Concomitant avascular necrosis of the scaphoid and lunate.

Simultaneous avascular necrosis of multiple carpal bones is rare. Concomitant avascular necrosis of scaphoid and lunate has been reported only once. We report one more case of this rare condition which can be a cause of wrist pain. Steroid intake is a known risk factor for avascular necrosis but in our case the patient had been taking herbal medicines for joint pain the composition of which was not known. Probably the presence of steroid in these medicines was the cause of avascular necrosis in this case.

[Arthrosis of the wrist joint due to carpal instability. Therapeutic alternatives].

A kinematic analysis of the movement of the scaphoid, the lunatum and the triquetrum has shown that their turning axes do not correspond with each other. Without arthritis, and within the range of normal motion, the bones move without imposing any appreciable load on the ligaments. They merely guide the bones and hold them together. In the case of arthritic disorders and in states where abnormal frictional resistance occurs, or following trauma, the ligaments are highly loaded. This could lead to increased wear and ruptured ligaments. A series of 20 arthroscopies has shown that LT and SC ligam

Chronic asymptomatic contralateral wrist scapholunate dissociation.

BACKGROUND: The authors report a series of six patients who presented with scapholunate dissociation, with no significant contralateral antecedent trauma and no rheumatoid arthritis or congenital ligamentous laxity. Such patients may present with pain or a snapping/popping sensation in one wrist, undergo radiographs of both wrists, and are discovered to have scapholunate gapping bilaterally. The literature contains few reports of this condition, and this series of six is a relatively large one for this infrequently reported condition. METHODS: The six charts were reviewed retrospectively; eac

Non-scaphoid carpal fractures: lessons learned from a series of 86 cases.

PURPOSE: To describe the epidemiology, injury mechanisms, diagnostic approach, management, and clinical outcomes of non-scaphoid carpal fractures, which are frequently overlooked or diagnosed late. METHODS: A retrospective review was conducted of patients with non-scaphoid carpal fractures treated at our institution between 2013 and 2020. Demographic data, fracture characteristics, time to diagnosis, treatment strategy, rehabilitation requirements, and clinical outcomes, including complications, were analyzed. RESULTS: 76 patients with 86 fractures were included; 54 (71%) were male. The most c

Open Volar STT Ligament Reconstruction to Augment the Mathoulin's Arthroscopic Dorsal Capsuloligamentous Reconstruction: Technique Description and Case Reports.

Background  The results of Mathoulin's arthroscopic dorsal capsuloligamentous reconstruction (ADCLR) are excellent in many patients with scapholunate instability, though less consistently good in higher grade instabilities. The purpose of this article is to describe a novel technique of volar scaphotrapeziotrapezoid (STT) reconstruction which may be used to augment rotational control of the scaphoid, in conjunction with the ADCLR, for use in European Wrist Arthroscopy Society (EWAS) grade IV/V instabilities. Description of Technique  Following completion of ADCLR, the STT joint is approached t

MDCT and radiography of wrist fractures: radiographic sensitivity and fracture patterns.

OBJECTIVE: The purpose of our study was to determine which wrist fractures are not prospectively diagnosed at radiography using CT as a gold standard and to identify specific fracture patterns. MATERIALS AND METHODS: Through a search of radiology records from January 1 to December 31, 2005, 103 consecutive patients were identified as having radiographic and CT examinations of the wrist. After excluding incomplete or nondiagnostic examinations and those with a greater than 6-week interval between imaging studies, the final study group consisted of 61 wrist examinations in 60 patients. Two musc

Carpal fractures in children.

Carpal injuries in children are relatively rare. Diagnosis is likely to be missed, or the severity of the injury might not fully be appreciated at the time of presentation. Fortunately, in the majority of cases the fracture heals uneventfully with a low incidence of complications. As in adults, the scaphoid is most susceptible to injury and the capitate, which often presents in association with other carpal bone injuries, the next most susceptible. Other carpal bone fractures are very unusual and the literature largely is confined to case reports or very small series. This review article lists

Fractures of the hand. Distribution and relative incidence.

One thousand consecutive fractures of the metacarpals, phalanges, and carpal bones have been recorded over a period of about 10 months in Bergen, Norway, to find out the relative incidence of these fractures in an unselected series: this corresponds well with the few other reports of the incidence of hand fractures that we could find. In the total series the metacarpals, phalanges, and carpal bones account for 36%, 46%, and 18% of the fractures, respectively. Fractures of the scaphoid make up 10.6% of the total, fractures of the neck of the fifth metacarpal 9.7%, and Bennett's fractures 1.4%.

