palsy. Patients were followed-up at 6 months postoperatively, and functionally assessed using the House classification, Manual Ability Classification System (MACS) and Upper Extremity Functional Index (UEFI). Fifteen children with a mean age of 8.1 years underwent tendon transfers. All patients were of Gschwind and Tonkin Grade 2 for pronation deformity; eight patients were of Zancolli's classification Group 1 and seven, Group 2 for wrist flexio…

PURPOSE OF THE STUDY: Latissimus dorsi transfer is a relatively complicated procedure subsequently requiring an active, long-term physical therapy. The surgeon's performance and a patient's effort are therefore of worth consideration. However, sutures of massive rotator cuff tears do not always produce good results. The aim of the study is to evaluate the role of latissimus dorsi transfer in the treatment of these shoulder pathologies.
MATERIAL: A total of 21 patients were evaluated. Their age at the time of surgery ranged from 48 to 63 years, with an average of 54.8 years. A minimum follow-u

The modified Zancolli arthroplasty for basal thumb arthritis is an alternative to different procedures described for this pathology. As most of the methods, it includes complete trapeziectomy. The main difference is that the interposition is made with capsule (resembling Colonna's method for the hips) and that the base of the metacarpal is stabilized in the correct place by a tendon transfer (performed with an accessory of abductor pollicis longus). The main advantage of this method is the short immobilization time (2 wk) and high percentage (95%) of complete pain-free results and a stable and…

Extensor carpi radialis longus muscle has been used in various types of procedures for corrective hand surgery and is a favored muscle for correction of finger clawing due to ulnar nerve palsy in leprosy because its removal leaves an insignificant motor deficit and gives a linear scar at the donor site. It is usually not paralyzed in leprosy. The muscle, being phasic, is easy to re-educate. The excursion of the muscle is similar to lumbrical muscles which it substitutes. Since the muscle is dorsally located, the transfer does not lose tension due to adaptive wrist flexing habit. Its tendon is

OBJECTIVE: Based on the anatomical study and clinical trial of transposition of the flexor pollicis brevis muscle (M. f. p.b.) for the reconstruction of thumb opposition, we suggested a new method for the treatment of irreparable median nerve injury which causes paralysis of the opponens pollicis and the abductor pollicis brevis muscles (M.a.p.b) and leads to loss of thumb opposition.
MATERIAL AND METHODS: Anatomical study and biomechanic analysis were performed on 20 cadaveric hands and 8 patients who had been treated and followed up on an average for 12 months.
RESULTS: The M.f.p.b. overla

After review of the pertinent literature authors report on a new tendon transfer technique not having been found in the available literature. The method has been considered suitable for the treatment of patellar dislocations due to erroneous stretching direction occurring before the end of growth. The semitendinous muscle was separated and transfosed in the tunnel drilled into the patella. By means of this technique the function of the muscle was used against the lateralization tendency of the patella. The paper presents the follow-up of 9 cases.

1. A forearm shattered by a close-range shotgun injury can be reconstructed by transposing the distal radius to the proximal ulna. 2. This surgery is considered when repair of the bones is impossible, but an effort at reconstruction is warranted as an alternative to amputation. 3. A successful forearm reconstruction not only saves the forearm, wrist, and hand, but restores a satisfactory cosmetic and functional result. 4. Because radioulnar transposition re-establishes the bony integrity, future reconstruction in the form of tendon transfers may be undertaken to improve hand and wrist function…

Techniques for anterior cruciate ligament (ACL) surgery have evolved from the traditional open procedures to arthroscopically assisted procedures. The rationale for arthroscopically assisted ACL reconstruction includes small incisions, no disruption of the extensor mechanism, shorter hospitalizations, reduced postoperative pain, and faster rehabilitation. A stepwise approach for ACL reconstructions is presented.

Insertional Achilles tendinopathy can be a painful debilitating condition that should initially be treated non-operatively. If pain becomes chronic and debilitating, despite appropriate conservative treatment, debridement of the diseased portion of the Achilles tendon and removal of the impinging calcaneal prominence and transfer of the flexor hallucis longus through a single incision can be a reliable pain relieving procedure with relatively high patient satisfaction.

BACKGROUND: For functional reconstruction of a thumb metacarpophalangeal joint defect with loss of opposition, we need to address both joint mobility and thumb opposition. Free vascularized second toe joint transplantation provides joint replacement, as well as extensor and flexor tendons for apprehensile thumb restoration. Furthermore, the extensor digitorum brevis (EDB) allows a simultaneous abductorplasty for the reconstruction of traumatized abductor pollicis brevis.
METHODS: Seven patients underwent one-stage vascularized second toe metatarsophalangeal joint transfers to thumb metacarpop

Posterior tibial tendon insufficiency is the most common cause of acquired adult flatfoot deformity. Although the exact etiology of the disorder is still unknown, the condition has been classified, on the basis of clinical and radiographic findings, into four stages. In stage I, there is no notable clinical deformity; patients usually present with pain along the course of the tendon and evidence of local inflammatory changes. Stage II is characterized by a dynamic deformity of the hindfoot. Stage III involves a fixed deformity of the hindfoot and typically also a fixed forefoot supination defo

PURPOSE: The primary treatment goals for advanced-stage thumb carpometacarpal (CMC) joint osteoarthritis are complete pain relief and restoration of thumb strength. The purpose of the present study was to introduce a variation of the abductor pollicis longus (APL) suspension arthroplasty using a single looping of a radial slip from the APL tendon around the flexor carpi radialis (FCR) tendon combined with RegJoint™ interposition and to determine its efficacy in the treatment of thumb CMC joint osteoarthritis.
METHODS: Between 2015 and 2017, 21 patients were included. The average age was 60.8

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2018eng
Surgical repair of shoulder instability is challenging, and multiple procedures have been proposed. In an attempt to reduce risk of recurrence following surgical reconstruction, some surgeons have added steps to prior arthroscopic procedures, and other surgeons have selected a bone reinforcement procedure. These additional augmented repair techniques have reduced the risk of postoperative recurrence, but introduced additional risk of complications related to hardware, fixation, and possible need for additional surgery. Surgeons should become familiar with multiple surgical procedures to treat

BACKGROUND: The clubfoot and congenital constriction band is a known association in which the clubfoot is considered as being rigid, responding poorly to casting, and requiring surgical interventions.
OBJECTIVE AND METHODS: The series describes 3 cases of clubfoot with deep ipsilateral congenital constriction bands managed with Ponseti method and immediate outcomes achieved.
RESULTS AND CONCLUSIONS: The feet responded favorably to casting although equinus correction required more extensive surgical interventions. The preexisting edema in distal limb often precluded a snuggly fitting cast inc
