Increased risk of avoidable hospitalization among patients with schizophrenia.

OBJECTIVE: Some studies suggest that hospitalizations for certain conditions, called ambulatory care sensitive conditions (ACSCs), are potentially avoidable. However, almost no study has addressed the risk of ACSC hospitalizations specifically for patients with severe mental illness. Our study examines the risk of ACSC hospitalizations among patients with schizophrenia in Taiwan, using a 5-year, nationwide, population-based database. METHODS: Our study included 2503 patients with schizophrenia and 20 024 matched patients without schizophrenia. Poisson regression analysis was then performed in

The use of a pediatric appendicitis pathway in a large integrated health system reduced computed tomography imaging in the ED.

BACKGROUND: Appendicitis is the most common cause of an acute surgical abdomen in children. Diagnosis is often challenging as few pediatric patients present with classic symptoms. Clinicians are thus dependent on imaging to reach an accurate diagnosis. Although computerized tomography (CT) has high sensitivity and specificity, it has the disadvantage of imparting ionizing radiation. Ultrasound (US) is readily available and has comparable accuracy to CT when performed by experienced sonographers. We sought to examine the impact of a system-wide process improvement plan on CT use and other metri

"Triggering the palliative intent"?: A qualitative implementation evaluation of a prognostication model for advanced dementia (PRO-MADE) in a geriatric tertiary care setting for the integration of early palliative care.

BACKGROUND: Prognostication uncertainty in advanced dementia impedes timely palliative referrals. Despite rapid development of dementia prognostication models, most have not progressed beyond external validation, with implementation in routine clinical practice as part of the electronic medical record system remaining underexplored. Empirical gaps exist in understanding interdisciplinary perspectives from palliative and geriatric specialties regarding implementation complexities of such hospital-based innovations. This formative study evaluated implementation of a validated PROgnostic Model fo

Reducing Recurrence and Complications Related to Clostridioides difficile Infection: A Panel Discussion Summary.

BACKGROUND: The Centers for Disease Control and Prevention identifies Clostridioides difficile infection (CDI) as an urgent threat to people and health care systems. CDI leads to high health care utilizations and results in significantly reduced quality of life for patients. The high burden of disease is seen across all health care settings, outside of the hospital, in the community, and in younger people. Individuals with CDI transition from hospitals to long-term care facilities to the community, and management of these transitions can reduce the incidence of recurrence and rehospitalization

Comparative phenotypic and genotypic analysis of distinct Pseudomonas aeruginosa T3SS effector genotypes.

BACKGROUND: Pseudomonas aeruginosa (P. aeruginosa) causes healthcare- and community-associated infections with high antimicrobial resistance and virulence diversity. The type III secretion system (T3SS) effectors genotypes (exoS+, exoU+, exoT+, and exoY+) play central roles in pathogenesis, yet their relationships with antimicrobial resistance, virulence profiles, serotypes, and clinical manifestations remain incompletely defined. METHODS: Five hundred clinical specimens, sputum, urine, burn, wound, and eye exudates, 100 each, were collected from multiple hospitals for isolation of P. aerugin

Rural Cancer Care Delivery in the United States: A 2026 Update on Disparities, Vulnerable Populations, and Emerging Models.

Rural Americans with cancer face persistent and widening disparities in incidence, stage at diagnosis, treatment access, and mortality. This paper synthesizes evidence on the financial sustainability of rural oncology programs, the populations bearing the greatest burden of rural cancer disparities, and the care delivery models most likely to narrow those gaps. We conducted a narrative review of English-language literature published through early 2026, drawing from peer-reviewed articles, national surveillance data, ASCO abstracts, and policy analyses. Rural oncology programs face significant

Monitoring disease trends and characterization of non-typhoidal Salmonella isolates from community wastewater in a mid-sized city in Florida, USA: a retrospective study.

UNLABELLED: Non-typhoidal Salmonella (NTS) poses a serious threat for multi-drug-resistant infections in the United States. However, most infections are not detected through traditional clinical surveillance, which severely underestimates the community disease burden. Wastewater-based epidemiology can overcome barriers in clinical surveillance and fill the gap. We performed a retrospective study to monitor disease trends and characterize viable NTS isolates from a medium-sized city's wastewater treatment plants. Of 106 weekly raw influent samples from September 2020 to September 2021, 81 sampl

OBJECTIVE: The Autonomous Community of Galicia has adopted DECREE 216/2011 on health standards for poultry production, in addition to the Spanish national programs. However, no program has yet been implemented to eradicate campylobacteriosis, which shares the same reservoir. The aim of this study was to compare the evolution of Salmonella spp. isolates with respect to those of Campylobacter spp. in faecal samples received by the Microbiology Department. METHODS: A retrospective descriptive comparative study was conducted through the Laboratory Information System (SIL) of Salmonella spp. isola

Systemic Inflammatory Response and Outcomes in Community-Acquired Pneumonia Patients Categorized According to the Smoking Habit or Presence of Chronic Obstructive Pulmonary Disease.

