A fourteen years old girl showed the classic signs of acrocephalosyndactyly I: dysostosis craniofacialis with hypertelorism, exophthalmus, strabism, amblyopia and cleft palate as well as syndactyly of the fingers and toes. The feet showed on both side a 6 cm long horny band. Since the twelfth year of life, she had suffered from papulo-pustular acne with many comedomes. Her menstruation started one year later. Intellectual development was normal. At time of her birth, her father was 54 years old, and her mother 36 years old. Two elder siblings are healthy. The inheritance of acrocephalosyndacty…

The pediatrician or family physician is usually the first person to examine an infant with a periocular capillary hemangioma. The physician should know that, in addition to being cosmetically disfiguring, capillary hemangiomas have the potential to cause serious ocular complications such as amblyopia, strabismus, proptosis, and optic atrophy. Prompt evaluation and management of periocular hemangiomas are essential in order to prevent permanent visual loss. Corticosteroids have been the mainstay in the treatment of patients with large periocular hemangiomas, but other therapeutic modalities suc…

PURPOSE: To report a child with the mitochondrial cytopathy of Pearson syndrome and zonular cataract.
METHOD: Case report. We describe a 6-year-old boy with Pearson syndrome.
RESULTS: At age 3 years, the boy developed secondary strabismus caused by bilateral zonular cataract. Subsequently, he underwent successful bilateral cataract extraction with intraocular lens implantation. Postoperative visual acuity with best correction was RE, 20/25 and LE, 20/40.
CONCLUSIONS: Children with Pearson syndrome should be examined ophthalmologically to rule out zonular cataract and possible amblyopia. Mit…

The pattern electroretinogram (PERG) has recently been introduced as a clinical procedure. It has been thought by many to represent activity … ertension and in retinal vascular disorders such as diabetes. It was also affected in some cases of amblyopia. This paper briefly describes the techniques used to record the PERG and reviews current literature pertaining to its clinical application.

A child who had had a congenital cataract phacoemulsified at age four months became contact lens intolerant, and at age two years had an implantation of a Worst metal-looped iris-clip intraocular lens. He developed painful pseudophakic bullous keratopathy three years later. Since the child remained with dense deprivation amblyopia and contact lens intolerance, when the IOL was removed an epikeratophakia graft was applied. The bullous keratopathy resolved and the patient has remained asymptomatic for 22 postoperative months. The proposed mechanism for relief of the bullous keratopathy is to inc…

Epikeratophakia is a form of refractive surgery in which the recipient's epithelium is removed and a pre-shaped donor lenticule is sutured to the patient's cornea. Seventeen patients ranging in age from two months to 6.6 years received 19 epikeratophakia grafts for the correction of aphakic vision. The correction provided by the graft in combination with vigorous amblyopia therapy has yielded some improvement in vision in the 12 patients with successful grafts. This procedure may be particularly suitable for pediatric aphakic patients because it is extraocular and reversible and may be employe…

During the period 1970-73, 1,046 children under 20 years of age were registered with the Canadian National Institute for the Blind. The three most common registration diagnoses were Cataract (13%), Optic Atrophy (12%) and Nystagmus (10%); Retrolental Fibroplasia was responsible for a smaller proportion (6%). Twenty per cent of the registration diagnoses were non-specific and included "Nystagmus", "Site or Type not Established", "Affection of Visual Centre" and "Amblyopia". Without a specific diagnosis one could not decide whether the blindness was due to genetic or environmental causes. It is …

After defining physiological fixation of the fovea and relating our knowledge of its normal development in the context of spatial perception, we have reviewed the pathological adaptations encountered in strabismus. Out of all the clinical examinations that are possible in very young children and which enable sensory disturbances to be revealed, the examination of fixation appears to be simple and dependable, sometimes well before the appreciation of the comparaison of visual acuity of the two eyes. Amblyopia can be suspected using this method in cases of poor foveal fixation when compared with…

Measurements of the refractive errors in 55 strabismic patients--one eye with visual acuity of 0.8 or better and the other with reduced vision, 0.4 or less due to amblyopia, and performed at roughly two-year intervals during growth and development to approximately age 25 years--showed that the eye with normal visual acuity was significantly more myopic with age, whereas there was no appreciable change in the average refractive error of the amblyopic eye. Both eyes of a control group of patients of similar age having normal distant vision and orthophoria were more myopic with age. In this group…

