PURPOSE: The study goal was to evaluate neurosensory changes after liquid nitrogen cryotherapy in the management of mandibular lesions in close proximity to the inferior alveolar nerve.
PATIENTS AND METHODS: The design of the study was a retrospective review. Sixteen patients with posterior mandibular lesions (15 odontogenic keratocysts and 1 fibromyxoma) in close proximity to the inferior alveolar nerve were treated with a standardized enucleation and cryotherapy technique. Postoperative evaluation included patients report of symptoms and formal neurosensory testing.
RESULTS: All patients h

A 63-year-old man underwent segmental mandibulectomy for ameloblastoma, followed by fibula free-flap reconstruction. As the bone volume was inadequate, guided bone regeneration (GBR) was performed using iliac particulate cancellous bone and marrow, titanium mesh, and tenting screws. After consolidation, 4 implants were placed, and restoration was completed with a computer-aided design and computer-aided manufacturing titanium bar and LOCATOR-retained overdenture. This staged approach ensured stable implant placement and successful rehabilitation. At 1 year postoperatively, the patient's appear

The fibular free flap (FFF) approach is the preferred option for reconstruction of segmental mandibular deficiencies. Fixed dental prostheses are recommended for patients who have undergone FFF jaw surgery to improve quality of life, but treatment of these patients with an implant-supported prosthesis is challenging because of the thin bone and thick soft tissue in the fibula. This case history reports the use of a bone-level implant guide fabricated with a computer-aided system and milled customized long abutments for implant placement and prosthodontic procedures. BACKGROUND: Although vascularized bone grafts have become well accepted in adults, especially following ablative head and neck procedures, there are few long-term reports of their use in pediatric patients.
METHODS: In this study, the authors analyzed the outcomes of 18 free fibula grafts in 16 patients aged 10 months to 21 years (mean, 12 years) with an average follow-up of 5 years. Eleven patients had cancer-related defects, four had craniofacial anomalies, and one had a posttraumatic deformity. All patients with congenital malformations had been followed since birth, and the others had bee

The efficacy of distraction osteogenesis in an irradiated area is controversial, although this procedure is now widely used in the field of craniomaxillofacial surgery. We report the clinical results from 4 patients with mandibular defects treated by lengthening of the irradiated mandibles. All patients had a mandibular defect caused by ablation of a malignant tumor. They had undergone radiotherapy at a total dose of 30 to 50 Gy to the surgical site after tumorectomy. Distraction osteogenesis was used as the secondary reconstruction method in 6 sites of the remaining irradiated mandibles and i

A case of multiple-site osteosarcomas in the jaw of a 56-year-old patient is reported. The disease occurred consecutively at three different sites (left maxilla, left mandible, right mandible) separated by time intervals of 12 and 18 months, respectively. Metachronous osteosarcomas of the long bones is a rare form of osteosarcoma and implies multiple lesions appearing at different times, each one behaving clinically as a primary lesion. The pathogenesis of this disease is unknown as it is unclear whether the lesions represent independent primary tumours or metastatic disease.

Osteosarcoma of bone is a recognizable entity if the histopathologist designates tumors as such when their malignatn cells produce osteoid substance even if only in small foci. Such definition distinguishes this lesion from other sarcomas that arise in bone, especially chondrosarcoma and fibrosarcoma. There is a general tendency to consider that osteosarcomas represent a stereotyped form of disease for which new modalities of treatment can be applied and assessed. The question of whether a given osseous lesion is actually malignant and not a benign neoplasm or even a reactive non-neoplastic co

INTRODUCTION: Ginigivobuccal cancers often involve the mandible, impacting surgical planning and outcomes. Clinical assessment of bone invasion is unreliable, making imaging essential. CT excels at detecting cortical erosion but may miss early medullary involvement, while MRI offers greater sensitivity for marrow changes but may overestimate invasion. Diagnostic performance varies depending on the interpretive criteria, Brown's Favors early marrow detection, McGregor's prioritizes cortical specificity. Few studies have directly compared CT and MRI using both criteria in the same cohort. This s

Numb chin syndrome (NCS) is a rare but important clinical symptom and sign. It describes a condition presenting as anaesthesia or paraesthesia over the chin, which is usually unilateral. The condition manifests spontaneously with no history of trauma, infection or obvious odontogenic cause. NCS is a clinically important finding as it may be the primary manifestation of Multiple Sclerosis (MS) or sign of systemic malignancy. We present a case of a 58-year-old gentleman who was referred to a regional oral and maxillofacial unit with a three-month history of sudden onset unilateral numbness of th

BACKGROUND: Metastatic adenocarcinoma of the colon is a frequently encountered medical situation. Metastasis to the mandible from adenocarcinoma of the colon is very unusual and rarely reported. We report the case of a 73-year-old man with metastatic adenocarcinoma to the mandible.
METHODS: The patient was referred for evaluation of a mass of 2 months' duration in the right parotid gland. He gave a history of watery bowel movements of unknown duration. Physical examination revealed a 7- x 6-cm hard mass, which seemed to be fixed to the right mandible. A CT scan revealed a destructive process

We reviewed a 20-year experience with squamous carcinoma of the gums in 347 patients who received definitive therapy. More than three-quarters of the lesions involved the lower gum and all but 37 patients were previously untreated. The proportion of patients with localized tumors (N0) remained the same (64 percent). Surgery continued to be the treatment of choice in 97 percent of patients, but proportionately more patients had a more conservative procedure which preserved lower jaw continuity. The 5-year determinate survival rate was little changed (54 percent). Advanced clinical stage (stages

OBJECTIVES: Through a novel observation of parathyroid adenoma revealed by brown tumors of the jaws and a review of the literature, the authors describe this rare mode of primary hyperparathyroidism discovery.
MATERIAL AND METHODS: The patient was a 23-year-old woman who consulted for a recurrent tumefaction of the maxillary; histology showed reparative giant cell granuloma.
RESULTS: The clinical examination found an osseous tumefaction in continuity with the zygomatic bone and a gingival tumefaction on the mandible symphysis. The radiological findings showed two osteophytic lesions: mandibu

Formalin-fixed paraffin-embedded sections of three cases of ameloblastic fibrodentinoma (AFD) were studied by the avidin-biotin-peroxidase complex method using antibodies against neuron-specific enolase (NSE), glial fibrillary acidic protein (GFAP) and S100 protein and the results were compared with those in ameloblastic fibroma (AF). A striking histopathological characteristic of AFD was the formation of abortive dentin with various degrees of maturation at the epithelial-mesenchymal tissue interface. Central cells of enamel organ-like epithelia with various stages of abortive dentin inductio

Radiological features of 21 odontogenic myxomas found in 14 women and 7 men were analysed. Ten of the tumors were unilocular and 11 were multilocular. Unilocular lesions were mostly located in the anterior and multilocular in the posterior areas of the jaws. Intralesional trabeculation was seen in all multilocular but only in 3 unilocular lesions. Because radiological appearance of myxoma may be essentially similar with many other lesions of the jaws careful radiological examination is mandatory for planning of proper treatment.