The systemic inflammatory response (SIR) may help to predict clinical progression, treatment failure, and prognosis in community-acquired pneumonia (CAP). Exposure to tobacco smoke may affect the SIR; the role of smoking in CAP has not been consolidated. We evaluated the SIR and outcomes of hospitalized CAP patients stratified by smoking habits and the presence of COPD. This retrospective analysis was conducted at the Hospital Clinic of Barcelona. Baseline, clinical, microbiological, and laboratory variables were collected at admission, using C-reactive protein (CRP) levels as a marker of SIR.

Hospital-based Patient-specific Templates for Total Knee Arthroplasty: A Proof of Concept Clinical Study.

INTRODUCTION: All available patient-specific instruments or patient-specific templates (PSTs) are controlled by implant companies. Most of these companies outsource some of the steps of the PST such as imaging, preoperative planning, manufacturing of PST, and packing/sterilization. This is a proof of concept clinical study on the hospital-based PST system for total knee arthroplasty (TKA). METHODS: A total number of 257 TKA procedures were performed on the basis of a new concept of hospital-based PSTs. All 5 steps of the PST [ie, imaging (computed tomographic scanning), planning, PST producti

Inpatient and outpatient treatment for acute malnutrition in infants under 6 months; a qualitative study from Senegal.

BACKGROUND: Treatment of acute malnutrition in infants under 6 months is a relevant topic regarding the global problem of maternal and child malnutrition. While treatment for older age groups has shifted more towards an outpatient, community based approach, young infants are mostly treated in hospital. This study aims to describe barriers and facilitators for outpatient and inpatient treatment of malnourished infants under 6 months in Senegal. METHODS: This qualitative descriptive study uses in-depth interviews with health workers and focus group discussions with mothers of malnourished infan

Zimbabwe's hospital referral system: does it work?

BACKGROUND: Anecdotal evidence has suggested inefficiency in the pyramidal health care referral system established in Zimbabwe in 1980, as part of its primary health care (PHC) model. AIM: To assess the functioning of the pyramidal referral system in two rural districts surrounding Harare, Zimbabwe, with regard to two common indicator conditions: pneumonia in children and malaria in adults. METHODS: For a three-month period, all complete inpatient records with discharge diagnoses of pneumonia or malaria from three hospitals representing different levels of care were analyzed (n = 227). Financial model for a transitional pain service at a large tertiary academic center in the USA.

Approximately 1 in 10 patients undergoing surgery is considered at high risk for poor pain and opioid-related outcomes due to chronic pain or persistent opioid use prior to surgery, leading to increased hospital lengths of stay, emergency department visits, hospital readmissions, and worse long-term outcomes. Multidisciplinary transitional pain services (TPSs) have been shown to effectively identify and optimize high-risk patients before surgery, leading to a reduction in healthcare utilization. We conducted a series of semistructured interviews, a literature search, and a financial analysis t

Definition and classification of chyle leak after pancreatic operation: A consensus statement by the International Study Group on Pancreatic Surgery.

BACKGROUND: Recent literature suggests that chyle leak may complicate up to 10% of pancreatic resections. Treatment depends on its severity, which may include chylous ascites. No international consensus definition or grading system of chyle leak currently is available. METHODS: The International Study Group on Pancreatic Surgery, an international panel of pancreatic surgeons working in well-known, high-volume centers, reviewed the literature and worked together to establish a consensus on the definition and classification of chyle leak after pancreatic operation. RESULTS: Chyle leak was defi

"A world crazier than us": Vanishing social contexts and the consequences for psychiatric practice in contemporary Romania.

Since the end of Communism, mental health care in Romania has increasingly sought to align its practices with idealized models of Western psychiatric practice. Much of this realignment has been made possible by accessing and integrating new pharmaceuticals into psychiatric hospital settings. Less straightforward have been the painful attempts to create a system modeled on international standards for training and certifying psychotherapists. Unfortunately, the political, economic, infrastructural, and epistemological environment of the Romanian mental health care system has prevented many other

Identification and management of women at high risk for hereditary breast/ovarian cancer syndrome.

Despite advances in identifying genetic markers of high risk patients and the availability of genetic testing, it remains challenging to efficiently identify women who are at hereditary risk and to manage their care appropriately. HughesRiskApps, an open-source family history collection, risk assessment, and Clinical Decision Support (CDS) software package, was developed to address the shortcomings in our ability to identify and treat the high risk population. This system is designed for use in primary care clinics, breast centers, and cancer risk clinics to collect family history and risk inf