A newborn male with cat eye syndrome presented with progressively worsening bilateral upper eyelid imbrication, floppy eyelids, and ptosis. Despite conservative management, he remained unable to open his eyelids. Surgical correction was planned to prevent bilateral sensory deprivation amblyopia and was delayed until 5 months of age due to systemic health concerns. Bilateral full-thickness wedge excision and frontalis suspension with silicone rods in a double rhomboid fashion was performed. Postoperatively, the patient demonstrated spontaneous eyelid opening, resolution of spastic eversion of t…

Posttraumatic aphakia is a serious deterrent to visual rehabilitation that usually follows sensory exotropia, thereby mandating urgent surgical intervention. In addition, abnormal iris configuration and iris defects postocular trauma can lead to amblyopia and glare and require surgical correction. However, in individuals where such multiple posttraumatic extraocular and intraocular abnormalities coexist, confusion remains about whether to perform sequential or concomitant surgeries. In these situations, an unusual triple technique consisting of strabismus correction, glued scleral fixation of …

A 27-year-old woman presented with a history of long-standing poor vision in both eyes. Ophthalmologic examination after pupillary dilatation revealed bilateral lens coloboma situated in the inferotemporal quadrant. No associated ocular abnormalities were seen, except amblyopia. A bicuspid aortic valve was observed during echocardiography during systemic evaluation. Lens coloboma usually occurs unilaterally; however, bilateral lens coloboma which is isolated or associated with other ocular malformations is also encountered. This is the first description of bilateral isolated lenticular colobom…

Penetrating keratoplasty in infants and young children is performed on an infrequent basis. The most common indication is visually significant congenital corneal opacity. Surgery must be performed early to avoid amblyopia. Surgical techniques differ from those used in adult penetrating keratoplasty because of the reduced ocular rigidity encountered in infants and young children. Use of a multispecialty team approach is important to improve visual outcome. Poor prognostic indicators include bilateral disease, concomitant infantile glaucoma, lensectomy and vitrectomy at the time of surgery, prev…

Pattern-reversal visual evoked potentials elicited by patterns of 60' and 20' checks and 33 Hz steady-state flash visual evoked potentials were recorded in low-vision children (visual acuity less than 20/120) with optic atrophy (18 eyes), amblyopia (18 eyes), macular degeneration (7 eyes) and controls (24 eyes). Steady-state visual evoked potentials were non-recordable in 17 out of 18 eyes of optic atrophy patients and normal in other children. Abnormalities of pattern-reversal visual evoked potentials were similar in all groups of patients. 33 Hz steady-state flash visual evoked potentials ar…

In 10 cases, aphakia after silicone oil surgery was corrected as follows: after completion of all retinal interventions, but not less than 3 months after silicone oil removal, an anterior chamber lens (Kelman Multiflex) was implanted. Binocular vision was interrupted for periods ranging from 4 months to 4.6 years (mean 2 years). Tests of the recovered binocular vision, on average 13 months after implantation of the lens, revealed simultaneous vision in 7 cases and exclusion in 2 (primary amblyopia; secondary divergent squint). Stereoscopic vision of 3800" was achieved in 4 cases, and of 200" i…

Surgical treatment of ectopia lentis has traditionally been associated with a poor visual outcome and a high complication rate. We treated a series of nine children (15 eyes) whose visual acuity could not be improved with optimal optical phakic or aphakic correction with limbal lensectomy. Preoperative visual acuity after amblyopia treatment ranged from 20/60 to 20/200. Improvement in postoperative visual acuity was documented in all operated-on eyes, ranging from 20/20 to 20/50 during a follow-up period ranging from 8 months to 118 months (median, 33 months). The only complication was a child…

Twenty heterophoric and 10 heterotropic patients with long standing severe visual symptoms were corrected with prisms for permanent wearing using the full-correction method of H.-J. Haase. The patients were reexamined 1 and 5 years after completion of treatment. Stable corrections obtained by stepwise optical full-correction were also maintained during a follow-up period of 5 years. Sensory and motor adaptations were largely removed and asymptomatic binocular vision was obtained for nearly all patients. A few cases of partly irreversible amblyopia were indicated, but even in these cases largel…

Congenital esotropia represents the most common type of strabismus. Its pathogenesis, however, remains uncertain. It is typically characterized as a large angle, constant esotropia with onset during the first six months of life. Associated clinical findings include normal refractive errors for age, amblyopia, dissociated vertical deviation, inferior oblique muscle overaction and nystagmus. It must be distinguished from Duane's retraction syndrome, Moebius syndrome, nystagmus blockage syndrome, and early onset accommodative esotropia, as well as other causes of esotropia in infancy. The surgica…
