[Single-cut derotational osteotomy of the distal femur for correction of torsion and frontal axis].

OBJECTIVE: A rotational osteotomy requires a complete cut of the bone in order to correct maltorsion. An additional correction of the frontal axis can be achieved via an oblique cut of the bone. The osteotomy with bone to bone contact is fixed with an angle stable plate. INDICATIONS: Symptoms such as anterior knee pain, inwardly pointing knee syndrome, lateral patellar subluxation or dislocation, lateral patellar hypercompression syndrome are a common indication for derivational osteotomy if clinically increased femoral internal rotation and radiologically increased femoral antetorsion is det

[Combination of a Lisfranc injury and a dislocation of the proximal tibiofibular joint].

Lisfranc injuries and dislocations of the proximal tibiofibular joint are both rare and frequently missed on initial examination. However, delayed treatment can lead to chronic instability. This is a case report of a 26-year-old man, who received both of these injuries on the same extremity from a collision during a basketball game. The dislocation of the proximal tibiofibular joint was treated with closed reduction, however the Lisfranc injury required open reduction and internal fixation. At the follow-up after ten weeks, the patient was doing well and started rehabilitation.

Medial Patellofemoral Ligament Reconstruction with Open Physes.

Through this article, the authors aim to summarize the techniques performed on both first time and recurrent skeletally immature patients experiencing patellar dislocation. This article focuses on several key points, such as the importance of medial patellofemoral ligament femoral insertions being distal to the growth plate and performing extensive lateral release and quadricep tendon lengthening in cases of obligatory dislocation. Although acknowledging the procedures discussed cannot be considered for all patients, as individuals with open growth plates may require additional operative time,

[Reality of care of patella stabilizing operations : A status quo among active members of the German Society for Orthopedic and Trauma Surgery (DGOU)].

BACKGROUND: Patellofemoral instability is one of the most common pathologies of the knee joint. The planning and implementation of patella-stabilizing operations are very variable. With regard to the operative measures, the preoperative planning is of decisive importance, especially with regard to the complexity of underlying pathologies. OBJECTIVES: The aim of this study was to depict the current healthcare reality in relation to planning and implementation of patella stabilizing operations in Germany. Furthermore, it was to be ascertained whether automated analysis options would facilitate

[Traumatic isolated dislocation of the proximal tibiofibular joint is a rare injury].

Dislocation of the proximal tibiofibular joint should be promptly reduced when recognised in the emergency room. If closed reduction technique is unsuccessful, open reduction with the possibility of fixation should be performed. Currently, no consensus exists regarding the optimal post-reduction regime. We present a case report of a 30-year-old male who suffered a dislocation of the proximal tibiofibular joint during soccer. Closed reduction was successful, and the patient did not suffer from any sequalae later on.

Editorial Commentary: What Is the Optimal Management of First and Recurrent Patellar Instability? Patellofemoral Instability Management Continues to Evolve.

Patellar instability patients may present for evaluation after their first instability episode or after recurrent episodes. Current management differs for these 2 groups. The accepted "common denominator" of patellar instability is the medial patellofemoral ligament, or medial patellar restraints, to be more all-encompassing. These patients often have multiple contributing comorbidities (e.g., trochlear dysplasia, patellar alta, excessive lateral position of tibial tubercle). Historically, the recommended treatment for a first-time patellar dislocation was nonoperative because medial soft tiss

[A boy with a twisted knee].

A 14-year-old boy presented with a painful knee following a distorsion. He was unable to actively extend his lower leg. Conventional radiography showed no signs of a fracture. Both ultrasound imaging and MRI, however, demonstrated haemarthrosis and an osteochondral defect of the lateral femoral condyle. Apparently, an osteochondral lesion had developed due to patellar luxation. The osteochondral fragment was fixated using absorbable compression screws.

Trochleoplasty procedures show complication rates similar to other patellar-stabilizing procedures.

PURPOSE: Trochleoplasty aims to restore patellar stability. Various techniques have been described and almost all authors report successful results. However, the procedure has a significant risk of complications. Purpose of this study was to perform a systematic review and meta-analysis of the available literature to assess the rate of complications after the various techniques used for trochleoplasty procedures. MATERIALS AND METHODS: MEDLINE, EMBASE, Web of Science and Cochrane Library databases were searched. Studies on patients with recurrent patellar instability treated with a trochleopl

Anatomy and malalignment of the patellofemoral joint: its relation to patellofemoral arthrosis.

Patellofemoral joint function involves a complex orchestration of a number of dynamic, static, and neuromuscular factors. I will discuss the components of patellofemoral anatomy and whether the presence of certain anatomic variables may increase the risk of patellofemoral arthrosis. The patellofemoral surgeon is faced with several clinical challenges: the semantics of the language used to discuss these problems, the correlation of clinical symptoms and radiographic imaging to help diagnose a problem, and surgical challenges in optimizing extensor mechanism function.

Reconstruction of the lateral patellar retinaculum following lateral release: a case report.

The recognition and treatment of iatrogenic medial subluxation of the patella following lateral retinacular release may be difficult. The diagnosis even by magnetic resonance imaging may be difficult. The use of a medial patellar stabilising brace may be helpful in the diagnosis, alleviating symptoms in the short term and to gain the confidence of the patient. A technique of direct repair that was used successfully is described in a case report.

"Slip sliding Away": patellofemoral dislocation and tracking.

This article reviews the anatomy of the medial retinacular complex and the relationship of the anatomy to patellar dislocation. Clinical and radiographic findings of patellar dislocation are discussed, and the technical considerations and clinical usefulness of patellar tracking studies are addressed.

An analysis of the pivot shift phenomenon. The knee motions and subluxations induced by different examiners.

The description of the pivot shift test and its modifications is for the most part based on clinical observations. We wished to precisely determine the knee motions and medial-lateral tibiofemoral compartment subluxations that examiners induce in the knee joint to produce the pivot shift phenomenon. Eleven skilled knee surgeons performed the pivot shift test on an instrumented cadaveric lower limb. The anterior cruciate and superficial medial collateral ligaments (long fibers) of one limb were sectioned to produce an abnormal state. An instrumented spatial linkage allowed all six degrees of fr

Dysplasia of the quadriceps mechanism: hypoplasia of the vastus medialis muscle as related to the hypermobile patella syndrome.

Disturbance of the patellofemoral mechanism is the most common cause of dysfunction in the knee, especially in athletes of all age groups. This is primarily a congenital familial abnormality due to poor development in the vastus medialis. The patella develops in the line of maximum pull of this muscle group; in this congenital condition it develops in line with the stronger antagonist to the vastus medialis, the vastus lateralis muscle. Therefore, the position in the extremity that the patella assumes is the result of abnormal muscle development. This brings about other changes in the lower ex
