Dissemination of wear particles to the liver, spleen, and abdominal lymph nodes of patients with hip or knee replacement.

BACKGROUND: The importance of particles generated by wear and corrosion of joint replacement prostheses has been understood primarily in the context of the local effects of particle-induced periprosthetic osteolysis and aseptic loosening. We studied dissemination of wear particles in patients with total hip and knee replacement to determine the prevalence of and the histopathological response to prosthetic wear debris in the liver, spleen, and abdominal para-aortic lymph nodes. METHODS: Postmortem specimens from twenty-nine patients and biopsy specimens from two living patients with a failed

Adverse reactions to injectable soft tissue fillers: memorable cases and their clinico-pathological overview.

BACKGROUND: Filler injection is a minimally invasive procedure widely used for soft tissue augmentation. Although the safety profile is favorable, adverse events can occur, especially after illegal filler injection. OBJECTIVES: The authors present memorable cases of filler complications and review their clinico-pathological features and treatment strategies. PATIENTS AND METHODS: This is a retrospective, single-center case series. The authors identified eight patients with significant complications following filler injection. A medical record review was performed for clinical history, histop

[Foreign body in the urinary bladder after vascular surgery].

INTRODUCTION: We are presenting a rare complication after femorofemoral bypass surgery. CASE REPORT: In a 61-year-old male patient, a femorofemoral crossover bypass graft was inadvertently placed through the urinary bladder. Postoperatively, the patient developed macrohematuria, which cleared spontaneously. The diagnosis of an intravesical graft was made 3 months after surgery by cystoscopy performed because of dysuria. The patient underwent open bladder surgery 7 months later as he refused earlier intervention. The misplaced graft, which was not infected and showed good function and perfusio

[Intravesical foreign bodies. Review of the literature].

We want to make a literature review about the bladder foreign bodies. This clinical situation is not an usual emergency, but there are a lot of references in urological articles about it. In this article review we want to make a compilation about the symtoms, the different origins of the foreign bodies, and ways of diagnosis and treatment of this pathology. We also want to add our personal experience in a recent case.

The use of silastic as an orbital implant for reconstruction of orbital wall defects: review of 311 cases treated over 20 years.

PURPOSE: A retrospective review of silicone rubber (Silastic; Dow Corning, Midland, MI) implants placed in orbits was undertaken. These implants were used to reconstruct defects in the orbital floor and/or walls secondary to trauma, or those created during malar or orbital osteotomies. The purpose of the study was to determine the incidence of removal of these implants from the surgical site. MATERIALS AND METHODS: The records of 311 patients treated over a 20-year period were reviewed. Of these, 302 had received silastic implants secondary to trauma. RESULTS: Forty-one patients (13%) had th

Late migration of two covered biliary stents through a spontaneous bilioenteric fistula in a patient with malignant biliary obstruction.

We report a case of simultaneous late migration of two ePTFE-FEP covered biliary endoprostheses (Viabil, W.L. Gore, Flagstaff, AZ, USA) that were percutaneously implanted for the treatment of malignant obstructive jaundice. The first Viabil covered stent was placed successfully without any evidence of dislocation or other complication during follow-up. Occlusion of the stent occurred 4 months later and was treated with the placement of a second stent of the same type. Thirteen months later the patient became symptomatic. Percutaneous transhepatic cholangiography (PTC) revealed the presence of

Ureter reconstruction using a biotube in a canine model: A pilot stud.

INTRODUCTION: Ureteral injuries can lead to renal dysfunction, and conventional treatments such as ureteral stents and autologous tissue grafts have limitations. This study aimed to evaluate the biocompatibility of a "biotube" created using in-body tissue architecture (iBTA) technology for ureteral reconstruction in a canine model. METHODS: Biotubes were implanted in dogs to replace a segment of the ureter. Ureteral stents were used to prevent obstruction. Autopsies were performed 2-3 months after implantation, and the implantation sites were examined histologically to assess tissue regenerat

Clinicopathological Analysis of 90 Cases of Polyacrylamide Hydrogel Injection for Breast Augmentation Including 2 Cases Followed by Breast Cancer.

BACKGROUND: Although polyacrylamide hydrogel (PAAG) injection for breast augmentation has been prohibited for many years, the long-term complications will be significant for a long period. Few research articles have focused on the clinicopathological analysis. MATERIALS AND METHODS: We summarized clinical and pathological features of 90 cases after PAAG-injected breast augmentation, including 2 cases of breast cancer developed following PAAG injection. RESULTS: All patients were females between the ages of 30 and 64 years (mean, 44 years). The complications included masses (75.58%), pain (45

Surgical management of a brain abscess due to plant foreign body in a dog.

