Fetal and perinatal consequences of maternal obesity.

In many industrialised countries, one in five women booking for antenatal care is obese. As well as affecting maternal health, maternal obesity may have important adverse consequences for fetal, neonatal and long-term health and well-being. Maternal obesity is associated with a higher risk of stillbirth, elective preterm birth and perinatal mortality. The incidence of severe birth defects, particularly neural tube and structural cardiac defects, appears to be higher in infants of obese mothers. Fetal macrosomia associated with maternal obesity and gestational diabetes predisposes infants to bi

Health problems of Micronesian patients at a student-run free homeless clinic.

BACKGROUND: As of November 2006, 41.3% of the residents at a newly established transitional homeless shelter in Hawai'i were of Micronesian descent. A student-run medical clinic operates at this shelter every week. This study examines the medical problems found in the Micronesian patients at this clinic. METHODS: A retrospective chart review of 213 patients was conducted. Data was compiled from registration forms, intake forms, and progress notes. RESULTS: The most common medical conditions that patients reported having were musculoskeletal problems, hypertension, and diabetes. For actual pa

Health issues in men: part I: Common genitourinary disorders.

Common genitourinary health issues that arise in the care of male patients include prostatitis, benign prostatic hyperplasia, urogenital cancers, premature ejaculation and erectile dysfunction. Bacterial infections are responsible for only 5 to 10 percent of prostatitis cases. Benign prostatic hyperplasia is present in 90 percent of men by the age of 85. Common urogenital cancers include prostate cancer, transitional cell carcinoma of the bladder and testicular cancer. Although an estimated 10 percent of men eventually develop prostate cancer, screening for this malignancy is one of the most c

After neonatal care, what next? A qualitative study of mothers' post-discharge experiences after premature birth in Kenya.

BACKGROUND: Approximately 15 million babies are born prematurely every year worldwide. Sub-Saharan Africa (SSA) and Asia account for more than half of the global preterm deliveries. Prominent healthcare structural and socio-economic factors in SSA, for example poverty and weak health systems, amplify vulnerabilities for mothers and premature babies; often leading to poor outcomes. Post-discharge mortality rates are high, and readmission is common. For mothers of premature babies, the transition home from hospital is marked by challenges and uncertainties. This study explored the post-discharge

High-Dose Buprenorphine Initiation to Rapidly Transition From High-Dose Methadone in the Hospital.

Transitions from high-dose methadone to buprenorphine remain challenging, and the evidence base for high-dose buprenorphine initiation (HDBI) strategies in this context is limited. We reviewed the published literature on HDBI with a particular focus on their application for patients maintained on methadone and identified no prior reports of successful transition from methadone doses as high as 180 mg. We then describe a hospitalized patient who underwent a safe and effective transition from 180 mg of methadone to buprenorphine through a high-dose initiation protocol with support of adjunctive

Making the most of the surgical internship: Optimizing preliminary year for future vascular and interventional radiologists.

The surgical internship represents a unique opportunity over that of transitional years or internal medicine preliminary years because it allows trainees to develop procedural confidence, clinical judgment, and patient-care habits that directly translate to vascular and interventional radiology (VIR) practice. Although the internship year is often viewed as a prerequisite before radiology residency, available evidence suggests that surgical preliminary training may offer advantages in procedural exposure and perceived preparedness for trainees pursuing VIR. As VIR continues to evolve into a cl

The short tenure and long legacy of interior secretary Stanley K. Hathaway.

The second half of the 20th century saw significant progress toward the destigmatization of psychiatric illness and the embrace of mental health care by the American public. Attitudes toward political leaders and candidates with psychiatric diagnoses also evolved during this period-although not at the same pace. From the 1940s through the early 1970s, such diagnoses damaged the careers of prominent officials including Secretary of Defense James Forrestal, Republican presidential candidate Barry Goldwater and Democratic vice-presidential nominee Thomas Eagleton. By the 1980s and 1990s, candidat

Outpatient Patient Controlled Analgesia (PCA) for the Palliative Care Patient.

This study explores the under-researched domain of patient-controlled analgesia (PCA) for cancer pain management in adult outpatients, focusing on the transition from patient-controlled analgesia pumps (PCA pump) to oral medications. While existing literature primarily addresses the use of PCA in inpatient settings, this descriptive study investigates the initiation of outpatient PCA in palliative care patients. The retrospective chart review includes data from all admissions between July 1, 2014, and December 31, 2020. Among the 49 identified patients, 41 were admitted for cancer-related pain

Effects of Total Enteral Nutrition on Early Growth, Immunity, and Neuronal Development of Preterm Infants.

