Pancreatic stone formation is characteristic of chronic pancreatitis. Ductal obstruction is a major cause of pain and the principal disabling symptom of the disease. This article briefly reviews the pathophysiology of pancreatic stone formation and describes the current nonsurgical range of therapeutic modalities.

With the advent of laparoscopic techniques and other nonoperative techniques, the management of patients with common bile duct (CBD) stones became more complex. Sphincteroplasty is an internal biliary drainage technique that, along with choledochoduodenostomy, is offered to surgeons as one of the most effective techniques. We report our results and conclusions in a two-year study of a group of 46 patients. We prefer sphincteroplasty in the majority of patients with papillary sclerosis and consider it the most anatomic and physiologic drainage technique, since it eliminates the persistence of a cul de sac and the possible reflux of intestinal contents. Peribiliary varices are a rare complication of chronic portal hypertension and can lead to common bile duct narrowing. Because of this, they can complicate the management of biliary obstruction secondary to choledocholithiasis. Instrumentation such as during endoscopic retrograde cholangiopancreatography poses the risk of severe hemorrhage. INTRODUCTION: Brunner's gland hyperplasia (BGH) of the duodenum is an uncommon finding at endoscopy and is usually asymptomatic. Symptomatic BGH presenting with biliary obstruction and pancreatitis is extremely rare and the literature is limited to a few case reports.
CASE: We report an unusual case of diffuse BGH of duodenum involving the ampulla of Vater that led to this clinical presentation. Biopsies revealed BGH and successful endoscopic therapy utilizing sphincterotomy and plastic stent placement led to complete recovery.

One hundred and twenty cases of fistula-in-ano were treated in 11 years by fistulectomy and primary closure. Injection of dye into the tract helped in identifying the tract and its ramifications which were excised. A lateral sphincterotomy was added in most cases for relief of postoperative pain and to promote good healing. We present the case of a 79-year-old male who underwent endoscopic retrograde cholangiopancreatography (ERCP) after cholangitis. The papilla was rigid and the biliary tract was dilated with sharpening of the distal bile duct, with no obvious cause.
