Computationally Aided Discovery of LysEFm5 Variants with Improved Catalytic Activity and Stability.

Bacteriophage-derived lysin proteins are potentially effective antimicrobials that would benefit from engineered improvements to their bioavailability and specific activity. Here, the catalytic domain of LysEFm5, a lysin with activity against vancomycin-resistant Enterococcus faecium (VRE), was subjected to site-saturation mutagenesis at positions whose selection was guided by sequence and structural information from homologous proteins. A second-order Potts model with parameters inferred from large sets of homologous sequence information was used to predict the average change in the statistic

Antimicrobial resistance, genomic, and public health insights into Enterococcus spp. from Australian cattle.

OBJECTIVES: Enterococci are opportunistic, sometimes life-threatening pathogens with increasing antimicrobial resistance, particularly among clinical human isolates in Australia. While recent studies have ruled out pigs and chickens as major reservoirs of resistant enterococci, the role of cattle remains unclear. This study examines the antimicrobial resistance (AMR) profiles of enterococci from Australian cattle and explores the phylogenetic relationship of bovine E. faecium with isolates from other livestock and human sepsis cases. METHODS: A total of 1001 bovine faecal samples were tested,

Total Synthesis and Antimicrobial Evaluation of 23-Demethyleushearilide and Extensive Antimicrobial Evaluation of All Synthetic Stereoisomers of (16Z,20E)-Eushearilide and (16E,20E)-Eushearilide.

A novel stereoisomer of eushearilide, 23-demethyleushearilide, was synthesized, and the structure-activity relationships of this compound along with known eushearilide stereoisomers were investigated in order to design novel lead compounds for the treatment of fungal infections. It was discovered that all of these congeners, together with the natural product, exhibited a wide range of antimicrobial activity against not only fungi but also against bacteria, including methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE).

Treatment of infections caused by antimicrobial-resistant gram-positive bacteria.

An increasing proportion of Staphylococcus aureus infections are caused by methicillin-resistant S aureus. Treatment of infections caused by this organism is challenging, especially because therapy with vancomycin, the traditional antibiotic of choice for methicillin-resistant S aureus infections, is associated with an increasing frequency of treatment failure, and vancomycin insensitive and vancomycin-resistant strains have emerged. In addition, Enterococcus sp. isolated from human infections are increasingly resistant to multiple antimicrobial agents. Newer drugs available for treatment of r

Bacteremia due to Methicillin-Resistant Staphylococcus aureus: An Update on New Therapeutic Approaches.

Vancomycin and daptomycin are options for the initial treatment of patients with methicillin-resistant Staphylococcus aureus (MRSA) bacteremia. Treatment options for persistent MRSA bacteremia or bacteremia due to vancomycin-intermediate or vancomycin-resistant strains include daptomycin, ceftaroline, and combination therapies. There is a critical need for high-level evidence from clinical trials to allow optimally informed decisions in the treatment of MRSA bacteremia.

A pentad of vancomycin reactions.

Vancomycin is used in the treatment of infections caused by gram-positive bacteria resistant to beta-lactam antibiotics. It is also used in the treatment ofenterococci, although this use has been limited due to resistance. The authors report the clinical and pathological presentations of 5 different vancomycin hypersensitivity reactions, including morbilliform eruption, erythroderma, acute generalized exanthematous pustulosis, linear immunoglobulin A bullous dermatosis, and toxic epidermal necrolysis. With the increased prevalence of beta-lactam-resistant bacteria, reliance on vancomycin conti

Vancomycin heteroresistance caused by unstable tandem amplifications of the vanM gene cluster on linear conjugative plasmids in a clinical Enterococcus faecium.

Vancomycin heteroresistance is prone to missed detection and poses a risk of clinical treatment failure. We encountered one clinical Enterococcus faecium strain, SRR12, that carried a complete vanM gene cluster but was determined as susceptible to vancomycin using the broth microdilution method. However, distinct subcolonies appeared within the clear zone of inhibition in the E-test assay, one of which, named SRR12-v1, showed high-level resistance to vancomycin. SRR12 was confirmed as heteroresistant to vancomycin using population analysis profiling and displayed "revive" growth curves with a

Vancomycin MIC creep and the emergence of oxacillin-susceptible MRSA: A three-year surveillance study from a tertiary care centre.

INTRODUCTION: Methicillin resistant S. aureus (MRSA) remains a high-priority pathogen due to its multidrug resistance. Vancomycin remains mainstay for treating serious MRSA infections. Gradual upward shift of MICs among vancomycin susceptible S. aureus termed as "MIC creep" and emergence of oxacillin-susceptible MRSA (OS-MRSA) pose therapeutic and diagnostic challenges. This study aimed to investigate trends of vancomycin susceptibility and OS-MRSA among S. aureus isolates during the period 2021-2023. METHODS: A total of 1626 non-duplicate clinical isolates of S. aureus were characterized. Va

Emergence of Clinical Clostridioides difficile Isolates With Decreased Susceptibility to Vancomycin.

BACKGROUND: Clostridioides difficile infection (CDI) is a leading cause of hospital-associated antibiotic-related diarrhea and deaths worldwide. Vancomycin is one of the few antibiotics recommended for both nonsevere and severe CDI cases. We sought to determine whether vancomycin nonsusceptible C. difficile strains are circulating in the patient population. METHODS: Stool samples from patients with CDI were collected from 438 and 98 patients at a large university hospital in Houston, Texas, and Nairobi, Kenya, respectively. The stools were examined for the presence of vancomycin and metronida

[Activity of daptomycin, ciprofloxacin, clindamycin and cotrimoxazole against coagulase-negative Staphylococcus strains with diminished susceptibility to vancomycin].

INTRODUCTION: Coagulase Negative Staphylococci (CNS) have become one of the most common nosocomial pathogens and it has a high mortality rate due to the increased of seriously ill patients survival, long states immunosuppression and presence of foreign bodies, such as catheters, prostheses, pacemakers, etc. In addition, there is a significant increase in resistance to antimicrobial drugs, especially beta-lactams, and the increase in the MIC for vancomycin leads to a loss of clinical efficacy. This necessitates the search for new therapeutic alternatives, such as daptomycin. The aim of this pap

Synthesis and Antibacterial Evaluation of Novel Vancomycin Derivatives Containing Quaternary Ammonium Moieties.

A series of novel vancomycin analogues with quaternary ammonium moieties have been designed and synthesized for fighting with clinically isolated drug-resistant bacteria. Partial target molecules exhibited potent activity against the tested strains. Among all of the compounds, a triazole quaternary ammonium vancomycin (QAV) derivative QAV-a1 exerted the best antibacterial activities. QAV-a1 was found to be 4- to 32-fold more efficacious than vancomycin against MRSA. Meanwhile, QAV-a1 showed a good pharmacokinetic profile with a half-life of 5.19 ± 0.10 h, which is longer than that of vancomyci
