The time course of postflight simulator sickness symptoms.

Reports of posteffects following training sessions in Navy and Army flight simulators were obtained from pilots to determine the time course of recovery from simulator sickness. Results imply that posteffects may be a more serious problem than previously considered; 45% of all those queried (N greater than 700) reported having experienced symptoms of simulator sickness; 25% of the symptoms lasted more than 1 h after leaving the simulator; and 8% lasted more than 6 h. Postexposure symptoms were classified into three categories: visuomotor (based on disturbances in oculomotor control; e.g., eyes

Heat loss caused by immersing the hands in water.

The effect of immersing the hands up to the wrist in cold water to alleviate heat strain was examined in volunteers wearing chemical protective clothing and gloves. Each subject, who was monitored with skin and rectal thermistors, was observed while walking on a treadmill at two different work rates (283 +/- 47 and 455 +/- 58 watts) at 23 degrees C and at a resting state at 35 degrees C. After 20 min of work at 23 degrees C or after 120 min in the hot room, the hands were immersed in water at temperatures of 10, 15, 20, 25, and 30 degrees C. The amount of heat lost via the hands ranged between

Irregular nocturnal breathing patterns high altitude in subjects susceptible to high-altitude pulmonary edema (HAPE): a preliminary study.

We studied nocturnal breathing patterns and symptoms of acute mountain sickness (AMS) during trekking in the Japanese Alps (altitude: 2,760-2,920 m) for 4 d in five subjects susceptible to high-altitude pulmonary edema (HAPE-S-S) and five control volunteers. Breathing patterns were evaluated with the impedance plethysmograph, and symptoms of AMS were evaluated by the environmental symptoms questionnaire-III score for AMS of cerebral type (AMS-C score). In both groups, the percentage of time with periodic breathing significantly increased at high altitude and the percentage in controls was sign

Transplantation of cryogenically preserved mandibular condyles in the adult and juvenile goat.

Loss of a functional temporomandibular joint (TMJ), due to various causes including tumor resection, joint disease such as osteonecrosis, and injury or trauma, has long been a clinical challenge in both children and adults. It may be a significant problem facing physicians during another war. While reconstruction to date has been performed with various prosthetic devices or autogenous costochondral grafts, these procedures have a potential for complications and morbidity. This procedure could adversely delay the return of military members to a combat unit, or affect their ability to serve as f

Mechanisms of biological effects of radiofrequency electromagnetic fields: an overview.

Manmade sources of electromagnetic (EM) fields, and therefore human exposures to them, continue to increase. Public concerns stem from the effects reported in the literature, the visibility of the sources, and somewhat from confusion between EM fields and ionizing radiation. Protecting humans from the real hazards and allaying groundless fears requires a self-consistent body of scientific data concerning effects of the fields, levels of exposures which cause those effects, and which effects are deleterious (or beneficial or neutral). With that knowledge, appropriate guidelines for safety can b

A case of high altitude pulmonary edema followed by brain computerized tomography and electroencephalogram.

A case of high altitude pulmonary edema with high altitude cerebral edema was reported. A young Japanese male complained of severe palpitation and shortness of breath on the third day of climbing at 3,000 m above sea level. During the next 2 d at altitude, the following symptoms occurred: cough with foamy sputum, cyanosis, and loss of consciousness. Soon after evacuation, he showed severe hypoxemia and deep coma with decerebrate rigidity; electroencephalogram showed diffuse alpha waves, indicating "alpha wave coma." Brain computerized tomography revealed brain edema, showing small compressed v

Differentiation of muscarinic cholinergic receptor subtypes in human cortex and pons: implications for anti-motion sickness therapy.

Radioligand binding studies were used to analyze muscarinic cholinergic receptor subtypes in human cortex and pons. Muscarinic cholinergic receptors were labeled by 3H-quinuclidinyl benzilate (3H-QNB). Scopolamine was equipotent in both brain regions and did not discriminate subtypes of 3H-QNB binding. By contrast, the M1 selective antagonist pirenzepine was approximately 33-fold more potent in human cortex than pons. Carbachol, a putative M2 selective agonist, was more than 100-fold more potent in human pons than cortex. These results demonstrate that the human pons contains a relatively larg

Quantification of reaction time and time perception during Space Shuttle operations.

A microprocessor-based test battery containing simple reaction time, choice reaction time, and time perception tasks was flown aboard a 1985 Space Shuttle flight. Data were obtained from four crewmembers. Individual subject means indicate a correlation between change in reaction time during the flight and the presence of space motion sickness symptoms. The time perception task results indicate that the shortest duration task time (2 s) is progressively overestimated as the mission proceeds and is statistically significant (p less than 0.01) when comparing preflight and postflight baselines. Effects of scopolamine and dextroamphetamine on human performance.

The effects of two drugs used to prevent symptoms of motion sickness in the operational environment were examined in this study of human performance as measured by computer-based tests of cognitive and psychomotor skills. Each subject was exposed repetitively to five tests: Symbol-Digit Substitution, Simple Reaction Time, Pattern Recognition, Digit Span Memory, and Pattern Memory. Although there have been previous reports of decreases in human performance in similar testing with higher dosages of scopolamine or dextroamphetamine, no significant decrements were observed with the operational-lev

The Republic of Singapore Navy's Scopoderm TTS study: results after 2,200 man-days at sea.

