Subacute combined degeneration of the spinal cord following recreational nitrous oxide use.

We describe a case of a young woman who developed myelopathy and peripheral neuropathy following 1 year of recreational nitrous oxide (N2O) use. She presented with uncomfortable tingling sensation in her feet and poor balance. Physical examination revealed mild weakness and hyper-reflexia in the lower limbs. Proprioception and vibration were absent in the lower limbs. Pinprick and light touch sensations were reduced below the sixth thoracic dermatome. A broad-based gait and a positive Romberg sign were noted. The level of vitamin B12 was low (76 pmol/l). T2-weighted MRI scans showed hyperinten…

The role of MRI in spinal stab wounds compared with intraoperative findings.

INTRODUCTION: Spinal stab wound injuries are quite rare and only few patients have been reported on the basis of MRI scan. METHODS: A 25-year-old man was stabbed at C1/2 and had an incomplete Brown-Sequard syndrome. He underwent surgical exploration because of CSF leakage on the fourth day. RESULTS: After a follow-up period of 32 months, he was left with a remaining loss of the proprioception of the right foot. We show detailed CT and MR images with the focus on the lesions of the dura and myelon and compared them with intraoperative images. In addition, we contrast our findings with a revie…

Brown-Séquard-plus syndrome because of penetrating trauma in children.

Brown-Séquard syndrome is an uncommon condition involving incomplete spinal cord injury, with ipsilateral motor and proprioception loss, contralateral pain, and decreased temperature. Brown-Séquard-plus syndrome is associated with additional neurologic findings involving the eyes, bowel, or bladder. We describe an adolescent with Brown-Séquard-plus syndrome attributable to a stab injury. Our patient's clinical features of spinal and neurogenic shock overlapped at presentation. He was managed with high-dose steroids, along with intense physiotherapy and rehabilitation, resulting in good neurolo…

Non-operative rehabilitation for traumatic and atraumatic glenohumeral instability.

Glenohumeral joint instability is a common pathology encountered in the orthopaedic and sports medicine setting. A wide range of symptomatic shoulder instabilities exist ranging from subtle subluxations due to contributing congenital factors to dislocations as a result of a traumatic episode. Non-operative rehabilitation is utilized in patients diagnosed with shoulder instability to regain their previous functional activities through specific strengthening exercises, dynamic stabilization drills, neuromuscular training, proprioception drills, scapular muscle strengthening program and a gradual…

Fibularis Intersection Syndrome: A Rare Cause of Lateral Foot Pain in Runners.

Running is a common cause of foot and ankle injuries. Accurate diagnosis is important for appropriate management to resolve the issue. For clarity of diagnoses, the physician should be able to perform a thorough physical examination in conjunction with point-of-care ultrasound. The authors report a case study of a young male runner with lateral ankle pain. Based on clinical assessment and point-of-care ultrasound, a diagnosis of fibularis intersection syndrome was proposed. This type of injury can be treated with graded eccentric loading of fibularis tendons and proprioceptive exercises. This

Ankylosing Spondylitis and Balance.

Ankylosing spondylitis is a chronic inflammatory disease that mainly affects axial joints and the changes in the vertebrae cause typical kyphotic posture. In patients with ankylosing spondylitis, the postural changes, including cervical flexion, decreased lumbar lordosis, posterior rotation of pelvis, hip extension, knee flexion, and plantar flexion of ankle along with kyphosis, may occur in the upcoming terms of the disease and may negatively affect balance. Joint and eye involvements; muscular, vestibular and proprioceptional changes can also affect balance. This review evaluates the ankylos…

IMAGING DIAGNOSIS-IMAGING AND HISTOPATHOLOGIC CHARACTERISTICS OF A VERTEBRAL HAMARTOMA IN A CAT.

A 9-month-old domestic shorthair cat had progressive ambulatory paraparesis, proprioceptive ataxia, and thoracolumbar hyperesthesia. An extradural mass affecting the left pedicle and lamina of the second lumbar vertebra (L2) causing marked spinal cord impingement was identified in magnetic resonance (MR) images. The mass was predominantly calcified in computed tomographic (CT) images. A hemilaminectomy was performed to resect the mass. Clinical signs were greatly improved at 12-month follow-up. The histopathologic diagnosis was vascular hamartoma. To our knowledge, this is the first report des

Cerebellar cystic meningioma in a dog.

A 21-kg, seven-year-old, male, mixed-breed Labrador retriever was admitted for incoordination and a head tilt of approximately three months' duration. Ataxia was noted of the trunk and limbs, and there was a head tilt to the right side. Conscious proprioceptive deficits were present in the left thoracic and pelvic limbs (i.e., hemiparesis). These abnormalities were consistent with paradoxical vestibular syndrome and a lesion involving the caudal cerebellar peduncle. A mass lesion consisting primarily of fluid was present on magnetic resonance imaging and at craniectomy. Histopathological diagn

Back injuries in the young athlete.

