The role of AbaI quorum sensing molecule synthase in host cell inflammation induced by Acinetobacter baumannii and its effect on zebrafish infection model.

Acinetobacter baumannii is currently one of the most important opportunistic pathogens causing severe nosocomial infections worldwide. Quorum Sensing (QS) system is a widespread mechanism in bacteria to coordinate group behavior by sensing the density of bacterial populations and affect eukaryotic host cell. In Acinetobacter baumannii, AbaI protein is used as QS molecule synthetase to synthesize N- acyl homoserine lactones (AHLs). Currently, QS has made great progress in the study of drug resistance, but there is still a lack of complete understanding of its damage to host cells after adhesion…

The whole-genome molecular epidemiology of sequential isolates of Acinetobacter baumannii colonizing the rectum of patients in an adult intensive care unit of a tertiary hospital.

Acinetobacter baumannii is a multidrug-resistant nosocomial pathogen that colonizes and infects debilitated patients in the ICU. There is very little information on the genomic characteristics of colonizing strains. This information is important to understand the evolution of lineages of A. baumannii that develop resistance while patients receive antibiotic treatment in the ICU. Our study demonstrated different patterns of colonization of the rectum of ICU patients with different STs of A. baumannii while one ST colonized all patients. Some STs carried more antibiotic resistance genes compared…

Changes in Antibiotic Resistance of Acinetobacter baumannii and Pseudomonas aeruginosa Clinical Isolates in a Multi-Profile Hospital in Years 2017-2022 in Wroclaw, Poland.

In recent years, we have witnessed increasing drug resistance among bacteria, which is associated with the use and availability of an increasing number of broad-spectrum antimicrobials, as well as with their irrational and excessive use. The present study aims to analyze changes in the drug resistance of Gram-negative Pseudomonas aeruginosa and Acinetobacter baumannii, isolated from infections in a multi-profile hospital over a five-year period (from 2017 to 2022). Among the practical results of the evaluation of these data will be the possibility to determine changes in susceptibility to the …

The Novel β-Lactam Enhancer Zidebactam Augments the In Vivo Pharmacodynamic Activity of Cefepime in a Neutropenic Mouse Lung Acinetobacter baumannii Infection Model.

WCK 5222 is a combination of cefepime and the high-affinity PBP2-binding β-lactam enhancer zidebactam. The cefepime-zidebactam combination is active against multidrug-resistant Gram-negative bacteria, including carbapenemase-expressing Acinetobacter baumannii The mechanism of action of the combination involves concurrent multiple penicillin binding protein inhibition, leading to the enhanced bactericidal action of cefepime. The aim of the present study was to assess the impact of the zidebactam-mediated enhanced in vitro bactericidal action in modulating the percentage of the time that the fre…

Evolution of a clade of Acinetobacter baumannii global clone 1, lineage 1 via acquisition of carbapenem- and aminoglycoside-resistance genes and dispersion of ISAba1.

Resistance to carbapenem and aminoglycoside antibiotics is a critical problem in Acinetobacter baumannii, particularly when genes conferring resistance are acquired by multiply or extensively resistant members of successful globally distributed clonal complexes, such as global clone 1 (GC1) . Here, we investigate the evolution of an expanding clade of lineage 1 of the GC1 complex via repeated acquisition of carbapenem- and aminoglycoside-resistance genes. Lineage 1 arose in the late 1970s and the Tn6168/OCL3 clade arose in the late 1990s from an ancestor that had already acquired resistance to…

Assessment of the In Vivo Efficacy of WCK 5222 (Cefepime-Zidebactam) against Carbapenem-Resistant Acinetobacter baumannii in the Neutropenic Murine Lung Infection Model.

We evaluated the in vivo efficacy of human-simulated WCK 5222 (cefepime-zidebactam) against cefepime-resistant Acinetobacter baumannii strains (n = 13) in the neutropenic murine lung infection model. Twelve isolates were meropenem resistant. In control animals and those that received cefepime or zidebactam alone, the mean bacterial growth at 24 h was >2 log10 CFU/lung compared with 0-h controls (6.32 ± 0.33 log10 CFU/lung). WCK 5222 produced a decline in the bacterial burden for all isolates (mean reduction, -3.34 ± 0.85 log10 CFU/lung) and demonstrated remarkable potency.

