When two rare diseases meet: atypical hemolytic uremic syndrome and alkaptonuria in two siblings.

Atypical hemolytic uremic syndrome (aHUS) is a rare complement-mediated thrombotic microangiopathy (TMA), whereas alkaptonuria (AKU) is an uncommon inherited metabolic disorder. Their coexistence has not previously been described. We report two siblings from a consanguineous family with CD46-associated aHUS and AKU to highlight an unusual diagnostic challenge and the importance of recognizing coexisting inherited diseases. Two brothers experienced recurrent episodes of TMA characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury, while maintaining preserved…

Siblings of young children with congenital heart disease: parent perspectives from a crowdsourcing study.

OBJECTIVE: To gather parents' perspectives on the experiences of siblings of young children with congenital heart disease (CHD), the impact of CHD on siblings, and the types of resources and supports they need to adjust to CHD within their family. METHODS: A community advisory council guided the study. Parents of children with CHD, currently 1-7 years old, who had surgery in their first year of life, were eligible for participation if they were fluent in written English and had internet access. Recruitment through several CHD-specific nonprofit organizations produced a national sample of pare…

Experiences during childhood and adolescence among siblings of children with childhood-onset chronic illness.

BACKGROUND: It remains unclear what experiences siblings of children with childhood-onset chronic illnesses have across settings such as home, school, and healthcare. Although these experiences can have long-term psychosocial effects, how they are interpreted in adulthood is not yet well understood. AIM: This study examined the experiences of adult siblings during their school-age and adolescent years, as well as how they interpreted and made sense of those experiences. METHODS: A qualitative descriptive study using criterion-based purposive sampling was conducted with 14 adult siblings recr…

Rhizomelic short stature with dysmorphism in two siblings due to PKDCC gene pathogenic variants.

Skeletal dysplasias are a common cause of short stature and are often linked with multisystemic features. The Hedgehog signaling pathway plays a vital role in skeletal development and is regulated by the protein kinase domain-containing cytoplasmic (PKDCC) gene, also known as vertebrate lonesome kinase. Pathogenic variants in the PKDCC gene have been reported in very few patients worldwide and are inherited in an autosomal recessive pattern. We report 2 siblings-a 6-year-old girl and a 3-year-old boy-who present with rhizomelic short stature, facial dysmorphism, low-set ears, and umbilical her…

Biallelic SLCO2A1 variants in two siblings with primary hypertrophic osteoarthropathy and possible chronic enteropathy.

BACKGROUND AND AIMS: Pathogenic variants in the SLCO2A1 gene are responsible for two rare monogenic disorders: primary hypertrophic osteoarthropathy (PHO) and chronic enteropathy (CEAS). The clinical and genetic characteristics of individuals with either PHO or CEAS have been extensively documented in previous studies. However, the clinical and genetic features of patients presenting with both PHO and CEAS concurrently remain largely unknown. This study aims to elucidate the clinical and genetic features of patients with overlapping PHO and CEAS. METHODS: We evaluated two affected individuals

Children's participation in assent to paediatric oncology research: experiences of children, siblings and parents.

BACKGROUND: Research is essential for improving outcomes in paediatric oncology, yet decisions about research participation often occur during emotionally vulnerable periods for families. Although children's assent is ethically required, it is not always meaningfully implemented in practice. Understanding how children, siblings and parents experience the child's involvement in the assent process, including their attitudes towards research, is crucial for promoting ethically sound research and supporting genuine child participation. The aim of this study was to describe how children with cancer…

Keeping Up With School During Hospitalization for Children With Chronic Illnesses, Siblings and Parents.

BACKGROUND: Children who have chronic illnesses (CIs) and their siblings often miss school when the child with a CI is hospitalized. Understanding the perspectives of the child with an illness, siblings and parents will provide information about how they keep up with schoolwork, their perceptions of not being in school, or of homeschooling that will inform educators, families and medical professionals. METHODS: English-speaking children diagnosed with CI, siblings and parents participated. They were residing at a Ronald McDonald House. A qualitative study using interviews with children with C…

Ichthyosis Prematurity Syndrome Caused by a Novel Homozygous SLC27A4 Mutation in Two Emirati Siblings.