The diagnosis of back pain in the young athlete should be specific and not attributed to nonspecific, mechanical causes. Risk factor identification and intervention are required. Treatment is then initiated in a specific pattern, addressing flexibility and muscular imbalances. Bracing is often used to allow healing of growth tissue. The lumbosacral orthosis may be molded in a lordotic posture to unload the disc or antilordotic posture to relieve the posterior column; however, customizing the lordosis to the individual biomechanics may be required. Spinal stabilization is initiated with therapy

Reconstruction of the lateral ankle ligaments using an inferior extensor retinaculum flap.

The aim of this study was to assess the results of 32 cases of chronic ankle instability. These were treated by ligament shortening and reinforced with an inferior extensor retinaculum flap. All patients complained of persistent functional instability unrelieved with proprioceptive exercises. Results were assessed clinically (pain, instability, recovery of sports activity, mobility) and radiologically (correction of laxity on stress x-rays). This enabled us to draw up a revision score on a scale of 100 points. We obtained a mean score of 86.7 points (45-100 points), and subjective results show

Locomotion and motion sickness during horizontally and vertically reversed vision.

Locomotion and motion sickness during reversed vision were studied in ten normal subjects and a patient with bilateral labyrinthine loss. Whereas horizontal reversal produced moderate to severe gait disturbances as well as motion sickness in all normal subjects, vertical reversal failed to induce such symptoms. The patient, being free of motion sickness during both reversals, could not walk straight during horizontal reversal. The difference in the strength of sensory mismatch between both directions seemed to result from a difference in the role of vision for spatial orientation which is prod

Afferent limb of the human jaw reflex: electrophysiologic and anatomic study.

Jaw reflexes were recorded before and after neurosurgery of the trigeminal sensory root or the gasserian ganglion in 11 patients with idiopathic trigeminal neuralgia and in 2 patients with orbital malignancies. The reflex was also studied in three patients with unilateral midbrain lesions verified by postmortem findings. Jaw reflex abnormalities appeared in all cases, although there were no clinical or EMG signs of masseter dysfunction. Proprioceptive afferent fibers of the jaw reflex therefore seem to be mediated by the sensory and not the motor root. The afferent limb of the jaw reflex seems

Improved hamstring strength and knee position sense are associated with enhanced landing mechanics after anterior cruciate ligament reconstruction.

INTRODUCTION/OBJECTIVES: Return-to-sport decisions after anterior cruciate ligament reconstruction (ACLR) rely on diverse criteria, including neuromuscular performances and functional testing. This study investigates the relationships between neuromuscular parameters, specifically angle-specific strength and proprioception, and knee kinematics during the landing phase of a single-leg hop task in patients 6-12 months post-ACLR. METHODS: This cohort study included 25 participants (44% women; mean age 28 ± 10 years) who underwent primary ACLR. Demographic and surgical data were collected at 6 …

A session of transcutaneous electrical nerve stimulation changes the input-output function of motoneurons and alters the sense of force.

Transcutaneous electrical nerve stimulation (TENS) is commonly used in research and clinical settings for pain management and augmenting somatosensory inputs for motor recovery. Besides its functional effect, TENS acutely alters kinesthesia and force steadiness. However, the short-term impact following a session of TENS on proprioception and motor unit behavior is unknown. We evaluated the effect of a session of TENS on the senses of force, joint position, touch, and discharge activity of motor units. Fifteen healthy participants underwent two experiments, each with two visits randomly adminis…

A role for proprioceptors in sngception.

Proprioceptors are primary mechanosensory neurons to monitor the status of muscle contraction and/or body position (1). Although propriocept … ut not pain. Conversely, in a spinal cord-injured patient who lost pain sensation in the right leg, proprioception and sngception were remaining somatosensory functions, associated with the spinal dorsal column. Together, evidence from both mouse and human studies suggests a role for proprioceptors in sngception.

[Charcot's osteoarthropathy : How to identify a superimposed osteomyelitis?].

Charcot's osteoarthropathy is defined as an osteodestructive process characterized by peripheral neuropathy with lack of protective proprioception in which improper weight bearing leads to recurrent microtrauma, triggering of a proinflammatory process, pathological bone remodeling, joint instability, and subsequent joint destruction. The most common cause is diabetes which can lead to neuropathy over time. Magnetic resonance imaging is the most important imaging modality enabling subtle analysis of soft tissue and bony changes. Thus, it plays a key role in detection of early disease stages, as…
