Superior oblique muscle palsy after frontal sinus mini-trephine.

OBJECTIVE: The aim of this study is to present a case of superior oblique muscle dysfunction after a frontal sinus mini-trephine. METHODS: This is a case report of an 18-year-old woman where a mini-trephination approach and endoscope were used to open and marsupialize a symptomatic, opacified type IV cell within the left frontal sinus. After surgery, the patient developed a persisting diplopia; a left superior oblique muscle palsy was diagnosed. Nine cadaveric dissections of the trochlea were undertaken to clarify mechanisms for potential trochlear damage. RESULTS: Cadaveric dissection revea

[Frontobasal fractures in children].

In 1987, four children between 5 and 13 years of age with severe frontobasal trauma and dural tears were treated operatively. A CSF rhinorrhea was manifest clinically in only two cases. In addition high-resolution computerised tomography was essential in diagnosis and planning of the operation. In each of the cases an intracranial pneumatocele indicating dural laceration was shown. The fractures were confirmed during surgery in: Case 1. The roof of the ethmoid sinus and the roof of the orbit. Case 2. Both the sphenoid sinuses. Case 3. The roof of the ethmoid sinus and the posterior wall of the

Orbital Rim Sparing Single-piece Fronto-orbital Keyhole Craniotomy Through Eyebrow Incision: A Technical Report and Comparative Review.

BACKGROUND: The classical eyebrow supraorbital keyhole craniotomy has limited working volume due to the thick incurving of the frontal bone necessitating generous drilling of the frontal base. However, the basal variant with sectioning of the orbital rim results in increased periorbital edema. OBJECTIVE: We describe a novel orbital rim sparing single-piece fronto-orbital keyhole approach, probably the first such report with a comparative review of relevant literature. METHODS: Following eyebrow incision, bidirectional drilling with a ball tip behind the fronto-zygomatic suture exposes the pe…

Surgical Obliteration of Anterior Cranial Fossa Dural Arteriovenous Fistulas via Unilateral High Frontal Craniotomy.

BACKGROUND: Surgical obliteration is generally recommended for anterior cranial fossa dural arteriovenous fistulas (ACF DAVFs) because of a high risk of bleeding and the difficulty of endovascular approaches. Surgical obliteration is generally performed via a frontobasal craniotomy; however, it is slightly excessive over the target fistula. Here, we present 2 cases of ACF DAVFs treated with small craniotomy without frontal sinus involvement and a review of the related literature. METHODS: We present 2 cases including a 63-year-old woman who presented with a right-sided ACF DAVF that was fed b

Comparison of osteotomy technique and jig type in completion of distal femoral osteotomies for correction of medial patellar luxation. An in vitro study.

OBJECTIVES: Femoral osteotomies are frequently completed to correct malalignment associated with patellar luxation. The objectives of this study were to compare the use of: 1) two different types of jig; and 2) different types of osteotomy in the realignment of canine femoral bone models which possessed various iterations of angular deformity. METHODS: Models of canine femora possessing distal varus, external torsion and a combination of varus and torsion underwent correction utilizing two alignment jigs (Slocum jig and Deformity Reduction Device) and either a closing wedge ostectomy (CWO) or

[Drainage of the Frontal Sinus to Cure Epidural and Brain Abscesses Developed after Surgery via Anterior Interhemispheric Approach].

We report a woman whose anterior communicating artery (AcomA) aneurysm was clipped via an anterior interhemispheric approach when she was 49 years old. At the age of 51, she presented with a subcutaneous abscess and osteomyelitis, so the cranioplastic bone was removed. Six months later, she underwent cranioplasty using hydroxyapatite. Her subcutaneous abscess recurred and the epidural abscess and hydroxyapatite were removed 11 years later after the first operation. The patient underwent observation therapy for the next 4 years, as the dura was ossified. She presented with frontal swelling 15 y

The hourglass facial deformity as a consequence of orbital irradiation for bilateral retinoblastoma.

We report a case illustrating characteristic facial deformities following irradiation during infancy for bilateral retinoblastoma. CT and MR imaging revealed distinctive features of the resulting "hourglass facial deformity": hypotelorism, enophthalmos, depressed temporal bones, atrophy of the temporalis muscles, and a depressed nasion. This case also displayed premature metopic craniosynostosis and frontal sinus aplasia; these may be additional hallmarks of this deformity.

Frontobasal approach for trauma and tumor.

