Early palliation of univentricular hearts with subaortic stenosis and ventriculoarterial discordance. The arterial switch option.

The optimal Fontan-type operation greatly depends on appropriate initial palliation. Several surgical techniques have been used in infancy to palliate patients with univentricular hearts, ventriculoarterial discordance, and subaortic stenosis. The two most common are pulmonary artery banding and Damus-Norwood procedures. Palliative arterial switch operation is another surgical option that was used in this early series of seven infants. The principle of this operation is to "switch" the subaortic obstruction into a subpulmonary obstruction; the coronary artery relocation on the large pulmonary

Microcirculatory Alterations and Endothelial Glycocalyx in Fontan Circulation: Association With Central Venous Pressure.

OBJECTIVE: Fontan circulation is characterized by chronically elevated central venous pressure (CVP) and reduced cardiac output, which may adversely affect the systemic microcirculation. We investigated whether Fontan circulation is associated with alterations of the endothelial glycocalyx. METHODS: Forty-four patients with congenital heart disease undergoing postoperative endothelial glycocalyx evaluation were included. The perfused boundary region (PBR), an inverse indicator of glycocalyx dimensions, was measured noninvasively using GlycoCheck. PBR was compared between patients with Fontan

Percutaneous Interventions for Heart Failure in Congenital Heart Disease.

PURPOSE OF REVIEW: Heart failure (HF) is a major cause of morbidity and mortality in patients with congenital heart disease (CHD), driven by residual lesions and progressive ventricular dysfunction. The heterogeneity of CHD anatomy and physiology often limits the applicability of standard HF therapies. This review aims to summarize contemporary catheter-based strategies used to diagnose, stabilize, and treat HF in patients with CHD across both acute and chronic clinical settings, and to evaluate their role in improving outcomes and delaying surgical or transplant-based therapies. Uhl's anomaly after Glenn shunt - clinical image of a rare congenital heart defect.

Uhl's anomaly is an extremely rare congenital heart defect, with approximately 100 cases reported since its first description in 1952 by Dr Henry Uhl. Characterized by partial or complete absence of the right ventricular myocardium, it leads to progressive right ventricle dilation and right heart failure. Due to its rarity, there is no consensus regarding its surgical or medical management. We present a case of an 18-year-old male patient with Uhl's anomaly diagnosed in early infancy, managed by palliative Glenn shunt. This report provides insight into management of this rare condition.

Transhepatic Left Bundle Branch Area Pacing in a Patient With Ebstein Anomaly and Prior Glenn Surgery: A First Report.

BACKGROUND: Venous access exhaustion poses a significant challenge for the implantation of intracardiac devices. Epicardial alternatives are often used in these cases, despite their long-term deleterious effects. This case presents a transhepatic approach for left bundle branch pacing in a patient with Ebstein anomaly and a prior Glenn procedure. METHODS: Following pre-procedural evaluation with triphasic CT, ultrasound-guided hepatic vein puncture was performed. Guidewires were advanced into the right heart chambers, allowing passage of a delivery sheath for physiologic pacing. Atrial lead p

Treatment of hepatocellular carcinoma in patients with Fontan-associated liver disease with radioembolisation therapy.

This case report describes two patients with congenital heart disease (CHD) who underwent Fontan palliation surgery in their youth and subsequently developed Fontan-associated liver disease (FALD) in adulthood. While undergoing routine screening for hepatocellular carcinoma (HCC), both patients had significantly elevated serum alpha-fetoprotein testing and diagnostic radiographic findings consistent with early-stage HCC. At the same academic medical centre, both patients were treated with localised yttrium-90 (Y90) radioembolisation therapy, and neither underwent liver transplantation (LT). Af

In Silico Evaluation of a Self-powered Venous Ejector Pump for Fontan Patients.