The Herbert screw for the treatment of scaphoid fractures.

Forty-nine patients with fifty fractures of the scaphoid were reviewed more than six months after surgical treatment using the Herbert bone screw. Twenty-nine patients (mainly those with delayed union and non-union) also had bone grafts. The mean period of follow-up was 18.3 months. In 47 patients (94%) the fracture had definitely or probably united. The three patients whose fractures did not unite had proximal pole fractures with pre-existing avascular necrosis and osteoarthritis. Immediate post-operative mobilisation was possible in thirty patients. Wrist function as measured by grip strengt

Robot-assisted scaphoid screw fixation versus free-hand technique for scaphoid fractures: a systematic review and meta-analysis.

This meta-analysis and systematic review set out to compare the results of robot-assisted scaphoid screw fixation to those of more conventional, manual methods of fixing scaphoid fractures. The studies pertaining to scaphoid fractures published from database inception to January 2024 were culled from PubMed, CNKI, Embase, and the Cochrane Library’s extensive database. Included studies compared conventional surgical procedures with robot-xassisted scaphoid screw fixation. Both English and Chinese versions of these articles were taken into consideration. The selection process followed recognized

Surgical options and outcomes for treatment of osteoarthritis of the scaphotrapeziotrapezoidal joint.

Osteoarthritis of the scaphotrapeziotrapezoidal joint is frequent but often pain-free or well tolerated with non-surgical treatment. Surgical options are numerous and none seems to have clear evidence of superiority. In addition to well-established procedures, such as scaphotrapeziotrapezoidal joint arthrodesis, distal scaphoid resection and trapeziectomy, more recent surgical techniques have been reported. These include scaphotrapeziotrapezoidal arthroplasty and arthroscopic treatment. This article provides an overview of the procedures and their outcomes that can be proposed for isolated sca

The "Standing Peanut" Scaphoid View: A Semi-supinated Radiographic View for Intraoperative Evaluation of Screw Placement in Scaphoid Waist Fractures.

Scaphoid waist fractures are the most common fracture of the scaphoid. Operative management is indicated with unstable fractures and often for nondisplaced waist fractures to decrease time to union and return to work/sport. Screw placement within the central axis of the scaphoid is paramount and correlates with outcomes. Assessment of intrascaphoid screw placement is classically done via intraoperative fluoroscopy. An additional fluoroscopic view is presented to assist in confirming implant positioning. Along with the standard anterioposterior, lateral, pronated oblique, and "scaphoid" view we

[Scaphoid percutaneous osteosynthesis by screw using computer assisted surgery: an experimental study].

Scaphoid fractures are sometimes difficult to diagnose and even more difficult to fix. Recent progress such as miniaturization of osteosynthesis material, adoption of the percutaneous route, and widening of the indications to include undisplaced fractures has still not abolished complications. In this context, computer assisted surgery (CAS) may be useful and deserves further study. To apply it to the scaphoid, it is initially necessary to immobilize the "wrist hand fingers" unit in a device adapted to make it a rigid unit. It is then necessary to choose the correct configuration of CAS system

Dorsal transscaphoid-transtriquetral perilunate dislocation in pseudarthrosis of the scaphoid.

A patient reported that he sustained a minor fall on the outstretched hand in hyperextension, pronation, and in ulnar deviation. Initial radiographs suggested dorsal transscaphoid-transtriquetral perilunate dislocation. Traditionally, however, this injury is the result of a high-energy impact. A CT scan obtained after closed reduction of the dislocation revealed not only a fresh fracture of the triquetrum but also two corticalized fragments of the scaphoid. A former major fall on this hand and a normal scaphoid of the other hand made pseudarthrosis more likely than scaphoid bipartition. Arthro

[A comparison of different treatment methods of pseudarthroses and recurrent pseudarthroses of the scaphoid].

From 1981 to 1990 110 scaphoid non-unions were operated on in our clinic. The period from 1986 to 1990 was followed up. During this time, 45 cases were surgically treated: 27 by Matti-Russe plasty, 11 by Herbert screw, and 7 by vascularized radial bone graft. The following criteria were compared: 1. Radiological signs of healing; 2. Period of immobilization; 3. Clinical results. The shortest period of immobilization and best wrist function was observed in patients treated by Herbert screw.
