Orbital involvement in multifocal fibrosclerosis.

Multifocal fibrosclerosis is a condition of unknown aetiology, characterised by fibrous lesions occurring at a variety of sites. Clinical variants include retroperitoneal fibrosis, Riedel's thyroiditis, sclerosing cholangitis, and mediastinal fibrosis. Orbital pseudotumour has been reported as a manifestation of this condition. Three patients with multifocal fibrosclerosis in whom orbital involvement was the dominant feature are described.

[Surgical treatment of orbital fracture].

In last six years, 51 cases (34 males, 17 females) of orbital deformities caused by fracture were treated. According to careful examinations, x-ray and CT scan, the operation plan was made. The coronal incision or plus lower eyelid incision were employed. Subperiosteal dissection began from the upper margin of the orbit to release input soft tissue and reposition the eyeball. Rib bone was grafted for orbital defects. Canthoplasty was used if necessary. The surgical results were satisfactory.

Recurrent spheno-orbital meningioma.

A series of 15 patients who underwent neurosurgical procedures for recurrent spheno-orbital meningioma is reported. There were 11 women and four men, with a mean age of 46 years. The mean duration between the first and second operations was 46 months. Progressive proptosis without neurological deficit was the most common symptom. All tumors were large at the time of reoperation and involved the greater and lesser wings of the sphenoid bone and the orbit. Aggressive resection in all patients resulted in no deaths and only slight morbidity, with the exception of one patient who developed blindne…

Osseous reconstruction of the midface and orbits.

The midface or orbitonasomaxillary complex is an intricate three-dimensional entity with important functional and aesthetic purpose. Bony reconstruction of defects caused by resection of malignancy, congenital abnormalities, or trauma requires close attention to both function and the complex anatomy. Replacement of bone should be done in the functional anatomic position. Free bone grafts require a well-vascularized bed and may require adjuvant soft-tissue reconstruction prior to their placement. Vascularized bone through the temporoparietal fascial flap and free tissue transfer provides an exc…

Orbital involvement in Wegener's granulomatosis.

Orbital granulomas may complicate the course of Wegener's granulomatosis (WG). Granulomas that grow and compress the optic nerve may lead to blindness. Careful magnetic resonance imaging (MRI) is recommended for early detection and follow-up of intraorbital granulomas. Sufficient systemic immunosuppressive treatment should be given as usual for Wegener's granulomatosis. In order to preserve vision orbital decompression may be necessary in rapidly progressive pseudotumor of the orbit.

Orbital "blow-in" fracture: MRI.

A case of traumatic orbital encephalocele following a "blow-in" fracture is presented. Computed tomography (CT) and magnetic resonance imaging (MRI) findings are shown. Although CT was useful for identifying the orbital roof fracture, bone fragments and soft tissue abnormalities, MRI was more sensitive to brain herniation and an intraorbital haematoma.

A wooden orbital foreign body.

A case is reported in which a patient presented, 18 months after initial injury, with a progressive proptosis of the left eye. X-rays did not reveal a foreign body but a fracture of the lateral orbital wall and an area of soft tissue density were seen on the CT scan. Exploration of the orbit revealed a 2 cm wooden fragment in the floor of the orbit.

[Peri-orbital surgical approaches].

After having made a definitive diagnosis in patients with orbito-frontal pathology, the choice of incision is an important factor to achieve good intra-operative exposure of the region, as well as to obtain a good functional and aesthetic result. In the majority of cases we prefer to utilize the incisions commonly used for blepharoplasty, especially the transconjunctival approach popularized by Tessier. Other more classic accesses such as the superolateral, infra-orbital and intra-oral incisions, are reserved for treatment of cases of orbitomalar (communition) fractures. We feel there are limi…

[The dumb-bell orbital dermoid].

Bony resorption was a characteristic radiogram finding in a series of 25 cases of dumb-bell orbital dermoid, and a definitive diagnosis of the nature and locality of the tumor could be done by ultrasonography, CT, or MRI. In all the 25 cases, the mass was lodged between the orbit and the temporal fossa, the frontal sinus, or the cranial fossa like a dumb-bell. The authors emphasized the use of X-ray, ultrasonography, CT or MRI before operation, and thorough clearance of the cystic and fistula wall with cauterization by carbolic acid was imperative for the prevention of recurrences.