The potential for remediation of a host of varied disease processes afflicting the skull base has been increasingly realized over the past two decades. Advances in medical technology, the development of new approaches, and most importantly the close cooperation of the head and neck surgeon and the neurological surgeon have made this possible. Surgeons are attempting to diminish the injury to normal un-involved structures while at the same time maximizing exposure and enabling rectification of the disease or disorder. This is especially pertinent in a number of frontobasal approaches used in an

Fronto-orbital feminization technique. A surgical strategy using fronto-orbital burring with or without eggshell technique to optimize the risk/benefit ratio.

The demand for facial feminization is increasing in transsexual patients. Masculine foreheads present extensive supraorbital bossing with a more acute glabellar angle, whereas female foreheads show softer features. The aim of this article is to describe our surgical technique for fronto-orbital feminization. The mask-lift technique is an upper face-lift. It provides rejuvenation by correcting collapsed features, and fronto-orbital feminization through burring of orbital rims and lateral canthopexies. Depending on the size of the frontal sinus and the thickness of its anterior wall, frontal rem

Cleidocranial dysplasia: craniofacial morphology in adult patients.

The aim of the study was to carry out a detailed quantitative analysis of craniofacial morphology in a relatively large sample (N = 35) of adult patients with cleidocranial dysplasia (CCD) in order to contribute to the understanding of the characteristic craniofacial abnormalities. Roentgencephalometric films in the lateral, frontal, and basal projections were analyzed and mean facial diagrams for the lateral and frontal films were produced based on registration of 216 reference points. In the calvaria an increased width and a relatively more prominent frontal region were found. In the cranial

[A case of brain metastasis of rhabdomyosarcoma in a child].

We report here a specific case of metastatic brain tumor orienting from rhabdomyosarcoma. An 11-year-old boy came to our hospital with complaints including headache, exophthalmos and tumors in the right frontal and left occipital region. 6 months previously subtotal removal of rhabdomyosarcoma in his right foot had been performed. CT scan demonstrated tumors in the right orbit, right frontal region and left occipital region. The tumors existed bilaterally in the skull and markedly enhanced by contrast medium. But the changes of the cranium were mild, only the dipole was slightly dilated. MRI s

Silent danger behind the forehead: Pott's puffy tumour presenting without systemic symptoms.

Pott's puffy tumour (PPT) is a rare complication of frontal sinusitis involving subperiosteal abscess and frontal bone osteomyelitis, often leading to serious intracranial sequelae. We report a case of a man in his 50s who presented with progressive, painless forehead swelling despite multiple antibiotic courses. Imaging confirmed bilateral subperiosteal collections and a right-sided epidural empyema. Initial management included broad-spectrum antibiotics, endoscopic sinus surgery and external drainage. Due to persistent collections, a second-stage osteoplastic approach was performed, resultin…

Combined endoscopic trephination and endoscopic frontal sinusotomy for management of complex frontal sinus pathology.

BACKGROUND: The advances in endoscopic sinus surgery have revolutionized the management of frontal sinus disease. Despite the successes, the purely endoscopic approach has its limitations, especially in patients with alterations in anatomy caused by previous surgical intervention or complex frontal sinus pneumatization patterns. The purpose of this study was to evaluate the efficacy of combined endoscopic trephination and endoscopic frontal sinusotomy (the above and below approach) in the management of these difficult cases. METHODS: Chart review was performed on patients undergoing the combi

[20-year experience in obliteration of the frontal sinuses with formalinized allobrephobone in chronic frontal sinusitis].

Outcomes of surgical treatment of chronic recurrent frontal sinusis (FS) using obliteration of the frontal sinuses with allobrephobone in 228 patients (190 males and 38 females aged 15-71 years) are presented. 45 of them had undergone surgery on the frontal sinuses with reestablishment of the frontonasal shunt. Obliteration consists in removal granulations and polyps from the frontal sinus through the anterior wall followed by sinus mucosa scraching and the sinus separation from the nasal cavity with filling with small pieces of formalized allobrephobone. The technique is simple, postoperative

Use of titanium mesh for reconstruction of extensive defects in fronto-orbito-ethmoidal fracture.

Traumatic injuries treatment of the fronto-naso-orbito-ethmoidal region has been one of the most challenging treatments within maxillofacial surgery, particularly of extensive orbital defects, very common in this type of pathologic condition. A 48-year-old man involved in a car collision presented an extensive bilateral fracture of the orbit medial wall, nasal bones, the nasal septum, and the frontal anterior table. The clinical and tomographic findings concluded the diagnosis of a maxilla and fronto-naso-orbito-ethmoidal fracture. Among the variety of biomaterials, the titanium mesh was elect
