[Transhepatic endless drainage according to Goetze-Dick in stenoses of the intrahepatic and extrahepatic bile ducts (author's transl)].

Results and experiences in 51 transhepatic endless drainage operations according to the method of Goetze-Dick are dealt with, the indication as well as the technique of the operation are discussed in detail. The advantage of this method is based on the fact, that the drain can be easily changed. So patients with inoperable obstructive jaundice can often be given many month of worthwhile life. The extracorporal part of the drain is hardly found to trouble the patients. Also benign intrahepatic obstructions, where no bile duct is left to be anastomized, can be operated on satisfactorily with thi

Percutaneous stent placement for malignant hilar biliary obstruction: side-by-side versus stent-in-stent technique.

BACKGROUND: Currently, side-by-side (SBS) and stent-in-stent (SIS) are the two main techniques for stent deployment to treat hilar biliary obstructions. Previous studies comparing these two techniques are very limited, and thus, no consensus has been reached on which technique is better. The purpose of this study is to compare the clinical efficacy and safety of SBS and SIS deployment via a percutaneous approach for malignant hilar biliary obstruction. METHODS: From July 2012 to April 2019, 65 patients with malignant hilar biliary obstruction who underwent bilateral stenting using either the

Pre-operative stenting and complications following pancreatoduodenectomy for pancreatic cancer: an analysis of the ACS-NSQIP registry.

BACKGROUND: Historically, pre-operative biliary stenting has been associated with higher infectious complication rates following pancreatoduodenectomy. However, alleviation of biliary obstruction is necessary for consideration of pre-operative chemotherapy, which may improve disease-free survival, or for mitigation of symptoms while awaiting surgery. Our aim is to compare contemporary post-operative complication risk among patients with pre-operative endoscopic retrograde cholangiopancreatography (ERCP) stenting compared to those without. METHODS: Patients who underwent a pancreatoduodenectom

Validity and safety of endoscopic biliary stenting for biliary stricture associated with IgG4-related pancreatobiliary disease during steroid therapy.

Background  Patients with IgG4-related sclerosing cholangitis and autoimmune pancreatitis frequently develop obstructive jaundice, which requires endoscopic biliary stenting (EBS) during steroid therapy to prevent bile duct infection from cholestasis and adverse steroid effects. However, it is controversial whether EBS during steroid therapy is advisable, because the procedure itself carries a risk of cholangitis and procedure-related adverse events. This study aimed to clarify the validity and safety of EBS for patients with biliary stricture associated with IgG4-related pancreatobiliary dise

[Results of repeated operations in cancer of the major duodenal papilla].

15 repeated operations for cancer of the major duodenal papilla were carried out. Gastroduodenal obstruction, recurrent jaundice, external bile fistulas, uneffective first operation served as indications for these interventions. Pathological conditions following a radical operation in cancer of the major duodenal papilla are not always due to the progress of the tumor. Repeated operations for cancer of this localization are by far justified with appropriate indications.

A rare cause of persistent cholestatic jaundice: gallbladder amyloidosis.

Gallbladder amyloidosis is an exceptionally rare condition that may clinically mimic extrahepatic cholestasis. An 81-year-old man presented with persistent cholestatic jaundice and recurrent right upper quadrant pain. Laboratory evaluation showed conjugated hyperbilirubinemia and elevated cholestatic liver enzymes without signs of inflammation. Imaging studies, including ultrasound, MRCP, and CT, demonstrated gallstones and gallbladder wall thickening but no bile duct dilation or obstruction. Endoscopic retrograde cholangiopancreatography and endoscopic ultrasound were unremarkable. Because of

Liver disease induced by radioembolization of liver tumors: description and possible risk factors.

BACKGROUND: To the authors' knowledge, liver damage after liver radioembolization with yttrium90-labeled microspheres has never been studied specifically. METHODS: Using a complete set of data recorded prospectively among all patients without previous chronic liver disease treated by radioembolization at the authors' institution from September 2003 to July 2006, patterns of liver damage were identified and possible risk factors were analyzed. RESULTS: In all, 20% of patients developed a distinct clinical picture that appeared 4 to 8 weeks after treatment and was characterized by jaundice and

Laminin and procollagen-III-peptide as a serum marker for hepatic fibrosis in congenital biliary atresia.

The extracellular matrix (ECM) is a complex of macromolecules that includes collagens, proteoglycans, and complex glycoproteins. In fibrotic liver tissue there is an increase in all of these matrix components, and they increase in serum in the patients with alcoholic hepatitis or liver cirrhosis. These ECM components have been used as a serum marker of hepatic fibrosis. Prolonged obstruction of bile flow results in morphologic and biochemical changes and the development of secondary biliary cirrhosis. In congenital biliary atresia (CBA) there is a close correlation between the degree of the he

Jaundice and bleeding caused by cystic artery pseudoaneurysm after cholecystectomy: A case report.

RATIONALE: Cystic artery pseudoaneurysm (PSA) after laparoscopic cholecystectomy is a rare but potentially fatal complication that can lead to both jaundice and gastrointestinal bleeding. PATIENT CONCERNS: A 42-year-old woman, who was previously asymptomatic and undergoing a routine health examination, presented with jaundice and gastrointestinal bleeding 5 months after her laparoscopic cholecystectomy. DIAGNOSES: Initial MRCP raised concerns for bile duct obstruction, prompting consideration of biliary trauma or tumor. However, contrast-enhanced CT and subsequent angiography revealed a 2.5

Biopsy-confirmed fenofibrate-induced severe jaundice: A case report.

