Efficacy of botulinum toxin type A administration within the current therapeutic algorithm for chronic anal fissure: study protocol for the prospective multicentre observational study FISURBOT-GECO6.

BACKGROUND: Chronic anal fissure is a common, benign anorectal condition associated with impaired quality of life. Despite the availability of several therapeutic options, considerable variability persists in its management, particularly regarding the role of botulinum toxin type A administration within the current treatment algorithms. Although botulinum toxin type A injection is a widely used approach to achieve chemical sphincterotomy, its real-world efficacy and safety across different clinical scenarios remain insufficiently characterised. AIM: Our objective is to evaluate the efficacy a

Outcomes of low-risk endometrial cancer with isolated tumor cells in the sentinel lymph nodes: a prospective, multi-center, single-arm, observational study (ENDO-ITC study).

BACKGROUND: It is unclear whether isolated tumor cells (ITCs) in sentinel lymph nodes (SLNs) adversely affect prognosis, especially in low-risk endometrial cancer. In a retrospective study, we showed a worse recurrence-free survival for low-risk endometrial cancer with ITCs than the node-negative group. PRIMARY OBJECTIVE: Our aim is to evaluate whether the likelihood of disease recurrence differs between a prospective cohort of patients with low-risk endometrial cancer with ITCs and an historical cohort with negative SLNs. STUDY HYPOTHESIS: We hypothesize that patients with low-risk endometr

Role of inflammation and haemostasis on aetiology and prognosis in young patients with ischaemic stroke: study protocol of the Observational Dutch Young Symptomatic StrokE study-EXTended (ODYSSEY-nEXT) - a multicentre prospective cohort study.

INTRODUCTION: The cause of ischaemic stroke at a young age remains unknown in 30% of cases, highlighting the need to identify hidden causes and risk factors in young patients. Transient and chronic risk factors may interact with the inflammatory and haemostatic systems, potentially driving key mechanisms in the pathogenesis. The 'Observational Dutch Young StrokE study-Extended' (ODYSSEY-nEXT) aims to enhance our understanding of these complex interactions through detailed phenotyping of the immune and haemostatic system and explore their relationship with long-term prognosis. METHODS AND ANAL

Indocyanine green for axillary sentinel lymph node biopsy in patients with breast cancer (INSEAN study).

INTRODUCTION: Sentinel lymph node (SLN) detection is the technique of choice for staging early-stage breast cancer (BC). The preferred technique for SLN detection is labelling with the radioisotope Technetium-99 (Tc-99). Other methods have been evaluated, including methylene blue, magnetic tracers, iodine seeds, or fluorescent substances. Various studies have shown the non-inferiority of indocyanine green (ICG) for SLN detection; however, inclusion criteria are selective and restrictive. MAIN AND SECONDARY HYPOTHESES: Main hypotheses: SLN detection using ICG provides results that are not infe

Protocol for a multi-country retrospective observational paediatric sepsis epidemiological study (SENTINEL International).

INTRODUCTION: Improving outcomes from sepsis in children is a WHO Global Health Priority, yet mortality from sepsis remains high, particularly in low- and middle-income countries (LMICs). This database from children with community-acquired childhood sepsis in LMICs and some high-income countries will allow analysis of the burden of disease, including incidence, severity and outcomes. Understanding these aspects of sepsis care is fundamental for the design and conduct of future international interventional trials to improve childhood sepsis outcomes. METHODS AND ANALYSIS: This multicountry ret

Safety and effectiveness of the Chocolate balloon catheter in the treatment of peripheral artery disease with percutaneous transluminal angioplasty: a multicentre, prospective, observational study protocol.

BACKGROUND: Peripheral artery disease (PAD) represents a significant global health burden. Percutaneous transluminal angioplasty (PTA) is the most commonly employed endovascular intervention; however, its long-term efficacy is often compromised by restenosis and flow-limiting dissection. The Chocolate balloon catheter, characterised by a nitinol-constrained design that enables controlled, segmental vessel dilation, has demonstrated potential in minimising vascular trauma and enhancing patency. Nevertheless, there remains a lack of robust multicentre evidence evaluating its safety and effective

Practice of oxygenation and respiratory support during fibreoptic bronchoscopy: the OxyFOB study protocol.

INTRODUCTION: Flexible bronchoscopy (FB) is widely used for diagnostic and therapeutic procedures in pulmonary medicine. However, FB can cause respiratory and haemodynamic complications, especially in patients with pre-existing lung and/or cardiovascular comorbidities. Despite the range of oxygenation and ventilatory approaches available to prevent these risks, evidence regarding their real-world application and clinical impact is limited. The OxyFOB study aims to assess the prevalence and outcomes of various oxygenation and ventilatory support strategies used during FB across Europe. METHODS

Assessing the impact of COmorbidities and Sociodemographic factors on Multiorgan Injury following COVID-19: rationale and protocol design of COSMIC, a UK multicentre observational study of COVID-negative controls.

