Timing of Anticoagulation after Stroke in Patients with Non-Valvular Atrial Fibrillation Assessment of Provider Practices.

BACKGROUND: Optimal timing of oral anticoagulation (TOAC) in acute ischemic stroke (AIS) in patients with atrial fibrillation (AF) is unknown. The risk of recurrent ischemic events when treatment is delayed is often weighed against that of hemorrhagic transformation (HT) when anticoagulation is started in the subacute phase, especially in moderate to large infarctions. Despite substantial evidence for the benefit of oral anticoagulation (OAC) in reducing stroke recurrence, current nationally recognized practice guidelines do not provide clear recommendations on the TOAC after AF-related AIS.

FIP1L1-PDGFRA-Associated Hypereosinophilic Syndrome as a Treatable Cause of Watershed Infarction.

BACKGROUND AND PURPOSE: Ischemic stroke has been reported in various conditions associated with eosinophilia. FIP1L1-PDGFRA fusion ([Fip1-like 1-platelet-derived growth factor receptor alpha]; F/P) leads to the proliferation of the eosinophilic lineage and thus to a clonal hypereosinophilic syndrome that is highly responsive to imatinib. METHODS: We previously reported on a nationwide retrospective study of 151 patients with F/P-associated clonal hypereosinophilic syndrome. Patients from this cohort with a clinical history of ischemic stroke (as well as 2 additional cases) were further analyz

[The retinal artery occlusion].

Retinal artery occlusion (RAO) remains one of the most vision-threatening conditions in ophthalmology. Most patients experience a dramatic, unilateral and painless loss of vision, which can lead to blindness. No causal therapy has yet been established, which is why there is currently no obligation to treat. Intravenous fibrinolysis therapy is currently being investigated in clinical trials and is the most promising option to date, but its benefits have not yet been proven. This therapy is established for ischaemic stroke but is based on the very narrow 4.5-hour time window which is comparable

Post-Ablation Stroke Despite NOAC Use: Successful Reperfusion Therapy After Dabigatran Reversal with Incidental Discovery of a Large MCA Aneurysm-A Case Report.

Background: Catheter ablation is an established rhythm-control strategy for atrial fibrillation (AF), yet peri-procedural embolic stroke may still occur despite uninterrupted NOAC therapy. Case presentation: A 49-year-old woman on dabigatran developed acute ischemic stroke three days after AF ablation, presenting with left hemiparesis and dysarthria. Idarucizumab (5 g) enabled safe intravenous thrombolysis followed by emergency endovascular thrombectomy (EVT), achieving complete recanalization (mTICI 3). Angiography revealed an incidental 7 mm right MCA aneurysm at the occlusion site. Dabigatr…

Late-onset major depression and subthreshold depressive symptoms as early clinical correlates of brain small vessel disease.

Late-onset depression (LOD) and subthreshold depressive symptoms (SDS) have been associated with cerebral small vessel disease (SVD), but the evidence regarding pre-stroke depression remains limited. This study examined the frequency of DSM-5 LOD and SDS prior to the first non-embolic lacunar stroke. Fifty-eight patients (mean age 62.2 ± 8.2 years; 41.4% female) with a first lacunar ischemic stroke underwent Magnetic Resonance Imaging (MRI) (T1, T2, FLAIR sequences) and psychiatric evaluation using the MINI, with data from caregivers and medical records. Patients were classified as having LOD

Transcervical Versus Transfemoral Approach in Carotid Stenting Real World Experience in a Community Hospital.

INTRODUCTION: Peri-procedural embolic events are the Achilles' heel of carotid stenting. To overcome this complication, transcervical access to the carotid artery was introduced. In this study we describe our "our life" experience with the transcervical approach in a community hospital. MATERIAL AND METHODS: All carotid stent procedures between January 2010 and December 2020 were included in this retrospective analysis. The transcervical approach was compared to the transfemoral approach. In both procedures open-cell, closed-cell design and hybrid stents were used. In-hospital stroke was the

Hemodynamics and oxygen extraction in chronic large artery steno-occlusive disease: Clinical applications for predicting stroke risk.

Depending on the adequacy of collateral sources of blood flow, arterial stenosis or occlusion may lead to reduced perfusion pressure and ultimately reduced blood flow in the distal territory supplied by that vessel. There are two well-defined compensatory mechanisms to reduced pressure or flow - autoregulatory vasodilation and increased oxygen extraction fraction. Other changes, such as metabolic downregulation, are likely. The positive identification of autoregulatory vasodilation and increased oxygen extraction fraction in humans is an established risk factor for future ischemic stroke in so

Swinging subclavian artery web.