BACKGROUND: Intracranial abscesses as a result of grass awn migration have been rarely described in the veterinary literature. The identification of their radiological features is mandatory for proper diagnosis. As occurs with abscesses in other organs, surgical drainage and directed antibiotic therapy should be considered the treatment of choice. CASE DESCRIPTION: A clinical case of a Great Dane dog with forebrain signs and magnetic resonance imaging and computed tomography findings compatible with intracranial abscess associated with inflammatory changes in orbital musculature is described.

Ultrasound biomicroscopy and management of late posterior migration of a ripcord suture after glaucoma drainage implant surgery.

Three years after receiving a Baerveldt tube implant for neovascular glaucoma, a 50-year-old man presented to the Wills Eye Hospital Glaucoma Service with pain and decreased vision in his left eye. On examination, a 4-0 nylon ripcord suture was seen to have migrated posteriorly through the pupil and in front of the intraocular lens. The distal end of the suture was not seen on dilated indirect ophthalmoscopic examination or by standard B-scan ultrasonography. Ultrasound biomicroscopy examination was performed, and the distal end of the suture was found to lie just anterior to the pars plana. C

Scleral loop fixation for posteriorly dislocated intraocular lenses. Operative technique and long-term results.

Surgical management of a posteriorly dislocated intraocular lens requires vitrectomy techniques to remove adherent vitreous and prevent traction on the retina. The lens implant may be repositioned in the posterior chamber by ciliary sulcus fixation or by transscleral or iris suture fixation. Previous suture fixation techniques require multiple needle penetrations through the sclera, cornea, or both. We described a simplified technique for repositioning a posteriorly dislocated intraocular lens in which sclerotomy incisions are utilized for fixation. Six patients are described, with long-term f

Bladder Foreign Body of Gastrointestinal Origin: A Case Report and Literature Review.

BACKGROUND: Gastrointestinal-origin urinary foreign bodies are rare and often overlooked, particularly because early Computed Tomography (CT) scans may be inconclusive, especially in cases involving wooden materials. CASE PRESENTATION: A 19-year-old man presented with lower-abdominal pain and gross hematuria one week after accidentally ingesting a toothpick. Initial local CT scans were negative. At our center, bedside ultrasonography revealed a linear hyperechoic tract crossing the bladder wall, while thin-slice non-contrast CT only detected a punctate hyperdensity at the bladder dome. Cystos

A pen in the liver.

A 24-year-old woman with anxiety, depression, and emotionally unstable personality disorder was referred to a tertiary center 2 weeks after ingesting multiple foreign bodies. She had undergone a laparoscopic cholecystectomy and a laparotomy for extraction of ingested foreign bodies several years ago. A sagittal CT scan view showed a ballpen and a hair clip in the stomach. A coronal view demonstrated that a second ballpen had penetrated the duodenal wall to enter the liver parenchyma. There was no free intraperitoneal air or fluid or evidence of abscess formation. At laparotomy, a toothbrush, a

Intra-abdominal propylene suture fragment leading to complete small bowel obstruction: A case report.

INTRODUCTION: Propylene sutures are non-absorbable sterile surgical sutures, which when present intra-abdominally act as a foreign body leading to adhesion formation. Post-operative adhesions are a common cause of bowel obstruction, with complete closed loop obstruction presenting as a surgical emergency. CASE DETAILS: A 66-year-old man who had undergone a laparotomy for bowel obstruction two years back, presented to our emergency department with features of complete bowel obstruction. Midline laparotomy revealed propylene suture and intra peritoneal adhesions acting as a constricting band ar

An Unusual Complication of Bone Wax Utilization.

Bone wax is a commonly used hemostatic agent with minimal complications. Some of the known complications include inflammation, granuloma formation, infection, and impaired osteogenesis. Several clinical reports of bone wax migration have also been reported. In this paper, the authors present a rare patient of bone wax migration intracranially in a 6-year-old patient who initially underwent craniotomy for the evacuation of subdural hematoma and repair of depressed skull fracture. The patient then underwent craniotomy scalp scar revision several months later. Postoperatively he developed short-t

Oxinium femoral head damage generated by a metallic foreign body within the polyethylene cup following recurrent dislocation episodes.

Oxinium femoral heads are supposed to be more scratch-resistant thanks to their oxidized layer. However, damages to this thin layer can jeopardize implant's properties. Following revision total hip arthroplasty performed for recurrent posterior dislocations, the Oxinium femoral head initially implanted was observed to be dramatically damaged. A metallic foreign body from a trochanteric fixation wire was found within the polyethylene cup. Only few cases of damaged Oxinium femoral heads have been reported and all were related to either dislocation or reduction of THA. The aim of this report is t