INTRODUCTION: SARS-CoV-2 disease (COVID-19) has had an enormous health and economic impact globally. Although primarily a respiratory illness, multi-organ involvement is common in COVID-19, with evidence of vascular-mediated damage in the heart, liver, kidneys and brain in a substantial proportion of patients following moderate-to-severe infection. The pathophysiology and long-term clinical implications of multi-organ injury remain to be fully elucidated. Age, gender, ethnicity, frailty and deprivation are key determinants of infection severity, and both morbidity and mortality appear higher i

Changes on clinical and participatory outcomes in people with severe-to-profound hearing loss after cochlear implantation: protocol of a multicentre prospective observational cohort study - Societal Merit of Intervention on Hearing Loss Evaluation (SMILE).

INTRODUCTION: Cochlear implantation (CI) is a (cost-)effective intervention for people with severe or profound hearing loss. Since its introduction experience increased and the technology evolved, leading to better results and relaxation of CI eligibility criteria. Meanwhile, with national healthcare costs increasing there is a need for evidence of healthcare technology's value. This protocol describes a study to investigate clinical and participatory outcomes after CI for the currently (expanded) eligible hearing impaired population. The study adds to the current evidence base through its mul

Outcome of the surgical decompression for traumatic optic neuropathy: a systematic review and meta-analysis.

There exists a persisting controversy regarding the indications for optic nerve surgical decompression (ONSD) in traumatic optic neuropathy (TON). A meta-analysis is warranted to help guiding therapeutic decisions and address gaps in knowledge. The authors conducted a search of PubMed and MEDLINE electronic databases. Primary endpoint was the improvement in the visual function with ONSD in comparison with the conservative management. Secondary endpoint was visual function improvement when surgery was performed within the first 7 days. A random effects model meta-analysis was conducted. Data fr

Risks of endometrial cancer associated with different hormone replacement therapies in the E3N cohort, 1992-2008.

We assessed whether different oral progestogens in hormone replacement therapy may differentially affect the risk of endometrial cancer, using data from the Etude Epidémiologique auprès de femmes de l'Education Nationale (E3N), a French cohort study (1992-2008). Hazard ratios and their confidence intervals were derived from Cox models. Among 65,630 postmenopausal women (mean follow-up: 10.8 years), 301 endometrial cancers occurred. Compared with never use, ever use of estrogen + micronized progesterone was associated with an increased risk of endometrial cancer (hazard ratio (HR) = 1.80, 95% c

Obesity-fertility cohort study: protocol for the assessment of children aged 6-12 years and their mothers.

INTRODUCTION: Maternal preconception obesity and adverse gestational metabolic health increase the risk of childhood obesity in offspring, but the preconception period may be an opportune time to intervene, given the motivation of the mother and the epigenetic changes that may be beneficial for the gametes during this period. However, there is a lack of studies evaluating children born to women who have had a preconception intervention. Our group has therefore designed an ancillary study to assess children born to women enrolled in the obesity-fertility randomised controlled trial (RCT), who w

Fetal-hope study: Home monitoring of fetal heart rate in SSA + pregnant women: Rationale and design.

INTRODUCTION: Worldwide, approximately 0.5-1 % of women of childbearing age are positive for anti-Ro/SSA antibodies and anti-La/SSB. The presence of these antibodies may be responsible for the appearance of Neonatal Lupus. The pathogenesis of this disease is probably mediated by the binding of these antibodies to fetal myocardiocytes. The typical manifestation is atrioventricular block (AVB), which in most cases is complete (CHB). AVB commonly develops between 16 and 26 weeks of gestation and is cause of severe heart failure. CHB appears to be irreversible, but anecdotal reports suggest that t

Concerns about the health effects of industrially produced seed oils are without scientific foundation: a scoping narrative review of the clinical and observational evidence.

To reduce cardiovascular disease (CVD) risk, worldwide dietary guidelines emphasize limiting saturated fatty acid (SFA) intake, and replacing SFAs with unsaturated fatty acids, especially polyunsaturated fatty acids (PUFA), when exceeding recommendations. Nevertheless, a largely social media-driven controversy challenging the healthfulness of oils high in PUFA, namely seed oils, a subset of vegetable oils, and diets high in linoleic acid (LA) has emerged. To aid health professionals in their understanding of this controversy, we conducted an extensive review of the human literature to determin

Association Between the Triglycerides-to-High-Density Lipoprotein-Cholesterol (TG/HDL-C) Ratio and Chronic Kidney Disease: A Systematic Review and Meta-Analysis of Observational Studies.

BACKGROUND: The relationship between the triglycerides-to-high-density lipoprotein cholesterol (TG/HDL-C) ratio and the risk of developing chronic kidney disease (CKD) remains unclear, as CKD is a major health challenge worldwide. This study aimed to investigate the association between the TG/HDL-C ratio and the risk of developing CKD. METHODS: To find studies that examined the association of TG/HDL-C ratio (without stratification restrictions for TG/HDL-C) with CKD or renal disorders, PubMed, Embase, Scopus, Google Scholar, and Web of Science databases, as well as study references, were sear
