[Functional impact of carbon fiber springs in ankle-foot orthoses].

BACKGROUND: Patients with calf muscle insufficiency are often dependent on ankle-foot orthoses (AFO). The orthosis should improve walking and posture and should prevent structural deformities. AFOs are usually manufactured with a dorsiflexion stop. In the current study, orthoses with a dorsal carbon fiber spring were compared with the classic design. PATIENTS AND METHODS: Five patients with spina bifida, one with poliomyelitis, and one with a tibial nerve block took part in the study. All participants underwent a 3-D gait analysis including data collection of kinematics (VICON) and kinetics (

Comparison between custom and noncustom spinal orthoses.

Although economically priced OTS orthoses can and should be used under certain prescribed conditions to satisfy carefully described treatment objectives, they cannot control motion as well as custom-molded TLSOs. Patients should not pay for expensive products that cannot help to ensure the best possible clinical outcome. The custom-molded TLSO provides superior fit and support relative to its OTS relative and induces better compliance among its wearers because of its simplicity. It is also the most cost-effective option.

[Mandibular advancement orthoses used in the treatment of obstructive sleep apnea syndrome].

Mandibular advancement devices represent a therapeutic option for obstructive sleep apnea syndrome (OSAS). Clinical improvement has been proven in the different available studies, mainly on nocturnal respiratory events and quality of sleep. Less snoring have been noted by bed partners and objective studies have demonstrated decrease in snoring frequency and intensity. The effect of these ortheses on upper airways resistance syndrome is not yet well documented. The significant clinical improvement is secondary to the decrease in the occurrence of apneas and hypopneas. Polysomnographic improveme

Lumbar spine stability can be augmented with an abdominal belt and/or increased intra-abdominal pressure.

The increased intra-abdominal pressure (IAP) commonly observed when the spine is loaded during physical activities is hypothesized to increase lumbar spine stability. The mechanical stability of the lumbar spine is an important consideration in low back injury prevention and rehabilitation strategies. This study examined the effects of raised IAP and an abdominal belt on lumbar spine stability. Two hypotheses were tested: (1) An increase in IAP leads to increased lumbar spine stability, (2) Wearing an abdominal belt increases spine stability. Ten volunteers were placed in a semi-seated positio

The effectiveness of various cervical orthoses. An in vivo comparison of the mechanical stability provided by several widely used models.

STUDY DESIGN: The amount of motion allowed by various cervical orthoses was compared with the amount of unrestricted neck motion in vivo. OBJECTIVES: To determine how much mechanical stability commonly used cervical orthoses provide to the wearer. SUMMARY OF BACKGROUND DATA: Few studies have compared quantitatively the restriction of motion from cervical orthoses that are commonly prescribed for patients. No studies have used methods allowing measurement of all three rotations throughout the range of motion in passive and active tests. Previous studies may have overstated the amount of restr

[Nocturnal orthosis: a new treatment of compression of the ulnar nerve at the elbow. 30 cases].

Compression of the ulnar nerve at the elbow was confirmed by electromyography in 30 patients including 29 cases of idiopathic aetiology and 1 case of late onset ulnar palsy. Six cases occurred after general anaesthesia and 3 had undergone an unsuccessful operation. All patients were managed solely with an orthesis worn at night. The orthesis held the elbow in 60 degrees flexion. Clinical improvement was noted in all cases (including the 3 cases of unsuccessful surgery) evaluated by the patient and the physician at 88% on the average. Improvement occurred rapidly and was evaluated at 50% on day

Displaced ankle fractures treated surgically and postoperative management.

Ankle fractures are common in young adults, often resulting from athletic activities. They can be subdivided into stable and unstable fractures. The unstable fracture consists of bone or a combined bone and ligamentous injury. In an unstable fracture the talus is displaced 2 mm or more and open reduction with internal fixation is recommended. Good functional results depend on three factors--anatomic restoration of the articulating surfaces, early range of motion, and early weight bearing. The role of early weight bearing is emphasized as a significant factor in contributing to good functional

Joint distraction versus custom-made unloading orthosis for knee osteoarthritis - A clinical and radiological study.