Orbital teratoma associated with trigonocephaly.

A rare case of congenital orbital teratoma that developed concurrently with the clinical findings of metopic sutural synostosis in an infant is presented. A cause-and-effect and/or embryologic relationship between these two cephalic malformations is possible, although coincidental occurrence is equally probable. The proximity of these congenital anomalies permitted complete resolution in a one-stage craniofacial procedure without alteration in the surgical technique.

The skeletal treatment of orbital hypertelorism.

Orbital hypertelorism, strictly defined as an increase in bony interorbital distance, is not itself an isolated syndrome, but is instead an anomaly that may occur as either part of a syndrome or malformation sequence. Evaluation of orbital hypertelorism and the various anomalies that accompany it is best performed by a multidisciplinary craniofacial team. The timing of surgery involves considerations of multiple variables, but when performed can offer marked cosmetic improvements. Equally important as the skeletal surgery is correction of the associated soft-tissue problems.

Orbital roof "blow-in" fractures.

Seven patients with significant frontal trauma involving the supraorbital region associated with orbital roof "blow-in" fractures were admitted to the Maryland Institute for Emergency Medical Services Systems (MIEMSS) at the University of Maryland Medical System over a 16-month period. High-resolution computed tomography (HRCT) with three-dimensional reconstructions demonstrated a downward displacement of the orbital roof in the absence of any orbital rim discontinuity. Associated findings included contussive and hemorrhagic injury to the ipsilateral frontal and parietal lobes, proptosis, ptos…

Orbital lesions with granulomatous inflammation.

Orbital lesions characterized by granulomatous inflammation are a heterogeneous group of diseases of various causes with a common histopathological substrate involving aggregates of epithelioid cells. Forty-one patients (27 females and 14 males) with biopsy-proven granulomatous inflammation were seen at an orbital clinic between 1978 and 1989. The mean age at presentation was 40.2 (extremes 6 and 77) years. Two main clinical presentations were noted: painless, subacute or chronic mass effect, and tender, subacute inflammatory process. Six patients had secondary features that were infiltrative …

Orbital complications of acute sinusitis.

Orbital complications of sinusitis may occur either by spread along the venous system or by direct extension of infection between contiguous structures. The association between sinusitis and inflammation of the orbits has been known for centuries. But in the post antibiotic era there is a marked decline in the number of case reports in the literature. This paper presents 10 cases of sinusitis who had orbital complications. The classification of this disease complex as given by Chandler helps in planning the treatment. Groups I and II respond very well to antibiotics but in groups III, IV, V su…

Primary and secondary orbital teratomas.

Two patients with primary teratomas of the orbit and a third patient with a teratoma invading the orbit from the maxillary sinus are presented. The clinical presentation of each patient was spectacular. In the primary teratomas, the globe was displaced out of the orbit by the attached tumor, causing extreme proptosis. Computed tomography was virtually diagnostic, showing a variegated orbital mass with solid and cystic components. Histologic examination showed multiple tissues derived from two or three germinal layers. The surgical removal of the tumors and reasons for the poor visual outcome a…

[Repair of orbital exenteration cavities].

Several plastic surgical procedures can attenuate the unsightly effects of a disinhabited empty orbit. Only the imperatives of follow-up or very poor general status can still justify spontaneous epithelialisation, which we prefer to split skin graft. In other cases, apart from the exceptional indications of preservation of the eyelids and conjunctival sac, closure by flap is the technique of choice: temporo-frontal flap in cases of simple exenteration and temporo-jugal for the superficial plane and medio-frontal for the deep plane in radical exenterations. In young adults, when the resection i…

Orbital neoplasia with enophthalmos in a cat.

A 9-year-old cat was evaluated for left-sided enophthalmos and protrusion of the third eyelid. A primary orbital fibrosarcoma was diagnosed. During the orbital exploration, normal periorbital fascia could not be identified in the lateral and ventral region, thus accounting for the enophthalmos.
