Imaging in Pott's spine: A review of typical and atypical imaging features and diagnostic challenges.

Pott's spine is associated with high morbidity, long-term disabling sequelae and even mortality. The classical paradiscal form, where there is destruction of end plates of adjacent 2-3 vertebral bodies along with intervening disc involvement and associated paravertebral abscess is well recognized. However, due to its varied clinical and radiological presentations, it remains a diagnostic challenge often resulting in delay in diagnosis. This is especially true of countries where tuberculosis has been almost eliminated but now are witnessing increase in incidence both due to immigration and HIV

A preclinical model of TB meningitis to determine drug penetration and activity at the sites of disease.

Tuberculosis meningitis (TBM) is essentially treated with the first-line regimen used against pulmonary tuberculosis, with a prolonged continuation phase. However, clinical outcomes are poor in comparison, for reasons that are only partially understood, highlighting the need for improved preclinical tools to measure drug distribution and activity at the site of disease. A predictive animal model of TBM would also be of great value to prioritize promising drug regimens to be tested in clinical trials, given the healthy state of the development pipeline for the first time in decades. Here, we re

Effectiveness of Kyphosis Reduction Using Cantilever Method in Thoracolumbar Spondylitis Tuberculosis: A Short-Term Follow-Up.

BACKGROUND: Kyphosis in spondylitis tuberculosis (STb) is more than just a cosmetic issue. It has a potentially detrimental effect on both spine-associated structures and cardiopulmonary function. It can be corrected in any stage of STb; however, the corrective surgery is challenging, especially in the late case, in which the additional stiffness of the spine can come into consideration. To date, the cantilever technique is still a gold standard for sagittal plane deformity correction. However, no study to date has explored its effectiveness for thoracolumbar kyphotic deformity, especially tha

Modified costotransverse approach combined with autologous iliac bone graft fusion and internal fixation in thoracic tuberculosis.

BACKGROUND: Due to the complexity of the thoracic spine's surrounding structures, thoracic tuberculosis surgery is challenging to perform, with difficult exposure, complex operations, numerous complications, and often unsatisfactory prognoses. Currently, it is believed that the posterior approach alone is superior to both the anterior and the combined anterior-posterior surgical methods in correcting spinal kyphosis and maintaining spinal stability. The posterior approach encompasses the transpedicular approach, costotransverse approach, and lateral thoracic approach. The aim of this study is

[Microbiological and molecular genetic characterization of Mycobacterium tuberculosis strains isolated from patients with multiorgan and generalized tuberculosis].

The paper presents the microbiological and molecular genetic characteristics of Mycobacterium tuberculosis (MBT) isolated from multiple lesion foci in 26 patients with multiorgan and generalized tuberculosis. Cultures of MBT of the family Beijing were isolated from the pathological specimens taken from 17 (65.4%) patients; those with individual genotypes were in 9 (34.6%) patients. The study indicated that cultured MBT from different types of pathological material from 25 of the 26 patients had identical spoligotypes irrespective of their clinical diagnosis. At the same time minor profile IS61

Red flags for the early detection of spinal infection in back pain patients.

BACKGROUND: Red flags are signs and symptoms that are possible indicators of serious spinal pathology. There is limited evidence or guidance on how red flags should be used in practice. Due to the lack of robust evidence for many red flags their use has been questioned. The aim was to conduct a systematic review specifically reporting on studies that evaluated the diagnostic accuracy of red flags for Spinal Infection in patients with low back pain. METHODS: Searches were carried out to identify the literature from inception to March 2019. The databases searched were Medline, CINHAL Plus, Web

Cervicothoracic Tuberculous Spondylitis in a Toddler: A Case Report.

Background and Importance Tuberculous spondylitis (or Pott's disease) is a common form of skeletal tuberculosis. Although primarily treated with tuberculostatic drugs, spinal surgery is necessary in patients with deformities or spinal cord compression. Literature describing surgery strategies in very young children remains scarce. Clinical Presentation A 20-month-old girl presented with torticollis without neurological deficits. Magnetic resonance imaging showed a cavitating mass, with destruction of the C7, T1, and T2 vertebrae. A combined anterior cervical and sternotomy approach was used to

Hartmannella vermiformis isolated from the cerebrospinal fluid of a young male patient with meningoencephalitis and bronchopneumonia.

Meningoencephalitis and bronchopneumonia were documented in a patient from Peubla, Mexico. The patient began with symptoms and signs of a common flu and 12 days after the onset of his disease he was admitted to the hospital presenting symptoms and signs of meningoencephalitis. The clinical course evolved into an endocraneal hypertension syndrome with bronchopneumonia, coma and death. Wide-spectrum antibiotics, immunosuppressive and anti-tuberculosis therapy were unsuccessfully administered. Important antecedents were degree I malnutrition and repeated contact with polluted water. Post-mortem a

Salmonella Typhi mimicking Pott's disease: A case report and review of the literature.