BACKGROUND: A promising treatment option to postpone knee replacement is knee joint distraction (KJD). This treatment shows clinical and radiological improvements, but is invasive, with substantial adverse effects. A custom-made orthosis was developed for unloading the knee, striving for comparable clinical outcomes with KJD with less side effects. The aim of this study is to evaluate the effectiveness of a KJD for 6 weeks compared to a knee-unloading orthosis for 8 weeks on clinical outcomes, progression of osteoarthritis and adverse events. METHODS: Two cohort studies investigating differen

Follow-up after successful treatment for developmental dysplasia of the hip using a Pavlik harness : is two years enough?

AIMS: The length of time for which children, who are treated successfully for developmental dysplasia of the hip (DDH) in a Pavlik harness should be followed up, remains uncertain. The aim of this study was to examine whether children who are treated in this way, who have normal clinical and radiological findings at the age of two years, can be safely discharged from routine follow-up. METHODS: Data were collected prospectively on all 101 children (170 hips) who were treated in a Pavlik harness in our children's hospital between January 2015 and June 2016, with follow-up to the age of five ye

Clinical usability and functional outcomes of a lightweight robotic orthosis (HKAFO) in chronic stroke patients with hemiplegic gait: a pilot study.

BACKGROUND: Gait impairment after stroke limits independence. Powered orthoses may augment task-specific over-ground practice, although clinical uptake is constrained by usability. We evaluated a powered hip–knee–ankle–foot orthosis (HKAFO) for functional outcomes and usability in ambulatory adults with chronic post-stroke hemiparesis. METHODS: In a prospective multicenter single-arm pilot, 15 participants completed six supervised over-ground HKAFO sessions across three weeks (40 min per session). Pre-specified clinical outcomes were the Berg Balance Scale (BBS), Dynamic Gait Index (DGI), and

Outcomes of glenohumeral dysplasia after brachial plexus birth injury using the Sup-ER orthosis.

We investigated the efficacy of the supination-external rotation ('Sup-ER') orthosis, designed as a non-operative treatment to maintain normal anatomical growth of the shoulder, on the progression of glenohumeral dysplasia in patients with brachial plexus birth injuries. The Sup-ER orthosis was fabricated for 20 infants diagnosed with glenohumeral dysplasia after brachial plexus birth injuries and its success in correcting glenohumeral dysplasia was confirmed by objective calculations of the alpha angle on serial ultrasound findings and improvement in Active Movement Scale scores. Of the 20 pa

Comparison of two different orthoses used in the treatment of lateral epicondylitis.

BACKGROUND: Lateral epicondylitis (LE) is one of the most common causes of lateral elbow pain, and orthoses are often used in its conservative treatment. However, the number of studies examining the effectiveness of the orthoses used and their differences compared to each other is limited. OBJECTIVE: To determine the effects of 2 orthoses types, wrist orthosis (WO) and counterforce brace (CB), commonly used in the treatment of lateral epicondylitis on muscle activation, grip strength, hand function, and satisfaction level in healthy individuals. STUDY DESIGN: Quasi-experimental study. METHO

Is There a Benefit to Weaning Pavlik Harness Treatment in Infantile DDH?

BACKGROUND: Following successful treatment of developmental hip dysplasia with a Pavlik harness, controversy exists over the benefit of continued harness use for an additional "weaning" period beyond ultrasonographic normalization versus simply terminating treatment. Although practitioners are often dogmatic in their beliefs, there is little literature to support the superiority of 1 protocol over the other. The purpose of this study was to compare the radiographic outcomes of 2 cohorts of infants with developmental hip dysplasia treated with Pavlik harness, 1 with a weaning protocol and 1 wit

Vertebral fracture during one repetition maximum testing in a breast cancer survivor: A case report.

RATIONALE: One repetition maximum (1-RM) testing is a standard strength assessment procedure in clinical exercise intervention trials. Because no adverse events (AEs) are published, expert panels usually consider it safe for patient populations. However, we here report a vertebral fracture during 1-RM testing. PATIENT CONCERNS: A 69-year-old breast cancer survivor (body-mass-index 31.6 kg/m2), 3 months after primary therapy, underwent 1-RM testing within an exercise intervention trial. At the leg press, she experienced pain accompanied by a soft crackling. DIAGNOSIS: Imaging revealed a parti
