Training room management of eye conditions.

The number of preventable sports eye injuries resulting in permanent vision loss remains unacceptably high. Convincing athletes to use protective eyewear for sports is challenging, but worth the effort. Regardless, injuries still occur, and they must be diagnosed and treated promptly. Maintaining the skills necessary to perform a good fundoscopic examination will help to methodically work through the differential diagnoses. Cultivating a working relationship with an ophthalmologist and feeling comfortable consulting with him is always beneficial when an obvious cause of vision loss is not appa

Long-term results of penetrating keratoplasty. A 10-year-plus retrospective study.

BACKGROUND: The aim of this study was to retrospectively analyse the outcome of a series of grafted patients over a period of more than 10 years and to determine their long-term survival probability. METHODS: The records of 89 patients who had 103 grafts performed in 97 eyes were analysed. Mean follow-up was 12.8 years (range 10-17 years). Life table analysis (Kaplan-Meier) was used to evaluate the graft survival of the total population and of different groups. RESULTS: Eighteen out of 89 patients (20.2%) had died. At the last visit before their death, 10 of the 21 grafts in those patients w

Rigid gas-permeable contact lenses and severe higher-order aberrations in postsurgical corneas.

PURPOSE: Even in the absence of significant residual refractive error, patients can be highly symptomatic after corneal refractive surgery. The presence of glare, halos, monocular polyopia, and loss of contrast are principally caused by the presence of higher-order aberrations. This study investigated the efficacy of rigid gas-permeable contact lenses in reducing these higher-order aberrations. METHODS: In a prospective study, 20 eyes of 10 consecutive, highly symptomatic patients who had undergone refractive surgery were fitted with semiscleral rigid gas-permeable lenses (Macrolenses). The m

The resiliency of the corneal endothelium to refractive and intraocular surgery.

PURPOSE: To describe stress factors (phenylephrine and contact lenses) from the corneal epithelium that can affect the corneal endothelium, and to describe the effects of refractive and intraocular surgery on the corneal endothelial structure and function. METHODS: Significant clinical and experimental publications are reviewed and recent experiments conducted in the author's laboratory to describe the corneal endothelial stresses. RESULTS: The corneal epithelium serves as a barrier to topical phenylephrine (2.5-10%). In a compromised epithelium, topical phenylephrine will cause drug-induced

Pupillary capture of implantable contact lens after blunt trauma.

A 33-year-old woman visited our clinic with blurry vision and periorbital swelling after experiencing blunt trauma to left eye. Ten months earlier, she had implantable contact lens (ICL) implantation in the left eye. Biomicroscopic examination showed that that 1 footplate of the ICL was entrapped in the pupillary aperture at the 7 o'clock position and the ICL was placed vertically. The patient had limited ocular movement in lateral gaze, and the computed tomography showed a medial orbital wall fracture. Pupillary capture of the ICL was surgically corrected with an iris manipulator under topica

Unraveling the impact of laser refractive surgery on corneal ectasia: an in silico study.

INTRODUCTION: Laser refractive surgeries are a safe option for low-to-moderate refractive corrections, providing excellent visual outcomes. Over the years, various procedures have been introduced into clinical practice, but the most performed today remain Photorefractive Keratectomy (PRK), Laser In-Situ Keratomileusis (LASIK), and Small Incision Lenticule Extraction (SMILE). Although laser refractive treatments are considered safe, clinicians have focused on the risk of post-surgical ectasia, a rare but serious complication. Ectasia is characterized by progressive corneal thinning and steepeni

Corneal dystrophies.

The term corneal dystrophy embraces a heterogenous group of bilateral genetically determined non-inflammatory corneal diseases that are restricted to the cornea. The designation is imprecise but remains in vogue because of its clinical value. Clinically, the corneal dystrophies can be divided into three groups based on the sole or predominant anatomical location of the abnormalities. Some affect primarily the corneal epithelium and its basement membrane or Bowman layer and the superficial corneal stroma (anterior corneal dystrophies), the corneal stroma (stromal corneal dystrophies), or Descem

Radial keratotomy for myopia. American Academy of Ophthalmology.

The operation of radial keratotomy is undergoing evolution and development. A few hundred thousand radial keratotomy procedures have been performed during the past decade by approximately 10% of the ophthalmologists in the United States. Clinical and laboratory data have now defined a relative level of safety and effectiveness for the procedure. Published data indicate that radial keratotomy usually achieves partial improvement in uncorrected visual acuity in patients with non-progressive low and moderate amounts of myopia. Undercorrection occurs commonly, and the amount of correction cannot b