Inhibition of aminoglycoside 6'-N-acetyltransferase type Ib by zinc: reversal of amikacin resistance in Acinetobacter baumannii and Escherichia coli by a zinc ionophore.

In vitro activity of the aminoglycoside 6'-N-acetyltransferase type Ib [AAC(6')-Ib] was inhibited by ZnCl2 with a 50% inhibitory concentration (IC50) of 15 μM. Growth of Acinetobacter baumannii or Escherichia coli harboring aac(6')-Ib in cultures containing 8 μg/ml amikacin was significantly inhibited by the addition of 2 μM Zn(2+) in complex with the ionophore pyrithione (ZnPT).

Distribution of beta-lactamases in Acinetobacter baumannii clinical isolates and the effect of Syn 2190 (AmpC inhibitor) on the MICs of different beta-lactam antibiotics.

The distribution of beta-lactamases in a group of 20 epidemiologically well defined Acinetobacter baumannii clinical isolates and the in vitro activity of Syn 2190, a novel beta-lactamase AmpC inhibitor, were determined. Twenty-five per cent of the strains carried and expressed a TEM-type beta-lactamase, whereas 35% had an OXA-type beta-lactamase. In nine out of 11 (82%) ceftazidime-resistant and four out of 13 (30.7%) cefepime-resistant strains, the MIC of these beta-lactam antibiotics decreased when determined in the presence of Syn 2190. Thus, our results suggest that in a high percentage o…

Assessing pathogenicity: OmpA and AbaR gene expression in foodborne Acinetobacter baumannii from fresh fruits and vegetables in Chattogram: addressing food safety concerns.

BACKGROUND: Fruits and vegetables can serve as reservoirs for Acinetobacter baumannii, a multidrug-resistant pathogen of growing concern. Its outer membrane protein A (OmpA) and autoinducer regulator (AbaR) are key virulence factors, contributing to biofilm formation, survival, and antibiotic resistance. However, little is known about the expression of these genes in foodborne isolates, particularly in Bangladesh. RESULT: Fifty-six fresh produce samples from Chattogram were screened, revealing that the prevalence of A. baumannii was nearly three times higher than that of other foodborne pathog…

Vaccination Drives Alveolar Macrophages Pool Remodeling via Training Resident Cells and Recruiting Monocyte-Derived Cells to Defend Against Multidrug-Resistant Acinetobacter baumannii.

Alveolar macrophages (AMs) serves as a frontline innate barrier against pulmonary bacterial invasion. The AM pool comprises tissue-resident AMs (TR-AMs) and monocyte-derived AMs (Mo-AMs); yet how vaccination remodels the mixed AM pool for long-term antimicrobial defense against multidrug-resistant bacteria remains poorly understood. In this work, we applied intranasal inactivated whole-cell (IWC) vaccination against Acinetobacter baumannii and systematically dissected AM population dynamics at cellular and molecular levels. Vaccination reshaped the AM pool by recruiting CD11B+CD13+ Mo-AMs that…

An integrated immunoinformatics and experimental pipeline identifies an LCL-scaffolded OmpA loop 3 construct as a protective vaccine candidate against Acinetobacter baumannii.

Acinetobacter baumannii has emerged as a critical nosocomial pathogen due to its extensive antimicrobial resistance and limited treatment options, highlighting the urgent need for effective vaccines. In this study, we developed and evaluated a rationally engineered epitope-based vaccine targeting extracellular loop 3 of outer membrane protein A (OmpA). Using an integrated immunoinformatics and experimental pipeline, the selected OmpA loop was grafted onto a loopless C-lobe (LCL) scaffold derived from transferrin-binding protein B to preserve native-like conformation and enhance epitope present…

Clinical phenotyping of polymyxin-associated nephrotoxicity and phenotype-specific prognostic utility of neutrophil-to-platelet ratio in carbapenem-resistant Acinetobacter baumannii infections.

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) infections remain therapeutically challenging, often necessitating last-line polymyxin therapy despite substantial nephrotoxicity. Conventional risk assessments for polymyxin-associated acute kidney injury (AKI) frequently overlook patient heterogeneity. We sought to delineate clinical phenotypes of polymyxin-associated AKI and determine whether the prognostic value of the neutrophil-to-platelet ratio (NPR) for mortality varies by phenotype. METHODS: We conducted a retrospective cohort study (2020–2025) of 547 patients with CRAB i…
