[Study of spontaneous acetylcholine-dependent tachyarrhythmias using isolated specimens of the right canine atrium by bilateral mapping of the spread of excitation].

The authors examined tachycardias induced by administering acetylcholine (AC), 1-2 micrograms into the artery of sinus node (ASN) of an isolated specimen of the canine right atrium, which had spontaneous automatism. Bilateral multielectrode mapping was employed. The episodes of tachycardia occurred during AC-induced arrest of sinus rhythm. In 81% of the cases, episodes of arrhythmia consisted of 2-3 beats, in 19%, 6-150 beats. The mapping revealed a focal picture of activation during short-term episodes of arrhythmia and transition from a focal type of activation to the re-entry in most cases

Effects of upright posture on anterograde and retrograde atrioventricular conduction in patients with coronary artery disease, mitral valve prolapse or no structural heart disease.

To assess the effects of posture on anterograde and retrograde atrioventricular conduction, electrophysiologic testing was performed in 25 patients in both the supine and 45 degrees upright positions on a tilt table. Retrograde conduction was present during ventricular pacing in 17 patients in the supine position; all 17 continued to manifest retrograde conduction in the upright position. In all patients with absent retrograde conduction while supine, retrograde conduction could not be demonstrated while upright. Upright posture significantly (p less than 0.05) shortened the sinus cycle length

Magnesium and potassium therapy in multifocal atrial tachycardia.

Eight patients with multifocal atrial tachycardia received 7 to 12 gm of magnesium sulfate intravenously over a 5-hour period. Potassium supplements were given initially or added later. Initial arterial blood gases showed mean pH 7.48 +/- 0.03, PcO2 39.7 torr, PO2 72 torr, HCO-3 29.8 +/- 4.5 mEq/L, and base excess 6.84 +/- 3.78 mEq/L. Initial serum magnesium correlated well with initial serum potassium. Three patients had subnormal levels of magnesium and potassium. The level of serum magnesium rose with an intravenous injection magnesium and serum potassium levels tended to fall unless they w

Simplified technique for permanent atrioventricular pacing via a single venipuncture.

The use of physiologic pacing utilizing transvenous atrial and ventricular electrodes has recently taken on increased importance. A simple, safe, technique for electrode implantation has been used in 14 patients and enabled the implanting physicians to establish permanent atrioventricular (A-V) sequential pacing with low risk in a short span of time.

Effects of phenytoin on refractoriness and conduction in the human heart.

Using His bundle electrograms and the atrial (A2) and ventricular extrastimulus (V2) techniques, anterograde and retrograde refractory period studies were performed (in 9 and 12 patients, respectively) before and 10 min after intravenous infusion of phenytoin (DPH; mean plasma level, 17.3 micrograms/ml). DPH had no effect on the duration of the QRS complex or the H-V interval of the sinus beats; it had variable but insignificant effects on the sinus rates and the atrial, A-V nodal, and ventricular muscle refractoriness. With the use of the A2 technique, the effective refractory period (ERP) of

Effect of pacing mode on health-related quality of life in the Canadian Trial of Physiologic Pacing.

BACKGROUND: Both "physiologic" (dual-chamber or atrial only) or ventricular-pacing-only permanent pacemakers provide chronotropic competence, with unknown health-related quality of life (QOL) differences between these options. The QOL studies within the Canadian Trial of Physiologic Pacing were performed to assess whether QOL differences exist in patients randomized to these 2 pacing modes. METHODS: Two QOL protocols were performed: 1) a substudy of 269 patients with detailed QOL measures (The Medical Outcomes Study, Short-Form [SF-36], the Pacemaker Syndrome Scale, an activity scale, and a p

Effects of hypoxia and ischaemia on coronary vascular resistance, A-V node conduction and S-A node excitation.

Hypoxia and ischaemia produce relaxation of the vascular smooth muscle of the resistance vessels in the myocardium. A low pH may contribute to the arteriolar dilatation but cannot account for the major response. In all likelihood the reduced pO2 acts indirectly by release of a vasodilator mediator rather than by a direct effect on the vascular muscle. Potassium release may account for a transient component of the vasodilation, but it appears that a major factor is the release of adenosine from the ischaemic tissue. Adenosine also appears to be responsible for slowing of the sinus rate and impa

Concentration-dependent enhancement of junctional pacemaker activity by verapamil in man.

The effects of verapamil on junctional and sinoatrial activity were studied in eight patients with third-degree atrioventricular block and stable junctional rhythms. After intravenous infusion of verapamil three steady-state levels were reached as follows: 34 +/- 16 (SS1), 68 +/- 30 (SS2), and 129 +/- 54 (SS3) ng/ml. At each steady-state level, spontaneous sinus and junctional cycle lengths, QT intervals, junctional recovery times after ventricular pacing, blood pressure, and circulating catecholamine concentrations were measured. These measurements were compared with observations made during

Canine electrophysiology of encainide, a new antiarrhythmic drug.

Encainide, a new benzanilide derivative with high potency and a good therapeutic/toxic ratio, was evaluated with the use of standard His bundle recording techniques to determine its effects on the cardiac conduction system in closed chest animals. Twenty mongrel dogs weighing 18 to 29 kg were anesthetized with 4 percent chloralose and classified into groups: group 1, a control group and groups 2, 3, and 4, which were given 0.3, 0.9 and 2.7 mg/kg body weight, respectively, of encainide in an intravenous infusion over a 15 minute period. Plasma concentration, blood pressure, surface electrocardi

Complete Heart Block and Ventricular Asystole Caused by Vagus Nerve Stimulation Therapy.

Left vagus nerve stimulation (VNS) is an advanced therapeutic option for refractory, drug-resistant epilepsy. A 45-year-old woman with a history of refractory catamenial focal epilepsy since age 16, treated with a five-drug antiepileptic regimen and VNS (implanted eight and one-half years prior), presented with dyspnea, chest discomfort, and lightheadedness. During observation, symptoms recurred and were associated with bradycardia (<20 bpm) and a complete atrioventricular node (AVN) block. Following admission, she continued to experience recurrent symptomatic AVN block and transient ventricul…

Cardiac Implantable Electronic Devices in Cardiac Transplant Patients: A Comprehensive Review.

A fraction of patients (approximately 10%) undergoing heart transplantation require permanent pacemaker (PPM) implantation due to sinus node dysfunction or atrioventricular block, occurring either shortly after surgery or later. The incidence of PPM implantation has declined to less than 5% with the introduction of bicaval anastomosis transplantation surgery. Pacing dependency during follow-up varies among recipients. A smaller subset (1.5-3.4%) receives implantable cardioverter-defibrillators (ICDs), but data on their use in transplant recipients are limited, primarily from cohort studies and

Efficacy of ivabradine in HIV-associated dilated cardiomyopathy.

Ivabradine is a pure heart rate-lowering drug that acts directly on the sinus node and does not affect atrioventricular or intraventricular conduction times, myocardial contractility or ventricular repolarization. Ivabradine is used in the treatment of adults with New York Heart Association (NYHA) class II-IV chronic heart failure (CHF) with systolic dysfunction, in sinus rhythm, with a heart rate of ≥75 beats per minute (bpm). This report describes the beneficial effect of ivabradine on clinical parameters in a HIV-infected patient with dilated cardiomyopathy presenting with NYHA class III CH
