Early motion programs for extensor tendon lacerations and repairs are relatively new and are proving to be extremely beneficial in the rehabilitation process. This article discusses when these programs are indicated and the details of how to provide a good early motion program.

A case of an irreducibled subcapital fracture of the fifth metacarpal bone caused by interposition of the extensor tendon to the fifth finger and the intertendineous connection between the fourth and the fifth extensor tendons is described. To reduce the fracture the surgeon had to remove interposed tissue.

A 47 years-old man presented an atraumatic, spontaneous index flexor tendons rupture. This patient has been treated two years ago for a pulmonary tuberculosis. At surgery, the flexor tendons were infiltrated by a granuloma. Histologic examination was compatible with tuberculosis sequelae. According to literature review, this is the second reported case of a tendon rupture due to tuberculosis granuloma.

Entrapment of tendons in pediatric forearm fractures has been reported rarely and often is diagnosed months to years after the initial injury. Tendon entrapments are reported to occur more frequently distally than proximally. Two pediatric cases of proximal entrapment of the flexor digitorum profundus in an ulnar shaft fracture are reported here. Early diagnosis permitted surgical release and early restoration of full function. These cases emphasize the difficulty in diagnosing this rare condition in a young child and the need for careful digital range of motion examination under ideal circums…

The effect of changes in diameter of the flexor sheath after tendon repair on tendon function was studied using the long toes of 64 white leghorn chickens. Biomechanical studies of gliding excursion, toe flexion, and ultimate load were carried out at 6 and 12 weeks after tendon repair to determine functions of the tendons healed in narrowed, directly closed, partially excised, and enlarged flexor sheath experimental groups. Histologic examination was used to evaluate the extent of adhesions, healing status of the tendons, and status of the managed sheath. At 6 weeks, gliding excursions and ran…

Large tendon rupture is a rare catastrophic occurrence in dialysis patients. Pathogenesis of this has been variably thought to be due to malnutrition, insufficient dialysis, amyloidosis, chronic acidosis, or hyperparathyroidism. We investigated contributory causes and timing of this complication in 44 dialysis patients (42 hemodialysis and two peritoneal dialysis patients). Five cases were our own; the other 39 were reported in the literature during the last two decades. Data were compared with a hospital database of 916 patients. The patients who experienced tendon ruptures had been on dialys

Fifteen cases of paralysis of the quadriceps muscles secondary to poliomyelitis were managed by hamstring tendon transfers with an average follow-up of 48 months. Results of 13.5% excellent, 73% good, and 13.5% fair were obtained, according to an objective scoring system. Significant improvement in gait pattern, relief from brace use, or "hand-on-thigh" ambulation was seen in all cases. A mean extension lag of 30 degrees was observed in patients examined in the sitting position. Lack of active terminal knee flexion, prohibiting comfortable floor sitting, was the main untoward effect and the so

A 40-year-old man with a history of previous cheilectomy and two steroid injections for first metatarsophalangeal degenerative joint disease sustained an acute rupture of the extensor hallucis longus tendon. He was treated successfully with delayed primary repair of the tendon.

Reviewing the history and etiology of extensor pollicis longus tendon rupture shows the most compelling mechanism of rupture apparently is interruption of the tendon's vascularity secondary to hemorrhage and pressure, which causes the damaged tendon to be more susceptible to rupture secondary to late ischemic necrosis and attrition. Treatment options tried have included direct repair, tendon grafting, and tendon transfer. The authors recommend the extensor indicis proprius tendon transfer as the most predictable procedure to restore the original function of the EPL. This technique can be perfo

Injury to the anterior tibial tendon, extensor hallucis longus, extensor communis tendons, or dorsalis pedis artery, vein, and nerve are known as "boot-top" injuries. A series of five such cases illustrates that the size of laceration is often small and injury to the underlying structures can be missed. This injury should be preventable if equipment is worn properly.

A congenital attenuation of the central slip results in a flexion attitude of the affected digit that can be confused with more common conditions such as congenital trigger finger or camptodactyly. It is a mild form of congenital absence of the central slip. The diagnosis is discussed. Treatment consists of limited excision of the attenuated area and suturing of the normal components of the central slip. The author has treated five cases in the last 10 years.

Rupture of the triceps mechanism is an uncommon injury that has been recognized with increasing frequency in recent years. It has been propo … as a history of anabolic steroid abuse. Both systemic steroids and local injections may predispose tendons to rupture. Triceps tendon ruptures may result in uniformly good to excellent results if recognized and treated surgically. This case also serves as a reminder of the risks of treating inflamed tissues with local steroid injections, especially in st …

The rise of recreational sports activities and the incidence of ruptures of the Achilles tendons has increased simultaneously. Recently the use of fibrin sealing for surgical therapy of ruptured Achilles tendons has emerged as an alternative technique. Between 1982 and 1989 62 consecutive patients were operated for unilateral Achilles tendon rupture. Following causes for ruptures are known: 36 indirect traumas in sport (74%), 9 direct blunt injuries at occupational work (18%), 3 traffic-accidents (11%) and 1 rupture after operation of a Haglund exostosis. 25 patients (56%) were treated by sutu…

Tophaceous deposition of calcium pyrophosphate dihydrate (CPPD) crystals is considered unusual as is deposition within tendon sheaths. Associated tendon rupture is described but is rare. We report a lady with tophaceous CPPD deposition at the wrist, a previously unreported site. Contrary to previous cases, this occurred in the setting of symptomatic, polyarticular, pyrophosphate arthropathy. An additional point of interest was development of associated extensor tendon rupture with features favouring a non-inflammatory mechanism of tendon damage.

Ankle equinus is defined as an inability to dorsiflex the foot at the ankle a minimum of 10 degrees with the knee in full extension. Numerous causes have been presented, and proper evaluation and diagnosis is critical before a surgical procedure can be selected. Gross spastic contractures will be managed differently than the more subtle forms of limitation of ankle dorsiflexion. Lastly, it should be remembered that the desired increase in range of motion will be accompanied by a decrease in strength of the posterior group, which will vary with the procedure and age of the patient.

Intratendinous ruptures of flexor tendons about the hand and wrist are rare. Flexor pollicis longus and index flexor digitorum profundus tendon ruptures, most commonly seen in patients with rheumatoid arthritis, occurred in a nonrheumatoid 48-year-old man with an asymptomatic scaphoid nonunion. This rare injury may simulate an anterior interosseous nerve syndrome. The absence of prior symptoms does not preclude tendon rupture secondary to scaphoid nonunion. Restoration of power pinch provides good function despite limited range of motion.

We investigated the frequency of patellar tendon rupture in our patients with systemic lupus erythematosus (SLE) in order to characterize the features that might predict rupture and compared our experience to that described in the literature. Four of 180 patients with lupus seen in the last 10 years had patellar tendon rupture. Duration of disease ranged between 7-20 years. Each patient was in clinical remission at the time of rupture. Each patient also had deforming arthritis of the hands. All had been taking prednisone from 7 to 15 years and all had side effects associated with prednisone th