The feeding of colostrum and mother's transitional milk improves immune protection and neurodevelopmental outcomes. It also helps with gut maturation and decreases the risks of infection. The supply of nutrients from human milk (HM) is not adequate for preterm infants, even though preterm mother's milk contains higher concentrations of protein, sodium, zinc, and calcium than mature HM. The human milk fortifiers, particularly those with protein, calcium, and phosphate, should be used to supplement HM to meet the necessities of preterm infants. The management of fluid and electrolytes is a chall

Management of the Transitional Urology Patient: the Role of the Adult Reconstructive Urologist.

PURPOSE OF REVIEW: Patients with congenital urologic conditions present unique challenges as adults. Herein, we review the literature relevant to the adult reconstructive urologist confronted with complex surgical concerns affecting their patients with a history of hypospadias, spina bifida, and other syndromes affecting the genitourinary tract. RECENT FINDINGS: Urethral stricture disease related to hypospadias is complex, but successful urethroplasty and penile curvature correction can be achieved with an anatomically minded approach. Multiple urinary diversion techniques can be considered i

Nia Imani Model of Care's Impact on Homeless African-American Women.

This qualitative study assessed the impact of the Nia Imani model of care on homeless African-American women, many of whom abused drugs, experienced interpersonal violence, and faced other challenges when seeking to improve their lives and health. Nia Imani Family, Inc., is Milwaukee, Wisconsin's, only long-term transitional living facility. Grounded in Black feminist thought, our study included focus group interviews with 39 women who had lived at Nia Imani, and successfully completed its programs; and one individual interview with the founder, who had also experienced homelessness (N=40). Establishing and developing a Teenage and Young Adult dermatology clinic with embedded specialist psychological support.

Skin conditions are common in adolescence and impart considerable psychological burden. The Department of Health has identified the specialized needs of adolescents transitioning from paediatric to adult services as a priority, yet there are few dedicated transitional clinics in the UK providing appropriate psychosocial support. We have established a monthly Teenage and Young Adult (TYA) dermatology clinic dedicated to managing teenagers and young adults with skin disease alongside open-access psychological support. Demographic data and Teenagers' Quality of Life Index (T-QoL) measures were re

User characteristics and out-of-pocket expenditures for progestin-only versus combined oral contraceptives.

BACKGROUND: Little is known about the proportion of oral contraceptive pill (OCP) users that use progestin-only pills (POPs), factors associated with POP use, and whether out-of-pocket expenditures and dispensing patterns are similar to combined oral contraceptives (COCs). STUDY DESIGN: Observational cohort using 1996-2008 Medical Expenditure Panel Surveys. RESULTS: Among all OCP users, 4% used POPs and changed little between 1996 and 2008. Women were more likely to use POPs if they received postpartum care (p<.001), had a diagnosis of hypertension (p<.001) or resided in the West (p<.01). PO

Building sustainable TB care systems: managing incentives in private sector engagement.

Private sector engagement (PSE) plays a vital role in enhancing TB care. Global Fund-supported PSE activities contributed to improved TB notifications and treatment outcomes, especially where non-financial incentives were complemented by financial incentives, delivered directly or via social health insurance. Transitioning such activities to government-led health systems is essential for long-term sustainability and to end TB. Well-designed incentive strategies could ensure quality care where patients seek services. However, payment delays, administrative burdens, and donor dependency threaten

Outpatient Total Joint Arthroplasty: Future Directions.

➢ Over the past 4 decades, total joint arthroplasty (TJA) has transitioned from primarily an inpatient procedure to a procedure increasingly performed in outpatient settings, driven by advancements in perioperative care protocols, regional anesthesia, reimbursement models, and a shift in cultural expectations. ➢ Studies have shown that outpatient TJA can be performed with similar surgical outcomes compared with inpatient TJA, provided that there is appropriate patient selection and optimization of perioperative management. Patient selection for outpatient TJA relies on risk assessment, utilizi

Clinical consensus guidelines for the management of Noonan syndrome in Japan.

Noonan syndrome (NS) is a multisystem RASopathy marked by characteristic facies, congenital heart disease, short stature, and variable neurodevelopmental and systemic complications. Although molecular and clinical knowledge has advanced, evidence-based clinical guidance in Japan has been limited. A multidisciplinary committee in pediatric endocrinology, clinical genetics, and pediatric cardiology developed consensus guidelines on diagnosis, comorbidity assessment, treatment, and transitional care. Clinical questions were defined, and the literature published through March 2024 was systematical…

Role clarity and organizational capacity as prerequisites for reducing readmissions: Comparing high- and low-performing VA facilities.

BACKGROUND: Older adults face a high risk of unplanned hospital readmissions during transitions from hospital to home. Despite system-wide initiatives, substantial variation persists across Veterans Health Administration (VHA) facilities. OBJECTIVE: To identify organizational factors and transitional care processes associated with high and low readmission rates. METHODS: We conducted a qualitatively driven explanatory sequential study of 10 VA medical centers selected based on 30-day risk-adjusted readmission rates. Ten facilities (five high-performing, five low-performing) were purposively …
