Clinical and radiographic delineation of odontochondrodysplasia.

The association of dentinogenesis imperfecta (DI) with a distinct form of chondrodysplasia in a boy was reported by Goldblatt et al. [1991; Am J Med Genet 39:170-172] and has been given the name of Goldblatt syndrome or odontochondrodysplasia (ODCD; OMIM#184260). Since the original description, only four further individuals have been reported (one sib pair and two unrelated cases). We report on an additional six individuals, including a second sib pair (brother and sister), with clinical and radiographic features that cluster and thus confirm the nosologic status of this entity. The main radio

Bridge plating of the wrist for temporary stabilization of concomitant radiocarpal, intercarpal, and carpometacarpal injuries: a report of two cases.

Concomitant injuries of the radiocarpal, intercarpal, and carpometacarpal joints are rare and usually result from very high-energy trauma. The skeletal injury is often accompanied by severe soft tissue trauma. Repair of the radiocarpal and intercarpal ligaments needs to be augmented with immobilization. Traditional methods of immobilization, such as casts and external fixators have limitations. We describe our experience in 2 patients using a technique in which a wrist arthrodesis plate is temporarily placed from the radius to metacarpal to span the carpus and protect the radiocarpal and inter

[Asphyxiating thoracic dysplasia or Jeune's syndrome. Description of 2 mild familial cases].

Asphyxiating thoracic dystrophy or Jeune's syndrome is characterized by respiratory distress, osseus dysplasia and short stature. Kozlowski and Masel have described four groups: lethal, severe, mild and latent. The mild form is generally characterized by long, narrow thorax with a reduced thoracic cage capacity, short stature, sometimes short fingers, and progressive renal involvement. Differential diagnosis can be difficult in some cases with atypical clinical manifestations. The Authors report on two cases of the mild form of Jeune's syndrome in two sibs without renal involvement and respira

Two isolated cases with symmetrically absent hands and feet.

We describe two unrelated boys with similar bilateral terminal transverse defects of the upper and lower limbs without other anomalies. Review of the literature did not reveal another report of this distinctive type of malformation.

[Reconstructive surgery of malunited joint fractures of the hand].

Malunion of fractures, joint fractures and pseudoarthroses leads to therapy-resistant pain during function and loading, especially in the wrist, with a risk of early arthrosis. In spite of the extraordinary adaptability of the hand, malunion in the axis and rotation can lead, for example, to cross-finger malfunction. Because of the relatively thin layer of soft tissue, secondary operations to correct the axis, rotation and the length of the phalanx are possible. Each reoperation on the hand diminishes the blood flow, and the anatomical structures change with cicatrices. Thus, the risk of damag

[Differential therapy of osteoporosis--an overview based on recent findings regarding the pathogenesis].

Osteoporosis had long been considered as an unavoidable consequence of aging for which no prevention was possible, whereas it has recently been recognized as a disease that can be prevented and treated. It is of outstanding importance to choose a therapy tailored to the individual patient based on new findings on the pathogenesis of the different types of osteoporosis and the differential diagnosis related to these. During phases of rapid bone loss, usually at the beginning of postmenopause, but sometimes also in older patients, it is useful to use therapeutic drugs to slow down the high bone

[Advances in the molecular genetics of brachydactyly].

Brachydactyly (BD) is a general term that refers to shortening of the hands/feet due to small or missing metacarpals/metatarsalsand/or phalanges, and forms part of the group of limb malformations characterized by bone dysostosis. It may occur either as an isolated trait or as part of a syndrome. BD may also be accompanied by other hand mal-formations, such as syndactyly, polydactyly, reduction defects, and symphalangism. In isolated brachydactyly, the inheritance is mostly autosomal dominant with variable expressivity and penetrtance. For the majority of isolated BD and some syndromic forms of

Erosive osteoarthritis.

