Perioperative dynamic changes of systemic inflammatory response, gut injury, and hypoxemia in patients with acute type-A aortic dissection: an observational case-control study.

BACKGROUND: The details regarding the pathogenesis of hypoxemia in the presence of acute type-A aortic dissection (ATAAD) remains to be fully elucidated. In this study, we investigated the dynamic changes in systemic inflammatory response, gut injury, hypoxemia, and succinate levels in patients with ATAAD and their impact on perioperative hypoxemia. METHODS: We conducted a single-center, observational, case-control study that enrolled 18 patients with ATAAD who underwent emergency total arch repair (TAR) combined with frozen elephant trunk (FET) procedure under hypothermic lower-body circulat

Clinical features and surgical outcomes of type A intramural hematoma.

BACKGROUND: Data on the clinical features and surgical outcomes of type A intramural hematoma (IMH) in Chinese patients are very limited. We aimed to present the surgical experiences on type A IMH in our center, and report early and late outcomes. METHODS: From February 2012 to April 2018, 106 consecutive patients underwent open surgery for type A IMH at our hospital. We adopted emergent operation for patients with cardiac tamponade or other severe complications, and recommended initial medical treatment followed by elective surgery for stable patients. The composite endpoints included operat

Impact of redo sternotomy on proximal aortic repair: Does previous aortic repair affect outcomes?

PURPOSE: Proximal aortic repair (AoR) in the setting of previous sternotomy may be associated with greater risk than primary repair. Our objective was to determine whether redo sternotomy increases the risk of adverse outcomes following proximal aortic surgery. METHODS: We reviewed all proximal AoRs from 1991 to 2014. Outcomes were compared between first-time AoR (non-redo = 1305) and redo AoRs, which were further classified into 3 categories: (1) previous acute type A aortic dissection (AAD) repair (redo-AAD = 146, 8.3%); (2) previous proximal aneurysm repair (redo-aneurysm = 165, 9.4%); and

Deep hypothermia with circulatory arrest. Determinants of stroke and early mortality in 656 patients.

We have retrospectively evaluated our results after aortic surgery in adults using deep hypothermia with circulatory arrest to determine the patient predictors of early death and postoperative stroke by logistic regression analysis. Of the 656 patients operated on between July 7, 1979, and January 30, 1991, 43% (n = 283) were female, the median age of the patients was 64 years (range 10 to 88 years), 12% (n = 77) had acute dissection, 26% (n = 173) had previously undergone either cardiac or ascending aortic operations, and 13% (n = 85) had a history of cerebrovascular disease. Eighty-four pati

Clinical outcomes of hybrid surgery in the treatment of aortic arch disease: a retrospective study.

BACKGROUND: Hybrid aortic repair (HAR) has emerged as a less invasive alternative to conventional open arch repair for selected patients with aortic arch disease, but its comparative safety and mid-term effectiveness remain uncertain. This study aimed to compare the clinical outcomes of HAR and the frozen elephant trunk (FET) procedure for the treatment of aortic arch disease. METHODS: We conducted a retrospective analysis of 785 patients with aortic arch disease treated at Beijing Anzhen Hospital between January 2015 and February 2023. Among these patients, 406 underwent HAR, which consisted

Hybrid Arch Aneurysm Repair With Ascending Aortic Wrap and TEVAR.

INTRODUCTION: Management of patients with large aortic arch aneurysms who are considered high risk for frozen elephant trunk technique have been challenging, especially when they have a dilated ascending aorta (AA) that precludes total endovascular branched repair (arch BEVAR). A viable option in our armamentarium is wrapping of the AA (AW), and zone 0 Ishimaru TEVAR. METHODS: Retrospective analysis of our aortic database from 2013 to 2024 to select high-risk patients with aortic arch aneurysm that had an AW and TEVAR. We performed CTA analysis before and after wrapping and TEVAR, and last av

Prognostic analysis of acute type A aortic dissection after different surgical interventions: a cohort study.

BACKGROUND: Acute type A aortic dissection (ATAAD) requires emergency surgery, but the choice of primary surgery remains controversial. It is believed that simple ascending aorta replacement may lead to higher postoperative survival rate, while the Sun procedure [frozen elephant trunk (FET) + total arch replacement (TAR)] performed in the first stage may obtain better long-term results. The study aimed to compare the outcome of ATAAD patients who underwent the Sun procedure with those without TAR + FET. METHODS: Data of 452 patients with ATAAD admitted to Beijing Anzhen Hospital for surgical

Ancillary techniques to facilitate endovascular repair of aortic aneurysms.

PURPOSE: The ability to treat abdominal aortoiliac aneurysms and thoracic aortic aneurysms may be limited by coexisting arterial disease. Device deployment may be impaired by occlusive disease and tortuosity of the arteries used to access the aneurysm or by suitability of the implantation sites. In this study we describe the auxiliary procedures performed to circumvent these obstacles and thereby enable endovascular aneurysm repair. PATIENTS AND METHODS: Between January 1, 1993, and December 31, 1999, 390 patients treated for aneurysm of the aorta with endovascular devices were entered prospe

Development and external validation of a nomogram to predict prolonged postoperative mechanical ventilation in patients with acute type A aortic dissection.

BACKGROUND: Prolonged mechanical ventilation (PMV) significantly affects outcomes in patients undergoing acute type A aortic dissection (ATAAD) surgery. This study aimed to develop a nomogram to predict the risk of PMV (defined as mechanical ventilation >48 h) to help clinicians identify high-risk patients and improve outcomes. METHODS: This retrospective study, designed and reported in accordance with the Transparent Reporting of a multivariable prediction model for Individual Prognosis Or Diagnosis (TRIPOD) statement, included 479 ATAAD patients from the First Affiliated Hospital of Anhui M

Six Years' Experience With a Novel Dissection-Specific Stent-Graft to Prevent Distal Stent-Graft-Induced New Entry.

INTRODUCTION: Thoracic endovascular aortic repair (TEVAR) in chronic dissection is associated with a risk for distal stent-graft-induced new entry (dSINE) in up to a quarter of cases. We assess the mid-term outcome of a novel dissection-specific stent-graft (DSSG), which is a custom-made device based on the Cook Alpha platform, with a modified graft design and a distal endovascular elephant trunk without any supporting stent to reduce the radial force on the dissection membrane at the distal landing zone. METHODS: A retrospective single-center study of chronic dissection patients at high risk

Utilization of percutaneous closure devices for large bore arterial access in patients with genetic aortopathy does not result in increased rates of access site complications.

OBJECTIVE: Percutaneous closure devices for arterial sheaths of sufficient caliber to deliver aortic endografts have a published success rate of 90% to 95%. Despite this, they are frequently avoided in patients with genetic aortopathy due to concern for high failure rates and increased complications in the setting of compromised tissue integrity. This study aims to compare rates of access site complications after large bore percutaneous access among patients with and without confirmed genetic aortopathy. METHODS: All patients undergoing endovascular aortic procedures requiring large bore (≥9F

Transesophageal echocardiography-guided algorithm for stent-graft implantation in aortic dissection.

OBJECTIVE: Endovascular stent-graft placement is replacing traditional surgery in type B aortic dissection. Usually stent-graft implantation is performed under fluoroscopic and angiographic guidance, but this relatively new procedure is still burdened with some complications. We investigated the value of a developed algorithm based on transesophageal echocardiography (TEE) for guiding stent-graft implantation in type B aortic dissection. METHODS: Forty-two patients with type B aortic dissection (chronic in 28 patients with aneurysmal dilatation of the false lumen, and acute complicated in 14
