Intrapartum electronic fetal heart rate monitoring and the prevention of perinatal brain injury.

OBJECTIVE: Electronic fetal heart rate monitoring (EFM) is the most widely used method of intrapartum surveillance, and our objective is to review its ability to prevent perinatal brain injury and death. DATA SOURCES: Studies that quantified intrapartum EFM and its relation to specific neurologic outcomes (seizures, periventricular leukomalacia, cerebral palsy, death) were eligible for inclusion. MEDLINE was searched from 1966 to 2006 for studies that examined the relationship between intrapartum EFM and perinatal brain injury using these MeSH and text words: "cardiotocography," "electronic f…

Autonomic nervous system evaluation using heart rate variability parameters in vasomotor rhinitis patients.

OBJECTIVE: To evaluate the autonomic nervous system in vasomotor rhinitis patients using heart rate variability parameters. PATIENTS AND METHODS: The heart rate variability parameters (SDANN, SDNN, SDNN index, RMSSD, pNN50, triangular index) of 26 patients with vasomotor rhinitis were compared with those of 25 control subjects. RESULTS: The decrease in the SDANN and SDNN index in the vasomotor rhinitis patients is statistically significant. Likewise, the SDNN and triangular index decreased but not to a statistically significant degree. The decrease in rMSSD and pNN50, which are signs of para…

Effects of air pollution on heart rate variability: the VA normative aging study.

Reduced heart rate variability (HRV), a marker of poor cardiac autonomic function, has been associated with air pollution, especially fine particulate matter [< 2.5 microm in aerodynamic diameter (PM2.5)]. We examined the relationship between HRV [standard deviation of normal-to-normal intervals (SDNN), power in high frequency (HF) and low frequency (LF), and LF:HF ratio] and ambient air pollutants in 497 men from the Normative Aging Study in greater Boston, Massachusetts, seen between November 2000 and October 2003. We examined 4-hr, 24-hr, and 48-hr moving averages of air pollution (PM2.5, p…

Traffic related pollution and heart rate variability in a panel of elderly subjects.

BACKGROUND: Particulate air pollution has been associated with increased cardiovascular deaths and hospital admissions. To help understand the mechanisms, the types of particles most involved, and the types of persons most susceptible, the association between exposure to summertime air pollution and heart rate variability (HRV) was examined in a panel study of 28 elderly subjects. METHODS: Subjects were seen once a week for up to 12 weeks and HRV (SDNN, r-MSSD, PNN50, low frequency/high frequency ratio (LFHFR)) was measured for approximately 30 minutes at each session using a defined protocol…

Control of heart rate versus rhythm in rheumatic atrial fibrillation: a randomized study.

BACKGROUND: Patients with rheumatic heart disease with atrial fibrillation incur significant morbidity and mortality. Which approach, ventricular rate control or maintenance of sinus rhythm, in this setting might be superior is not known. The role of amiodarone in this patient population for maintaining sinus rhythm has not been evaluated. METHODS AND RESULTS: We prospectively studied 144 patients with chronic rheumatic atrial fibrillation in a double-blind protocol in which rhythm control (group I), comprising 48 patients each with amiodarone (group Ia) and placebo (group Ib), were compared

Baroreflex control of heart rate during heating in subjects with low orthostatic tolerance.

BACKGROUND: Heat stress induces a reduction of orthostatic tolerance. The cardiovascular responses, including the cardiac baroreflex response to heat stress, were examined to test the hypothesis that subjects with orthostatically low tolerance demonstrate an impaired baroreflex control of heart rate (HR) during heat stress. METHODS: There were 44 healthy young volunteers who underwent whole body heat stress produced by a hot-water-perfused suit during supine rest for 45 min and 75 degrees head-up tilt (HUT) for 6 min. Esophageal temperature, HR, arterial pressure, and skin blood flow in the f…

Effect of heart rate on regulative features of the cortical activity-flow coupling.

OBJECTIVE: Heart rate plays an important role in compensatory conditions of arterial pressure changes. Very little information, however, exists on its role in the dynamic adjustment of stimulated organ perfusion. We studied the influence of heart rate on the activity-flow coupling mechanism which adapts local cerebral blood flow in accordance with cortical activity. Since it does not affect heart rate or arterial blood pressure by itself, the commonly observed heart rate variability in test conditions was used to compare the flow response between different heart rate groups. For evaluation und…

The critical period of non-reassuring fetal heart rate patterns in preterm gestation.

OBJECTIVE: Our purpose was to determine the critical period for non-reassuring fetal heart rate (FHR) patterns in preterm gestation in predicting fetal acidosis (umbilical arterial pH <7.1) at birth. STUDY DESIGN: A prospective descriptive study was performed. We reviewed the FHR pattern and umbilical blood gas level measurements, and investigated the correlations between non-reassuring FHR patterns and umbilical arterial pH. RESULTS: There was a significant difference in the frequency of fetal acidosis between the neonatal death and survival groups (5/13 versus 30/759, P=0.0001). Umbilical …

Changes in heart rate variability in chronic uremic patients during ultrafiltration and hemodialysis.

BACKGROUND: The analysis of heart rate variability (HRV) is a useful tool to evaluate cardiac autonomic modulation, which is frequently impaired in chronic uremia. AIMS: The aim of this study was to evaluate HRV in chronic uremics and to separately investigate the acute changes induced by volume depletion and solute removal during a hemodialysis session. METHODS: Fourteen uremic patients (8 males and 6 females, aged 50 +/- 15 years) on maintenance hemodialysis and 14 sex- and age-matched healthy controls were studied. Both groups underwent ambulatory electrocardiogram monitoring to evaluate …

Nonlinear dynamics of heart rate variability during experimental hemorrhage in ketamine-anesthetized rats.

Indexes of heart rate variability (HRV) based on linear stochastic models are independent risk factors for arrhythmic death (AD). An index based on a nonlinear deterministic model, a reduction in the point correlation dimension (PD2i), has been shown in both animal and human studies to have a higher sensitivity and specificity for predicting AD. Dimensional reduction subsequent to transient ischemia was examined previously in a simple model system, the intrinsic nervous system of the isolated rabbit heart. The present study presents a new model system in which the higher cerebral centers are b…

Preserved blood pressure and heart rate circadian rhythm in early stage Alzheimer's disease.

BACKGROUND: This study investigated the blood pressure (BP) values over the day-night period in 11 noninstitutionalized patients affected by probable Alzheimer's disease (AD) in its early stage. The scientific aim was to detect whether the BP circadian rhythm (CR) was preserved, given the fact that CR disruption was observed in advanced or institutionalized AD patients. METHODS: The BP within-day values were gathered via noninvasive ambulatory monitoring. The BP time series were analyzed according to the chronobiological procedure, called Cosinor method with three harmonic components. RESULT