Erosive osteoarthritis (EOA) represents a subset of symptomatic osteoarthritis of hand, characterized by intermittent and often frequent inflammatory episodes and progressive joint damage. A greater degree of inflammation and the presence of subchondral bone erosions on plain radiographs help distinguish EOA from generalized osteoarthritis of hand. High resolution ultrasound and magnetic resonance imaging (MRI) have demonstrated the presence of synovitis and MRI has additionally highlighted the role of inflammation in bone, tendons, and ligaments in EOA. Further evidence for the crucial role o

Pitfalls in scoring MR images of rheumatoid arthritis wrist and metacarpophalangeal joints.

This paper outlines the most important pitfalls which are likely to be encountered in the assessment of magnetic resonance images of the wrist and metacarpophalangeal joints in patients with rheumatoid arthritis. Imaging artefacts and how these can be recognised using various sequences and views are discussed. Normal structures such as interosseous ligaments and nutrient foramina may appear prominent on certain images and need to be identified correctly. Pathological change in the rheumatoid hand involves many tissues and when substantial damage has occurred, it may be difficult to identify in

Pseudomalignant myositis ossificans of the wrist causing compression of the ulnar nerve and artery. A case report.

Myositis ossificans (MO) is a condition characterised by focal, benign and self-limited idiopathic heterotopic bone formation. It is extremely rare in the hand and wrist and may lead to concomitant nerve compression. Because of the rare incidence of pseudomalignant MO at the wrist and hand, we found it of interest to report a case of this condition localised to the wrist. A 31-year-old female patient presented with swelling and pain of her left wrist. The physical examination findings, magnetic resonance imaging and Tc-99m bone scan suggested acute osteomyelitis or a tumoral condition. Incisio

[Radiotherapy for myeloma].

The quality of life, as well as the length of survival, in patients with myeloma has improved since the introduction of melphalan chemotherapy. Although myeloma is a radio-responsive disease, radiotherapy should be chosen for relapsed and refractory myeloma, because prolongation of overall survival is not improved. The aim of radiotherapy for solitary plasmacytomas is to mainly relieve pain, rather than the curative local therapy. On the other hand, total body irradiation has been used, as palliative therapy, for relapsed and refractory myeloma but treatment related toxicity is commonly. The h

[Problems in the surgical technic used in free toe transfer].

Based on the experiences of sixteen toe-to-hand transfers (nine for thumb reconstruction, seven for finger replacement), some technical problems of the operation are discussed. On the foot the incision should be planned in such a way that the wound can be closed directly to achieve a better aesthetic result The excision of the distal third of the second metatarsal bone marrows the foot, which improves the later appearance. On the hand deeper skin defects with exposed bone are easily closed by a forearm-island flap based on the distal radial artery. The rotation of the head of the transferred m

UCLA Conference. Diagnosis and treatment of erosive rheumatoid arthritis and other forms of joint destruction.

Single or multiple destruction of joints arises from many causes, ranging from the acute destructive process (usually monarticular) in septic joint disease to the chronic, slowly evolving destruction of cartilage and adjacent bone that occurs in osteoarthritis. Medical treatment of a few of these conditions (including rheumatoid arthritis) is reasonably satisfactory if it is initiated early. On the other hand, surgical procedures are indicated if physical and X-ray examination of the involved joints discloses the presence of a significant destructive process involving both articular cartilage

Conventional radiography of the hands and wrists in rheumatoid arthritis. What a rheumatologist should know and how to interpret the radiological findings.

Rheumatoid arthritis (RA) is a chronic inflammatory disease affecting the synovial membrane, leading to joint damage and bone destruction. Conventional radiography (CR) of the hands and wrists has been, for many years, the primary imaging modality used to diagnose and monitor RA. On the other hand, many investigators in clinical trials and observational studies used CR of the hands and wrists to demonstrate drug effectiveness and structural damage progression. The purpose of this review is to discuss the evaluation and interpretation of the hands and wrists by CR in RA patients and the radiogr
