Elimination is not the end: experience in handling the last residual filariasis focus in China.

BACKGROUND: A residual focus of bancroftian filariasis was found in Fuchuan County, Guangxi Zhuang Autonomous Region, China in 2007, and the response measures were implemented with participation and guidance from the National Institute of Parasitic Diseases at the Chinese Center for Disease Control and Prevention (China CDC). This study aims to assess the infection sources and summarize the management processes of the residual focus to fill the gap of the re-emergence of focus after elimination as a public health problem. METHODS: The assessment of the infection sources in the residual filari

High prevalence of bancroftian filariasis and comorbidities in the eastern coalfield regions of West Bengal, India following COVID-19 disruption.

Lymphatic filariasis (LF) is one of the most debilitating parasitic diseases worldwide, casting a shadow of socio-economic stigma over the affected underprivileged communities. The present study is a maiden investigation depicting LF prevalence in the previously unexplored remote eastern coalfield districts of West Bengal state, India during and after COVID-19 pandemic. A community-based cross-sectional survey (August 2021–March 2024) was conducted in four LF-endemic districts of West Bengal. Symptomatic cases were graded clinically, and asymptomatic infections confirmed by filaria-specific Ig

Confirmatory mapping for lymphatic filariasis in districts previously considered nonendemic in Ghana.

OBJECTIVES: Lymphatic filariasis (LF) elimination efforts in Ghana have been ongoing since 2001, achieving substantial progress through mass drug administration (MDA). However, despite significant advances, LF transmission persists in certain areas. Some districts previously classified as nonendemic have reported lymphedema and hydrocele cases, raising concerns about LF endemicity. To address these gaps, a confirmatory mapping survey was conducted to reassess LF prevalence in districts with uncertain morbidity. METHODS: A cross-sectional survey using the WHO-approved LF confirmatory mapping t

Elimination of lymphatic filariasis as a public health problem from the Arab Republic of Egypt.

Lymphatic filariasis (LF) has been known in Egypt since ancient times. By 1930s it was recognized to be a major public health problem in the Nile Delta, and to be caused by Wuchereria bancrofti and transmitted by Culex pipiens. Remarkably, as a result of widespread DEC treatment and intensive vector control by the Ministry of Health and Population (MoHP), the infection rate of LF declined in the 1960s. However, relaxation of these efforts resulted in resurgence of filariasis in the 1980s and 1990s. In 2000, Egypt was among the first countries to join the WHO global efforts to eliminate LF as a…

Elimination of lymphatic filariasis in the Gambia.

BACKGROUND: The prevalence of Wuchereria bancrofti, which causes lymphatic filariasis (LF) in The Gambia was among the highest in Africa in the 1950s. However, surveys conducted in 1975 and 1976 revealed a dramatic decline in LF endemicity in the absence of mass drug administration (MDA). The decline in prevalence was partly attributed to a significant reduction in mosquito density through the widespread use of insecticidal nets. Based on findings elsewhere that vector control alone can interrupt LF, we asked the question in 2013 whether the rapid scale up in the use of insecticidal nets in Th…

Serum immune complexes as diagnostic and therapeutic markers in lymphatic filariasis.

In the present study we examined the utility of measuring antigen-specific soluble immune complexes (ICs) for the differential diagnosis and therapeutic monitoring of Bancroftian filariasis. Antigen-specific ICs were detected in significantly elevated levels in 90% of lymphatic filarial subjects suffering from disease manifestations, and very low levels in 8-10% of microfilaria (MF) carriers. The high incidence of antigen-specific ICs in the circulation of filarial subjects with clinical manifestations in contrast to the negligible presence in MF carriers facilitates serological differentiatio

Assessment of intradermal test in longitudinal surveillance of bancroftian filariasis.

Longitudinal surveillance was carried out consecutively during 1982-1987 in two pilot villages where bancroftian filariasis was prevalent in different severity before 1982. Intradermal test (IT) with filaria polypeptide (FPT) antigen prepared from Dirofilaria immitis was made. It was shown that the level of immediate hypersensitivity reaction to the antigen in population as shown by IT appeared to be directly proportional to both the prevalence of the disease and the infection rate of mosquitoes harbouring larval filariae before mass and selective treatment with diethylcarbamazine. Time for ne

Filariasis eradication on Kinmen Proper, Kinmen (Quemoy) Islands, Republic of China.