BACKGROUND: Infectious spondylodiscitis, an infection that involves the intervertebral disc and adjacent vertebral body, often manifests as back pain, fever, and malaise. Although Staphylococcus aureus is the most commonly reported cause of infectious spondylodiscitis, other atypical organisms can also cause spinal infections. Salmonella enterica serovar Typhi has been rarely reported to cause spondylodiscitis. METHOD: Retrospective single-record review with informed written consent from the patient. RESULTS: We present the case of an immunocompetent 67-year-old female patient with subacute

Symptomatic Tuberculous Ligamentum Flavum Cyst Treated by Full Endoscopic Resection: Review with Technical Notes.

BACKGROUND: Tuberculosis (TB) of the posterior spinal element is an uncommon condition. In a developed country its diagnosis is becoming difficult due to low incidence. CASE DESCRIPTION: A 60-year-old lady presented with low back pain and right leg pain for 6 months. On examination there was tenderness over L4 and L5, a positive straight leg raise test at 70 degrees on the right side and free on the left, and sensory involvement on the right L5 dermatome. Initial magnetic resonance imaging (MRI) showed an L4-5 ligamentum flavum cyst, high signal intensity in the right pedicle and facet joint.

Use of Intramedullary K-Wire to Enhance Fibular Graft Placement in 4-Level Thoracolumbar Corpectomy for Pott Disease: Case Report and Review of Literature.

BACKGROUND: Reports of tuberculosis involving ≥4 contiguous spinal levels are rare. Surgery is often required in this population for neurological decompression and stabilization. These patients appear to be particularly predisposed to complications that include incomplete recovery of neurological function after decompression as well as post-treatment kyphosis. In settings with limited available instrumentation, reconstruction of the anterior column can be challenging. CASE DESCRIPTION: A 35-year-old woman presented with back pain, progressive weakness in her bilateral lower extremities, and i

Neurological manifestations of HIV infection in Kwazulu-Natal South Africa.

South Africa has one of the fastest growing HIV epidemics in the world and KwaZulu-Natal, one of its nine provinces, is the epicentre of the epidemic. Of the estimated 5.3 million people infected with HIV in South Africa, 1.2 million reside in KwaZulu-Natal. Transmission of HIV is almost exclusively heterosexual, intravenous drug misuse does not occur and the patients attending state hospitals are antiretroviral drug naive. The neurological complications of HIV infection include bacterial and fungal meningitis, intracranial mass lesions, acute disseminated encephalomyelitis, a variety of spina

In vitro NMR micro imaging of the spinal cord of chronic relapsing EAE rats.

Chronic relapsing experimental allergic encephalomyelitis (Cr-EAE) was induced in Lewis rats with an emulsion of guinea pig spinal cord tissue in complete Freund's adjuvant enriched with Mycobacterium tuberculosis H37 RA. The sensitized rats developed Cr-EAE showing two to three relapses during the first 40 days. In vitro transverse T2-weighted spin echo images of the spinal cord of Cr-EAE rats, sacrificed at the clinical height (hind leg paralysis and urinary incontinence) of the third bout and their controls, were compared with the corresponding histopathology. Lesions extended over the enti

Tuberculous spondylitis. A report of six cases and a review of the literature.

Tuberculous spondylitis in the United States is a disease of decreasing incidence and, when the incidence is compared to that of developing nations, affects an older population. The thoracic and lumbar spine is most frequently involved, and in advanced disease several vertebrae may be destroyed with resulting significant morbidity and mortality. The usual clinical presentation consists of fever, back pain, and nonspecific systemic symptoms of varying duration. More advanced disease presents with neurological deficits, kyphotic deformities of the spinal column, and paravertebral cold abscesses.

Disseminated infection due to Mycobacterium chelonae with scleritis, spondylodiscitis and spinal epidural abscess.

Mycobacteria other than tuberculosis (MOTT) have a low incidence as pathogens in human pathology. The most frequent clinical expression is the disseminated disease in subjects with compromised cellular immunity. Bacteriological characteristics in culture can generate confusion with other pathogens, which delays the appropriate diagnosis and treatment. We present a case of a disseminated infection due to Mycobacterium chelonae with scleritis, spondylodiscitis and spinal epidural abscess in a man with a medical background of cellular immunity deficit induced by therapeutic drugs. The antibiotic

[Causes of disabling forms of tuberculous spondylitis].

To define the causes of disabling forms of tuberculous spondylitis, 218 patients with advanced complicated forms of tuberculosis of the vertebral column were examined. Their specific process was complicated by abscesses, fistulas, deformity, and instability of the affected portion of the vertebral column, and neurological disorders. Analysis of clinical findings has indicated that the main causes of complications are late diagnosis of the disease and general practitioners' poor special knowledge. Diagnostic errors may be also caused by a larger number of asymptomatic and atypical forms of tube

Composite pituitary adenoma and intrasellar tuberculoma: report of a rare case.

Tuberculous involvement of the pituitary gland is rare. We report a unique case of a composite lesion consisting of pituitary adenoma and intrasellar tuberculoma. A 24-year-old lady presented with features of acromegaly and amenorrhea. Serum growth hormone levels were found to be raised. Radiological investigations were consistent with a pituitary adenoma. Decompression of the lesion was done through trans-sphenoidal approach. Histological examination revealed a growth hormone secreting pituitary adenoma in association with a granulomatous lesion suggesting of pituitary tuberculoma. No other e