BACKGROUND: Drug-induced liver injury (DILI) is the leading cause of acute liver failure in the United States. DILI is mainly caused by painkillers and fever reducers, and it is often characterized by the type of hepatic injury (hepatocellular or cholestatic). This report presents a case of fenofibrate-induced severe jaundice in a 65-year-old Korean male with no prior history of liver disease. We offer a strategy for patients who present signs of severe liver injury with jaundice and high elevations in serum transaminases. CASE SUMMARY: A 65-year-old male visited the gastroenterology outpatie

A Case of Metastatic Seminoma Mimicking a Primary Pancreatic Tumor.

Metastatic seminoma to the pancreas is exceedingly rare, with few reported cases in medical literature. We present a case of a 66-year-old male, six years post-remission from testicular seminoma, who presented with obstructive jaundice and a pancreatic mass mimicking primary malignancy. Diagnostic workup including endoscopic ultrasound-guided biopsy confirmed metastatic seminoma. He underwent successful treatment with four cycles of cisplatin and etoposide, achieving complete remission. This case underscores the diagnostic challenge of pancreatic metastases and emphasizes the role of biopsy in

[Pancreatic Cancer with Hemobilia after Biliary Self-Expandable Metallic Stent Placement during Neoadjuvant Chemotherapy-A Case Report].

A 67-year-old woman was diagnosed with borderline resectable pancreatic cancer and obstructive jaundice. A covered self-expandable metallic stent(SEMS)was placed endoscopically. Neoadjuvant chemotherapy with gemcitabine plus nabpaclitaxel was provided. Forty-seven days after the SEMS placement, she presented with hematemesis. Computed tomography revealed migration of SEMS into the small bowel. No pseudoaneurysms were detected. Upper digestive endoscopy demonstrated hemobilia without obvious causes of bleeding in the stomach or duodenum. As hemorrhage recurrence was confirmed in the bile duct,

Percutaneous Transhepatic Biliary Drainage in a Two-Month-Old Infant with Inspissated Bile Syndrome.

Inspissated bile syndrome (IBS) is a relatively rare condition. Many treatment options are available, including medication, surgery, and surgical interventions, such as insertion of cholecystostomy drain, endoscopic retrograde cholangiopancreatography, internal biliary drainage, and percutaneous transhepatic biliary drainage (PTBD). We herein report the first case of IBS that was successfully treated with PTBD in a two-month-old infant in Korea. PTBD was initiated on postnatal day 72. On postnatal day 105, we confirmed complete improvement and successfully removed the catheters. This report su

[A case of extrahepatic bile duct metastasis from gastric cancer].

A 56-year-old man was admitted with obstructive jaundice. Abdominal computed tomography and endoscopic retrograde cholangiopancreatography showed circumferential stenosis with irregular wall in lower bile duct, but the cytology of biliary brushing was no malignancy. The patient was given a diagnosis of gastric carcinoma with bone and skin metastasis. He died 2 months after the first hospital admission and autopsy was performed. The histological findings of gastric and bile duct tumor revealed signet ring cell carcinoma. The immunohistological findings of both tumors were identical. We definiti

Cholestasis and biliary dilatation associated with chronic ketamine abuse: a case series.

Ketamine is a dissociative anaesthetic agent that is still widely used in veterinary and human medicine. It is increasingly being used as a recreational hallucinogenic drug. Chronic ketamine abuse is known to account for lower urinary tract symptoms and urinary bladder dysfunction. There is now emerging evidence that ketamine misuse is also associated with abnormal liver function tests and biliary tract abnormality. We report three cases of chronic ketamine misuse in three young men who all presented with obstructive jaundice and biliary tract abnormality. We also describe the clinical feature

Extrahepatic biliary schwannomas: a case report.

Benign schwannomas arise in neural crest-derived Schwann cells. They can occur almost anywhere in the body, but their most common locations are the central nervous system, extremities, neck, mediastinum, and retroperitoneum. Schwannomas occurring in the biliary tract are extremely rare and mostly present with obstructive jaundice. We recently experienced a case of extrahepatic biliary schwannomas in a 64-yr-old female patient who presented with intra- and extrahepatic bile duct and gallbladder stones during a screening program. To the best of our knowledge, extrahepatic biliary schwannomas ass

IgG4-associated prostatitis complicating autoimmune pancreatitis.

A 65-year-old man underwent transurethral resection of the prostate on a diagnosis of benign prostatic hypertrophy. Almost simultaneously, a diagnostic resection of minor salivary glands was performed. After the resections he suffered from obstructive jaundice. Laboratory examinations revealed an increase in eosinophils and an elevation of serum IgG4. Pancreaticoduodenectomy was performed because of the possibility of pancreatic cancer. The prostate, salivary glands and pancreas showed common histological characteristics, namely, infiltration of lymphocytes and plasma cells accompanying dense

Magnetic resonance cholangiopancreatography: non-invasive imaging for the biliary tree and pancreatic duct.

Producing images similar to those acquired by the invasive procedures of endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous transhepatic cholangiography, magnetic resonance cholangiopancreatography (MRCP) is indicated in patients who are unable to undergo ERCP or have had previously unsuccessful ERCP. It is used increasingly in non-invasive evaluation of the pancreaticobiliary tree in cases where the need for intervention during ERCP is expected to be low. MRCP may help in identifying anomalous biliary anatomy or choledocholithiasis before laparoscopic cholecystectomy, and i