A control program for bancroftian filariasis was conducted on Kinmen Proper from 1970 to 1982. The combined method of mass chemotherapy with diethylcarbamazine (DEC) for microfilarial carriers and larvicide with sumithion for mosquitoes was used on the island for 3 years (1974-76). The microfilarial rate and microfilarial density decreased from 8.4% and 14.6/20 microns in 1970-73 to 0.7% and 5.3/20 microns in 1976. The infection rate, infective rate, and larval density of Culex quinquefasciatus, the only mosquito vector of the parasite, decreased from 10.3%, 6.4%, and 6.4 in 1970-73 to 0.3%, 0

Bancroftian filariasis in Tambol Na-Sai, Amphoe Li, Lamphun Province: a preliminary survey for microfilaraemia.

A preliminary survey for Bancroftian filariasis in villagers of Ban Prabat Heuy Tom, Tambol Na-Sai, Amphoe Li, Lamphun Province was performed between July 12, 1986 and March 19, 1987. A total of 1,435 villagers comprising 668 females and 767 males were examined for microfilaraemia in peripheral blood. Microfilariae were found in five males of which two cases were found to have developed hydrocele at a lesser degree and all of them had formerly resided in an endemic area of Tak Province. Microfilarial periodicity was studied in two cases, and it was shown to be nocturnally subperiodic.

The status of human filariasis in relation to clinical signs in endemic areas of the Niger Delta.

A study was carried out in selected parts of the Niger Delta Basin on human filariasis and its relationship to clinical signs. One hundred and seventy-six (13.0%) of 1351 individuals examined were infected with microfilaria. Forty-six percent of microfilaria positive cases had M. perstans; 25.6% had W. bancrofti, 19.3% had L. loa, and 9.0% had O. volvulus. The prevalence of filariasis increased with age. Microfilarial density was generally low, 28.4% of infected individuals had counts of 50-100 microfilaria per 20 mm3 blood, 12.5% had counts less than five microfilaria per 20 mm3 blood. Onchoc

Implementing a community vector collection strategy using xenomonitoring for the endgame of lymphatic filariasis elimination.

BACKGROUND: The global strategy for elimination of lymphatic filariasis is by annual mass drug administration (MDA). Effective implementation of this strategy in endemic areas reduces Wuchereria bancrofti in the blood of infected individuals to very low levels. This minimises the rate at which vectors successfully pick microfilariae from infected blood, hence requiring large mosquito numbers to detect infections. The aim of this study was to assess the feasibility of using trained community vector collectors (CVCs) to sample large mosquito numbers with minimal supervision at low cost for poten…

Changes in Antibody Levels during and following an Episode of Acute Adenolymphangitis (ADL) among Lymphedema Patients in Léogâne, Haiti.

INTRODUCTION: Episodes of acute adenolymphangitis (ADL) are often the first clinical sign of lymphatic filariasis (LF). They are often accompanied by swelling of the affected limb, inflammation, fever, and general malaise and lead to the progression of lymphedema. Although ADL episodes have been studied for a century or more, questions still remain as to their etiology. We quantified antibody levels to pathogens that potentially contribute to ADL episodes during and after an episode among lymphedema patients in Léogâne, Haiti. We estimated the proportion of ADL episodes hypothesized to be attr

Effect of annual mass administration of diethylcarbamazine and albendazole on bancroftian filariasis in five villages in south India.

Annual mass drug administration (MDA) is the recommended strategy for lymphatic filariasis (LF) elimination. We assessed the effect of six rounds of mass administration of diethylcarbamazine (DEC) and albendazole (ALB) on microfilaria (Mf) prevalence and intensity and vector infection and infectivity rates and circulating filarial antigenaemia (CFA) in a group of five villages in south India, endemic for Culex-transmitted bancroftian filariasis. During different rounds of MDA, 60-70% of the eligible population (>15 kg body weight) was treated. The MDA reduced the Mf prevalence from 8.10% (CI 6