Ichthyosis prematurity syndrome (IPS) is a rare autosomal recessive congenital ichthyosis caused by variants of the SLC27A4 gene. It is characterized by the clinical triad of premature birth, clay-like vernix at birth, and respiratory complications. Although the skin manifestations tend to improve with age, patients may continue to exhibit mild ichthyosis and atopic manifestations. We describe the first cases of two Emirati siblings with IPS caused by a novel homozygous SLC27A4 variant. Although the patients shared the same variant, they manifested differently, as one exhibited atopic features…

Medical Decision-Making for Children in Families with Siblings: parental discretion and its limits.

This article examines how parents should make health decisions for one child when they may have a negative impact on the health interests or other interests of their siblings. The authors discuss three health decisions made by the parents of Alex Jones, a child with developmental disabilities with two older neurotypical siblings over the course of eight years. First, Alex's parents must decide whether to conduct sequencing on his siblings to help determine if there is a genetic cause for Alex's developmental disabilities. Second, Alex's parents must decide whether to move to another town to ma…

A Novel Pathogenic Mutation in WNK1 Gene Causing HSAN Type II in Three Siblings.

Hereditary sensory and autonomic neuropathy (HSAN) is a rare genetic disorder that primarily affects the peripheral nervous system, leading to a progressive loss of the ability to perceive pain, temperature, and touch. This condition can result in severe complications, including injuries and infections due to the inability to feel pain. HSAN is classified into nine types, with types I and VII exhibiting autosomal dominant inheritance, while the others follow an autosomal recessive pattern. In this study, we examined three affected brothers of Turkish Azeri descent, aged 20, 23, and 25 years. T

THYROID DYSFUNCTION CAUSED BY MATERNAL AUTOIMMUNE THYROIDITIS: TWO DIFFERENT CLINICAL PICTURES IN TWO SIBLINGS.

OBJECTIVES: Maternal antibodies in cases of chronic autoimmune thyroiditis may be transferred to the baby via the transplacental route, leading to transient hyperthyroidism or hypothyroidism. The development of hyperthyroidism in one sibling and hypothyroidism in the other, however, is an extremely rare condition. We present two siblings, one with transient neonatal hyperthyroidism and the other with transient neonatal hypothyroidism, born to a euthyroid mother who was being treated for Hashimoto's thyroiditis. CASE PRESENTATION: Case 1: A term male baby was evaluated due to tachycardia, high…

Fine Motor Skills in Children with Tourette Syndrome and their Unaffected First-degree Siblings.

BACKGROUND: The exact etiology of Tourette Syndrome (TS) remains unclear, making the search for impaired neuropsychological functions possibly connected to the underlying cause of TS as important as it is challenging. One neuropsychological domain of interest is fine motor skills. METHOD: This study compared fine motor skill performance on the Purdue Pegboard Task (PPT) in 18 children with TS, 24 unaffected first-degree siblings and 20 controls. A set of screening questionnaires was administered to determine comorbid psychiatric illness. RESULTS: Children with TS, their siblings and controls…

Two siblings with Bosch-Boonstra-Schaaf optic atrophy syndrome due to parental gonadal mosaicism.

Bosch-Boonstra-Schaaf Optic Atrophy Syndrome (BBSOAS, OMIM 615722) is a rare autosomal dominant disorder characterized by intellectual disability, optic atrophy, cortical visual impairment, mild facial dysmorphism, hypotonia, hearing problems, attention deficit and a thin corpus callosum. The gene underlying this disorder is NR2F1 located on chromosome 5q15 which encodes for a nuclear receptor protein. Mutations and deletions have been identified in patients. Here we report on a brother and a sister carrying a pathogenic nonsense NR2F1 variant. The patients have a mild phenotype showing optic

Case Report: Novel Pathogenic Variant Detected in Two Siblings with Type 1 Gaucher Disease.

BACKGROUND: Gaucher disease (GD) is an autosomal recessive lysosomal storage disease. The disease develops due to glucocerebrosidase enzyme deficiency caused by biallelic pathogenic variants in the glucosylceramidase beta 1 (GBA1) gene, which encodes the glucocerebrosidase enzyme. The GBA1 gene is located at chromosomal location 1q22 and consists of 11 exons. In this article, we report a novel pathogenic variant in the GBA1 gene. CASE PRESENTATIONS: A 32-year-old female patient with no known chronic disease was admitted with complaints of weakness, bone pain, and abdominal pain. Her evaluatio
