OBJECTIVE: Using an evidence-based approach, a Vermont Oxford Network focus group whose goal was to reduce brain injury developed and implemented a number of potentially better practices. Each center approached implementation of the practices differently. Reducing the incidence of intraventricular hemorrhage and periventricular leukomalacia are important for improving long-term outcomes for low birth weight infants.
METHODS: Implementation approaches for some but not all of the practices at the various centers are discussed. The practices reviewed include optimal peripartum management, such a

BACKGROUND: Operating-room (OR) nurses encounter disinfectants, sterilizing agents, and latex products as embedded job tasks, yet their multi-domain symptom burden, safety awareness, and preventive practices have rarely been examined together, particularly in Chinese tertiary-care settings.
METHODS: We undertook a single-center cross-sectional investigation of operating-room nurses at a tertiary hospital in China. A rigorously structured questionnaire captured agent-specific exposure frequency, task-based exposure, occupational symptomatology, safety awareness, and preventive practices. Compo

BACKGROUND: Cardiogenic shock and sepsis are severe haemodynamic states that are frequently present concomitantly, leading to substantial mortality. Despite its frequency and clinical significance, there is a striking lack of literature on the outcomes of combined sepsis and cardiogenic shock.
METHODS: FRENSHOCK was a prospective registry including 772 patients with cardiogenic shock from 49 centres. The primary endpoint was 1-month all-cause mortality. Secondary endpoints included heart transplantation, ventricular assistance device and all-cause death rate at 1year.
RESULTS: Among the 772

The aim of this study was to determine whether early administration (2 hours after the first surfactant dose) of the second surfactant dose would be superior to late surfactant treatment (6 hours after the first surfactant dose) in preterm infants with severe respiratory distress syndrome. Between June 2003 and March 2005, 40 newborns born with respiratory distress syndrome in Uludağ University Hospital were investigated in this prospective study. The inclusion criteria for the recruitment of the infants were: age < or = 2 hours, birth weight between 600-2500 g, gestational age between 24-36 w

BACKGROUND: Some neonates with congenital diaphragmatic hernia (CDH) and persistent pulmonary hypertension are not adequately oxygenated with conventional treatment. The extracorporeal membrane oxygenation (ECMO) has been successful in some of them as an alternative in their management.
PATIENTS AND METHODS: We studied the charts of 47 neonates with CDH, symptomatic within 24 hours of birth, treated in our institution during the last seven years (1987-1994). In all of them, conventional ventilation and hemodynamic support was used. In 12 patients high frequency ventilation (HFV) was used and

BACKGROUND: Retinopathy of prematurity (ROP) remains a leading cause of preventable childhood blindness. While gestational age (GA) and oxygen therapy are established risk factors, prior research has largely treated oxygen exposure as an aggregate variable, without disaggregating the impact of specific respiratory support modalities. This study investigated whether the mode and duration of respiratory support independently influence ROP risk.
METHODS: We conducted a single-center retrospective cohort study of 83 preterm infants (GA <28 weeks) admitted to the neonatal intensive care unit (NICU

INTRODUCTION: Biphasic positive airway pressure (BIVENT) is a partial support mode that employs pressure-controlled, time-cycled ventilation set at two levels of continuous positive airway pressure with unrestricted spontaneous breathing. BIVENT can modulate inspiratory effort by modifying the frequency of controlled breaths. Nevertheless, the optimal amount of inspiratory effort to improve respiratory function while minimizing ventilator-associated lung injury during partial ventilatory assistance has not been determined. Furthermore, it is unclear whether the effects of partial ventilatory s

Electrosurgery refers to the passage of a high-frequency, high-voltage electrical current through the body to achieve the desired surgical effects. At the same time, these procedures are accompanied by a general increase of the electromagnetic field in an operating room that may expose both patients and personnel to relatively high levels of radiofrequency radiation. In the first part of this study, we have taken into account the radiation emitted by different monopolar electrosurgical devices, evaluating the electromagnetic field strength delivered by an electrosurgical handle and straying fr

BACKGROUND: Pulmonary ventilation imaging has become an increasingly important component in thoracic radiotherapy, as it is applied to functional avoidance radiotherapy, where radiation doses are strategically minimized to preserve functional lung regions.
PURPOSE: We developed a framework that generates computed tomography ventilation images (CTVI) from planning CT (PCT) images and demonstrated its estimation performance.
METHODS: The subjects were a patient cohort consisting of 102 locally-advanced non-small-cell lung cancer (NSCLC) patients who received radiotherapy from 2014 to 2023. The

OBJECTIVES: Transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) is an intraoperative ventilatory technique that allows avoidance of tracheal intubation (TI) or jet ventilation (JV) in selected laryngologic surgical cases. Unimpeded access to all parts of the glottis may improve surgical precision, decrease operative time, and potentially improve patient outcomes. The objective of this prospective, randomized, patient-blinded, 2-arm parallel pilot trial was to investigate the safety and efficacy of THRIVE use for adult patients undergoing nonlaser laryngologic surgery of shor

OBJECTIVE: Multiple organ dysfunction syndrome (MODS) is the sequential failure of several organ systems after a trigger event, like sepsis or cardiogenic shock. Mortality rate is high, up to 70%. Autonomic dysfunction may substantially contribute to the development of MODS. Our study aimed to characterize a) the spectrum of autonomic dysfunction of critically ill MODS patients; b) whether autonomic dysfunction is different in patients receiving sedation, mechanical ventilation, or catecholamines; c) the age dependency of autonomic dysfunction in MODS; and d) whether autonomic dysfunction pred

A periodic breathing (PB) pattern is often observed in chronic heart failure (CHF) patients (pts). In order to clarify the role of this abnormal respiratory activity upon heart period variability we investigated, in a group of 20 stable CHF pts (NYHA class II to III, median EF 24%) showing a PB pattern, 1) whether observed data were consistent with the instability hypothesis of PB and 2) the relationship between oscillations of heart period and the contemporary fluctuations of ventilatory and chemoreceptor activity. Univariate and bivariate spectral analysis were performed on short-term restin
