Children with severe developmental disabilities frequently have nutrition and growth problems that range from moderate to severe. Because of notable continuing medical concerns and lowered growth expectations, parents and physicians may fail to recognize gradual deterioration in nutritional status before severe medical complications occur. The two cases reported in this article illustrate the need for early identification and treatment to prevent the development of notable morbidity secondary to malnutrition. Children and adolescents who have growth parameters consistently below age norms requ

This article focuses on nutritional risk factors that predict the development of pressure ulcers in hospital and nursing home patients. Cross-sectional studies associate inadequate energy and protein intake; underweight; low triceps skinfold measurement; and low serum albumin, low serum cholesterol, and low hemoglobin levels with pressure ulcers. Prospective studies identify inadequate energy and protein intake, a poor score on the Braden scale (a risk assessment instrument that includes a nutrition component), and possibly low serum albumin level as risk factors for developing a pressure ulce

The treatment of anemia in patients with renal failure has been dramatically changed with the development of recombinant human erythropoietin (r-HuEPO). This review discusses the pathogenesis of the anemia renal failure and the biology of erythropoietin. Causes of poor response to r-HuEPO therapy are outlined, and the importance of adequate available iron is highlighted. Parameters used to measure iron adequacy include serum iron levels, transferrin saturation, and ferritin levels. Other nutritional deficiencies, such as folic acid and vitamin B-12, can also impair r-HuEPO response. Clearly, t

Magnesium and calcium play multiple dynamic roles in pulmonary structure and function. When magnesium is deficient, the action of calcium is enhanced. In contrast, an excess of magnesium blocks calcium. These interactions are important to the respiratory patient because the intracellular influx of calcium causes bronchial smooth-muscle contraction. The possibility exists that magnesium deficiency contributes to pulmonary complications. During the past few years, there has been an increase in calcium consumption in the US population but little change in magnesium intake, which has caused an imb

Ballet dancers have been shown to have nutrition problems and poor dietary practices. We asked dance instructors and dance directors to distribute a questionnaire to dancers involved in three dance types. Of the 106 dancers who completed the questionnaire, 71% participated in ballet, 60% in jazz, and 74% in modern dance; 31% participated in all three types of dance. The body mass index was similar for all types of dancers. More than half (60%) of the dancers used vitamin and mineral supplements, and 18% of the females reported irregular menstruation. Wide ranges existed in intake of foods from

To meet the objectives for dietary assessment in the Coronary Artery Risk Development in Young Adults (CARDIA) prospective study, we developed a dietary history to provide accurate and reliable quantitative data on habitual individual nutrient intakes at baseline. The CARDIA dietary history was an interviewer-administered method that included a short questionnaire regarding general dietary practices followed by a comprehensive food frequency questionnaire about typical intake of foods using the previous month as a reference for recall. For each broad category of foods, participants were questi

Sixty-four white boys between 10.6 and 14.3 years old participated in an adolescent nutrition assessment study evaluating dehydroepiandrosterone sulfate (DHEAS) as a measure of maturation. DHEAS, an adrenal androgen, is low in childhood and rises with the development of secondary sexual characteristics. Biochemical measures included plasma DHEAS assessed by radioimmunoassay, cholesterol assessed by an enzymatic method, and hemoglobin assessed by the cyanmethemoglobin method. Midarm muscle area (MAMA) was calculated from midarm circumference and triceps fatfold measurements. DHEAS was correlate

Balance (nitrogen, fat, carbohydrate) and growth studies were carried out in nine children (6 to 26 months old) recovering from protein-energy malnutrition (PEM) using a concentrated (1 kcal/mL) formula designed specifically for 1- to 6-year-olds. The formula provided 12% of energy as protein (18% whey, 82% casein), 44% of energy as carbohydrate (69% corn syrup solids, 31% sucrose), and 44% of energy as fat (50% high-oleic safflower oil, 30% soy oil, 20% medium-chain triglycerides). Means +/- standard deviations of apparent nitrogen retention and absorption were 90 +/- 3% and 39 +/- 13%, respe

The element boron is ubiquitous in the environment. Comparatively low concentrations of dietary boron affect several aspects of mineral metabolism in animals and human beings. Therefore, it is appropriate to determine precisely the concentration of boron in human foodstuffs and absorbed, inhaled, or ingested nonfood substances. In this article, we report the analyzed concentrations of boron and other elements in selected foods (animal products, water, condiments, confections, fruits, tuberized roots, vegetables, cereal grains, and spices) and personal-care products (analgesics, antibiotics, de

Individuals with bulimia nervosa have been found to experience a variety of dental complications. Because of purging behaviors and erratic food consumption, the oral manifestations of bulimia frequently include irreversible enamel erosion, xerostomia, and oral mucosal irritation. A pilot study was conducted to determine the effectiveness of the dental/dietitian team in assessing the dental health of individuals with bulimia. Control and bulimic subjects were compared. The results demonstrated a significantly different (p less than .05) intake of folacin-412 micrograms in controls compared with

Low body weight and rapid unintentional weight loss are highly predictive of mortality and morbidity in the elderly population. Weight loss is frequently reported in elderly patients. Acute and chronic diseases are leading causes of involuntary weight loss. Whereas physical disease probably accounts for a majority of cases of involuntary weight loss, psychiatric disorders such as dementia and depression also may result in severe nutritional deficiencies. Additional physiological, psychological, and social factors may affect food intake and body weight. Changes in body composition include loss

Advances in the understanding of amino acid metabolism and of the interaction of amino acids with skeletal muscle, liver, brain, and other tissues have led to refinements of parenteral amino acid solutions. Clinical situations may dictate the use of specific amino acid formulations. Branched-chain amino acid (BCAA) solutions may normalize aberrant amino acid profiles in patients with hepatic encephalopathy; however, controlled trials demonstrate little effect on clinical outcome, and the effectiveness in patients with acute liver failure or undergoing orthotopic liver transplantation is unprov

The nutritional consequences of dementia and the role of diet in the etiology, treatment, and prevention of dementia are the subjects of this review. The major cause of dementia is Alzheimer's disease. Although it has been suggested that aluminum intake may cause this disease, the bulk of scientific evidence suggests that this is unlikely. Dietary supplements of choline and lecithin have been used to treat Alzheimer's disease but are ineffective. Alzheimer's disease patients are at risk of developing protein-energy malnutrition because of poor food intake and increased energy requirements. The

Few studies have examined the long-term effectiveness of behavioral weight control programs. Interpretation of the results from these studies has been limited due to small sample size, use of only one sex, and the number of evaluation parameters. A 2-year follow-up study was designed to assess the effectiveness of a behavioral weight control program on 123 obese male and 386 obese female subjects. Body weights were measured by dietitians at baseline and after an 8-week treatment program. Two-year follow-up weights were self-reported from a mailed questionnaire. The subjects' mean weight at bas