Most clinical trials investigating prevention of seasickness with transdermal scopolamine have been short-term, and little information regarding long-term use is available. We report here a double-blind trial conducted on board a flat-bottomed Republic of Singapore Navy vessel of 2,490 tons, sailing in the South China Sea. The trip lasted 26 d, but self-assessment by the 122 adult male participants (using a simplified scale quantifying the level of seasickness) was confined to the 18 d spent at sea. We found that the protection rate with transdermal scopolamine was 46-57%, with maximum benefit

Acceleration tolerance of asymptomatic aircrew with mitral valve prolapse and significant +Gz-induced ventricular dysrhythmias.

The +Gz tolerance of asymptomatic aircrew with mitral valve prolapse (MVP) who had advanced Lown grade ventricular ectopy (multiformed or paired premature ventricular contractions and ventricular tachycardia) during +Gz-stress testing was investigated. The +Gz tolerance to an aeromedical centrifuge stress testing protocol revealed that these individuals had slightly lower +Gz tolerance for each of the different types of +Gz profiles, with the gradual onset of +Gz while using an anti-G straining maneuver (GOR-S) tolerance being significantly lower (p less than 0.05). As a subgroup of a larger g

Motion sickness and anxiety.

Ninety-four Israeli pilot trainees completed a battery of anxiety related questionnaires: Taylor Manifest Anxiety Scale, EPQ, 16PF, and Spielberger's State and Trait Anxiety Scores. Self reports and flight instructor observations of motion sickness symptoms were collected after initial flights. No significant correlations were found between these two sources. Anxiety scores derived from the battery of anxiety questionnaires were correlated with self reports of motion sickness but not with instructor observations. Discussion focused on the potential limitations of external observers in assessin

Ocular torsion in upright and tilted positions during hypo- and hypergravity of parabolic flight.

Four subjects considered resistant to motion sickness were tested in KC-135 parabolic flight to examine ocular torsion at hypo- and hypergravity. Three of these showed no significant torsion at zero G in either the upright position or when tilted 30 degrees to right or left. At 1.8 G in the tilted positions they showed greater ocular counterrolling than at 1 G. None of these three subjects became motion sick. The fourth subject showed eye torsion toward his left in all positions at zero G. This leftward bias could also be seen at 1.8 G when tilted left ear down, the side that induces rightward

Space motion sickness during 24 flights of the space shuttle.

The incidence and severity of Space Motion Sickness (SMS) were determined from 24 flights of the Space Shuttle. A standardized questionnaire developed at the NASA-Johnson Space Center (JSC) was administered to all crewmembers postflight during an oral debriefing with the examining flight surgeon. Cases of SMS were graded mild, moderate or severe using criteria developed at the JSC. The incidence of SMS during a first Shuttle flight for 85 crewmembers was 67% (57 cases). There were 26 mild cases (30%), 20 moderate (24%), and 11 severe (13%). Differences were found between males and females, cre

Assisted positive pressure breathing for augmentation of acceleration tolerance time.

The addition of assisted (chest counterpressure), positive pressure breathing (APPB) at 50 mm Hg (PPB50) and 70 mm Hg (PPB70) was compared to the current operational support of the G-suit and the anti-G straining maneuver (AGSM) without PPB (PPB0), during acceleration exposures of sustained 9 +Gz for 45 s and a 5-9 +Gz simulated aerial combat maneuver (SACM) to exhaustion. We selected 7 highly motivated male centrifuge subjects from a group of 31 volunteers. Positive pressure breathing was applied to the oronasal cavity with a Royal Air Force P/Q mask. Mask pressure was monitored continuously.

Instrument flight performance under the influence of certain combinations of antiemetic drugs.

Two different combinations of antiemetic drugs were evaluated using a digital flight simulator. Drug treatments consisted of a lactose placebo, a combination of thiethylperazine (10 mg) and cimetidine (300 mg), and a combination which added promethazine (25 mg) to the two-drug combination. The performance effects of these combinations were evaluated on both a dual task (instrument flight task with the Sternberg Memory Scanning task) and a single task condition (Sternberg task only) for 3 h post drug ingestion. Analysis indicated a significant treatment effect on three of the six flight perform

A near-infrared spectrophotometric method for studying brain O2 sufficiency in man during +Gz acceleration.

A technique for the noninvasive monitoring of cerebral oxygen status was evaluated on volunteer subjects on the USAF School of Aerospace Medicine centrifuge. By using multiwavelength near-infrared spectrophotometry, the instrumentation measured changes in the quantities of reduced and oxygenated hemoglobin (and their sum, an indicator of cerebral blood volume), and the quantity of oxidized cytochrome c oxidase within the forebrain. Tests used acceleration of up to 9 G with onset rates from 0.1 to 5.0 G.s-1, anti-G suits and straining maneuvers, and hyperoxic and hypoxic breathing mixtures. In

Predictive value of the resting electrocardiogram for Mobitz type I atrio-ventricular block on Holter monitoring in Israeli air force personnel.

During 1981-1984, a total of 52 airpersonnel had 24-h continuous ambulatory electrocardiographic (ECG) monitoring at the Israel Air Force Aeromedical Center because of an incidental finding of a first or second degree atrio-ventricular block on a resting 12-lead ECG. There were 230 other airpersonnel without AV block on the resting ECG monitored during the same period. Altogether 17 cases of second degree Mobitz type I (Wenckebach) block were identified. Mobitz type I was detected on Holter monitoring in 2 (0.9%) of the 230 cases with a normal PR interval on the resting ECG, in 6 (15.4%) of 39