PURPOSE: The Fontan circulation carries a dismal prognosis in the long term due to its peculiar physiology and lack of a subpulmonic ventricle. Although it is multifactorial, elevated IVC pressure is accepted to be the primary cause of Fontan's high mortality and morbidity. This study presents a self-powered venous ejector pump (VEP) that can be used to lower the high IVC venous pressure in single-ventricle patients. METHODS: A self-powered venous assist device that exploits the high-energy aortic flow to lower IVC pressure is designed. The proposed design is clinically feasible, simple in st

Sex-Specific Cerebral Blood Flow Alterations in Youth Operated for Congenital Heart Disease.

Background Lower cerebral blood flow (CBF) has previously been documented preoperatively in neonates with congenital heart disease (CHD). However, it remains unclear if these CBF deficits persist over the life span of CHD survivors following heart surgery. When exploring this question, it is critical to consider the sex differences in CBF that emerge during adolescence. Therefore, this study aimed to compare global and regional CBF between postpubertal youth with CHD and healthy peers and examine if such alterations are related to sex. Methods and Results Youth aged 16 to 24 years who underwen

Clinical and invasively-measured predictors of high exercise capacity in Fontan patients.

INTRODUCTION: Fontan patients have variable exercise capacity. Contemporary understanding as to which factors predict high tolerance is limited. METHODS: Records from the Ahmanson/University of California, Los Angeles Adult Congenital Heart Disease Center were reviewed for adult Fontan patients who underwent CPET. Patients were considered "high performers" if their maximum oxygen uptake (VO2 max/kg)-predicted was greater than 80%. Cross-sectional clinical, hemodynamic, and liver biopsy data was gathered. High-performers were compared to control patients across these parameters via association

Scaling Peak Oxygen Consumption for Body Size and Composition in People With a Fontan Circulation.

Background Peak oxygen consumption (peak V̇O2$$ \dot{\mathrm{V}}{\mathrm{O}}_2 $$) is traditionally divided ("ratio-scaled") by body mass (BM) for clinical interpretation. Yet, it is unknown whether ratio-scaling to BM can produce a valid size-independent expression of peak V̇O2$$ \dot{\mathrm{V}}{\mathrm{O}}_2 $$ in people with a Fontan circulation. Furthermore, people with a Fontan circulation have deficits in lean mass, and it is unexplored whether using different measures of body composition may improve scaling validity. The objective was to assess the validity of different scaling denomin

The impact of additional antegrade pulmonary blood flow at bidirectional Glenn shunt on long-term outcomes.

OBJECTIVES: We investigated the impact of additional antegrade pulmonary blood flow on the long-term outcomes after bidirectional Glenn shunt. METHODS: From 2001 to 2015, 279 patients underwent bidirectional Glenn shunt as an interim palliation for a functionally single ventricle. After excluding patients with a previous Kawashima or Norwood operation, 202 patients with preexisting antegrade pulmonary blood flow before bidirectional Glenn shunt were included in this study. Antegrade pulmonary blood flow was eliminated in 110 patients (no antegrade pulmonary blood flow group) and maintained in

Use of Pulmonary Arterial Hypertension Therapies in Patients with a Fontan Circulation: Current Practice Across the United Kingdom.

Background The Fontan circulation is a successful operative strategy for abolishing cyanosis and chronic volume overload in patients with congenital heart disease with single ventricle physiology. "Fontan failure" is a major cause of poor quality of life and mortality in these patients. We assessed the number and clinical characteristics of adult patients with Fontan physiology receiving pulmonary arterial hypertension (PAH) therapies across specialist centers in the United Kingdom. Methods and Results We identified all adult patients with a Fontan-type circulation under active follow-up in 10

Augmentation of Pulmonary Arterial Growth in Single Ventricle Patients by Interim Selective Shunts.

Stenosis or diffuse hypoplasia of central pulmonary arteries (PA) is common in patients with single ventricle physiology, often requiring surgical patching. Such repairs are prone to failure, particularly with low pressure venous flow (bidirectional cavopulmonary connection or Fontan). We describe our experience of disconnection of central PA and selective systemic-PA shunt to the hypoplastic vessel. Single ventricle patients (n = 12) with diffuse left pulmonary artery (LPA) hypoplasia (LPA:right pulmonary artery diameter <0.7) underwent PA disconnection (ligation clip) and selective arterial