Subclavian artery web (SAW) is a rare intimal variant of fibromuscular dysplasia that has been recognized as a potential cause of posterior-circulation stroke. On imaging, SAW typically presents as a thin, shelf-like intraluminal projection that could be misinterpreted as dissection or atherosclerotic plaque. Given the rarity of this entity, the optimal management strategy remains uncertain. Analogous to carotid artery web, stent implantation could resolve underlying hemodynamic disturbances and reduce the risk of embolic events. Herein, we report a case of SAW diagnosed via digital subtractio

A modified squid capture technique for improved laser position during in situ fenestration.

In situ laser fenestration during thoracic endovascular aortic repair is effective and widely utilized, although achieving orthogonal laser alignment can be difficult in patients with complex aortic arch anatomy. A modification of the previously described "squid capture" technique is presented, employing a temporary, removable wire loop to stabilize a sheath against the endograft and facilitate perpendicular laser positioning. This approach minimizes intravascular manipulation, reducing the risk of embolic events, stroke, and arterial injury. A case involving a 60-year-old male with residual d

Aortic Cannulation around Grade 5 Aortic Arch Atheroma Utilizing Transesophageal Echocardiography.

A 61-year-old male with severe aortic valve stenosis was scheduled for a minimally invasive bioprosthetic aortic valve replacement. Intraoperative transesophageal echocardiography (TEE) showed a unicuspid aortic valve and extensive aortic atheromatous disease. A large atheroma with mobile components existed near the distal aortic arch. A 17-French aortic cannula was successfully placed using TEE guidance with the tip proximal to the mobile atheroma to avoid inadvertent disruption and subsequent embolic sequelae. The patient had no evidence of perioperative stroke or other complications postope

E/Ea For The Prediction of Left Atrial Appendage Thrombi in Patients with Atrial Fibrillation and Severe Mitral Stenosis.

Atrial fibrillation (AF), left atrial appendage thrombi (LAAT) and subsequent embolic stroke are the most frequent complications of mitral stenosis .E/Ea is used traditionally for the assessment of left atrium pressure, and besides AF and dilated LA size which promotes stasis of blood in LA, high LA pressure also play a role to clot formation in LA .The receiver operator characteristics (ROC) curve point-coordinates identified an E:e' value of ≥ 36.5to have 57.14% sensitivity and 90.91% specificity for LAAT;We find this ratio relatively specific for prediction of LAAT, independent of MVA and L…

Patent foramen ovale (PFO) closure to prevent recurrent stroke in a patient with a nickel allergy.

The case of a 44-year-old female with multiple embolic cerebellar infarcts associated with a "high risk" patent foramen ovale (PFO) is presented. This article reviews current management of PFO in association with cryptogenic stroke in light of the results of three recent randomized controlled trials and two meta-analyses of observational data. The article will also discuss circumstances which merit consideration for closure despite the negative trial data, and will review our management strategy for closure in this patient with a nickel allergy that precluded the use of the most commonly used

Carotid artery stenting using proximal balloon occlusion embolic protection.

Carotid artery stenting has rapidly grown as an alternative to carotid endarterectomy for stroke prevention among selected patients with extracranial carotid artery stenosis. Development of mechanical embolic protection devices (EPDs) has been associated with improved clinical outcomes and is now a strongly advocated adjunct to the procedure. Characteristically, EPDs have been broadly defined into 3 primary categories, of which the distal filter elements have largely been the most developed and used. Improvements among the class of proximal balloon occlusion devices with flow reversal have a n

Diagnosis and management of typical atrial flutter.

Can "past decade" be rephrased to refer to more specific years? Typical atrial flutter (AFL) is a common atrial arrhythmia that may cause significant symptoms and serious adverse effects, including embolic stroke, myocardial ischemia and infarction, and, rarely, a tachycardia-induced cardiomyopathy resulting from rapid atrioventricular conduction. As a result of the well-defined anatomic and electrophysiologic substrate and the relative pharmacologic resistance of typical AFL, radiofrequency catheter ablation has emerged since its first description in 1992 as a safe and effective first-line tr…

Angiography of nonhemorrhagic cerebral infarction in young adults.

The radiographic examinations and hospital records of 93 young adult patients (15-45 years of age) with nonhemorrhagic cerebral infarction evaluated at our institution during the past 9 years were reviewed. The angiographic examinations were abnormal in 76% of patients. The most common abnormalities were embolic disease and atherothrombotic disease. Forty-seven patients underwent angiography within 7 days of their event. There were no major neurologic or systemic complications related to early angiography. We believe that angiography performed early in the course of the illness is a high-yield

Vertebro-basilar ischaemia with thrombosis of the vertebral artery: report of two cases with embolism.

Two cases with vertebro-basilar infarcts associated with severe stenosis of the vertebral artery in its cervical part is reported. Complete thrombosis of the vertebral artery was observed after a short delay in both cases. As the carotid arteries were normal and the controlateral vertebral artery was dominant, an embolic mechanism was suspected. No new event occurred in the following one and three years respectively after the stroke. These two cases demonstrate the possibility of embolism from severe stenosis of the cervical vertebral artery. This mechanism could explain some of the infarcts r
