High-Altitude Pulmonary Edema Management in a Special Operator.

This case report delves into the challenges in diagnosis and management of a 30-year-old Army Special Forces officer who experienced respiratory distress during high-altitude training in Nepal, where he gained 3,000m in elevation over 3 days. Notably, the patient was prophylactically treated with acetazolamide but did not receive nifedipine or tadalafil. At an elevation of 13,550 feet (4,130m), the patient developed classic high-altitude pulmonary edema (HAPE) symptoms as well as concomitant respiratory symptomatology of breathlessness and worsening productive cough. The complexity of this cas

Specialized vascular niches preserve adult neural circuits.

In this issue of Neuron, Zhu et al.1 show that Netrin-4 from specialized arterial smooth muscle cells restrains a latent death pathway in a subset of adult sympathetic neurons, revealing a neurovascular mechanism that selectively maintains mature neural circuits through spatially restricted survival signals.

Cost-effectiveness of the special care unit for persons with dementia.

INTRODUCTION: We aimed to estimate the cost-effectiveness of the patient-centered special care unit for behavioral and psychological symptoms of dementia (SCU-B) in the Respectful Caring for the Agitated Elderly (RECage) study. METHODS: A health-economic evaluation was performed alongside a controlled European multicenter three-year longitudinal cohort study enrolling 508 participants in a non-SCU-B cohort and SCU-B cohort. Health service resource use, costs, quality-adjusted life years (QALYs), and incremental cost per QALY gained were assessed. RESULTS: Total QALYs were lower (-0.13; boots

Preface to the Special Issue: "Microbiota and Gut-Brain Axis".

The microbiota-gut-brain axis has emerged as a central framework for understanding how microbial communities influence nervous system development, function, and disease. This Special Issue highlights three interconnected themes: microbial metabolites and neurochemistry, developmental and neuroimmune mechanisms, and translation to neurological and psychiatric disease. The articles in the collection examine how microbial metabolites modulate neuroinflammatory and neurochemical pathways, how stress, development, immune signaling, and gut health shape gut-brain communication across the lifespan, a

Advances in the role of specialized pro-resolving mediators in periodontitis.

Periodontitis is a chronic inflammatory disease driven by dysregulated host-microbial interactions and failure of endogenous inflammation resolution, leading to progressive destruction of periodontal tissues. Increasing evidence indicates that inflammation resolution is an active, tightly regulated biological program governed by specialized pro-resolving mediators (SPMs), including resolvins, lipoxins, protectins, and maresins. Unlike conventional anti-inflammatory agents, SPMs do not suppress host defense but instead actively terminate inflammation and promote tissue regeneration. Mechanistic

Special screw corridors and imaging in pelvic ring trauma.

Adequate intraoperative visualization is mandatory for implant application in pelvic ring injuries. Several fluoroscopic X-ray views are in practical use. The gold standard primary X-ray is the anteroposterior view of the pelvis. In addition to this view, oblique views for pelvic ring instabilities and acetabular fractures are well defined. Combinations of these views allow better identification of osseous corridors for screw applications. These corridors are based on the 3-ring concept of the hemipelvis. For pelvic ring stabilization the main osseous corridors include the retrograde and anteg

Special Considerations in Pediatric Inflammatory Bowel Disease Pathology.

Inflammatory bowel disease (IBD) in the pediatric population presents distinct characteristics compared to adult cases. Pathology plays a critical role in its diagnosis, and this review underscores key considerations in the pathologic evaluation of pediatric IBD. Recognizing inflammatory patterns in the upper gastrointestinal tract can improve disease classification and aid in diagnosing IBD in certain scenarios, such as isolated upper gastrointestinal or small bowel involvement. Additionally, familiarity with distinctive subtypes, including IBD associated with primary sclerosing cholangitis a

Special section: Self-other distinction in personality disorders.

Problems with self- and interpersonal functioning are defining features of personality disorders (PDs) that seem to stem from impairments in self-other distinction (SOD), that is, the sociocognitive capacity to distinguish between one's own and others' mental and physical experiences. There has been recent renewed interest in this topic across a wide range of fields, from clinical psychology to social neuroscience and experimental psychology. This special section on "Self-Other Distinction in Personality Disorders" is therefore very timely, particularly given the shift to dimensional views of

Shining a Special Light on an Unusual Case of Thoracic Lymphadenopathy.

Amyloidosis is an uncommon disease characterized by the buildup of misfolded protein fibrils outside cells in various tissues. Its diagnosis relies on tissue biopsy confirmation. Due to its diverse clinical presentations, diagnosis is often delayed. The symptoms depend on the organs affected, with the kidney and heart being most frequently involved. Although pulmonary involvement occurs relatively often, it seldom causes symptoms. Pulmonary amyloidosis can manifest as nodular pulmonary amyloidosis, diffuse alveolar-septal amyloidosis, or tracheobronchial amyloidosis. This case highlights a rar

Management of Cutaneous Melanoma, With a Special Focus on Desmoplastic Melanoma.

Not all melanomas are the same. They differ in presentation, mutational status, and behavior. Desmoplastic melanoma (DM) is a rare subtype of melanoma which carries a high tumor mutation burden and is found in highly sun-exposed areas. Although many cutaneous melanomas are pigmented, DM is amelanotic and frequently mistaken for more benign lesions. It is composed of spindle cells surrounded by a dense fibrinous matrix. It frequently carries NF1 mutations. It is exquisitely sensitive to single-agent PD1 blockade resulting in deep, rapid responses at all stages. These responses with single-agent
