Personalized approach for complex bilateral calcaneal osteomyelitis and defect reconstruction with bilateral abductor digiti minimi flaps.

OBJECTIVES: The best treatment for displaced, intra-articular fractures of the calcaneus remains controversial and it is generally agreed, that there is no single method that is suitable for all patients. CASE PRESENTATION: Here we report a rare case of bilateral calcaneal osteomyelitis with fistula formation following open reduction and plate fixation via an extensile lateral approach that could be salvaged with an interdisciplinary approach including orthopedic and plastic surgeons. We are not aware of a similar case in the literature. Abductor digit minimi flaps is a well-established proce

The use of topical vasodilators in microvascular surgery.

Topical vasodilators are widely used in reconstructive microsurgery in order to ameliorate intraoperative vascular spasm (vasospasm) and facilitate microvascular anastomoses. There is, however, a paucity of literature to support or discredit this practice. A survey of current practice in UK Plastic Surgery Departments was therefore undertaken. Email questionnaires were sent to all 281 consultant plastic surgeons in the UK and we received responses from 91 representing 35 out of the 49 'main' Plastic Surgery Units (71%). Of the 84 consultants who completed the questionnaire, the majority (94%)

Communication and Branding in Aesthetic Surgery: Insights from an ISAPS Fellowship.

Patient communication and ethical branding are crucial determinants of satisfaction and outcomes in aesthetic surgery. This article summarizes insights from an ISAPS Fellowship at Velthuis Kliniek, Rotterdam, under Dr. H.P.Jeroen Stevens and Dr. Jacques van der Meulen, highlighting strategies for patient-centered consultations and professional branding. Consultations emphasized precise, constructive language, avoiding anxiety-inducing terms such as "complication" or "revision," and favoring expressions like "refinement" and "individual variability" to manage expectations effectively. Vacuum-assisted closure: state of clinic art.

Treatment of wounds has been the cornerstone of plastic surgery since its inception. Vacuum-assisted closure provides a new paradigm that can be used in concert with a wide variety of standard existing plastic surgery techniques. It was originally developed as an alternative treatment for debilitated patients with chronic wounds. It has rapidly evolved into a widely accepted treatment of chronic and acute wounds, contaminated wounds, burns, envenomations, infiltrations, and wound complications from failed operations. The ease of technique and a high rate of success have encouraged its adaptati

[Evaluation of the external tissue extender (Ete) in secondary wound closure].

BACKGROUND: For surgical closure of large skin defects, elaborate reconstructive plastic surgery or other methods such as internal subcutaneous balloon tissue expanders are required in order to avoid tension on the closure margins. Here we point to the benefits and disadvantages of an improved and simple method of secondary wound closure by secondary sutures. PATIENTS AND METHODS: We employed a system called External Tissue Extender (ETE), which consists of silicone strings and plastic stoppers pulling the corresponding surgical sites together and evenly distributing the tension. Possible ind

Patient safety in the operating room: II. Intraoperative and postoperative.

BACKGROUND: The perioperative environment can be hazardous to patients and providers alike. Although many risks are best addressed preoperatively, some hazards require constant attention by the surgeon, anesthesiologist, and staff in the operating room. In a previous article, the authors discussed preoperative aspects of patient safety. In this article, the authors review intraoperative and postoperative risks and techniques to decrease these risks. METHODS: The authors reviewed the literature regarding operating room safety, both primary research and secondary reviews, via multiple PubMed qu

Plastic solutions for orthopaedic problems.

INTRODUCTION: Infection and exposure of the implant may occur in 1-12% of patients operated on for arthroplasty or osteosynthesis. Variables such as tissue viability, presence of infection, exposure of osteosynthesis material and patient-related factors contribute to the lack of general consensus regarding the management of these defects. MATERIALS AND METHODS: Between January 1999 and January 2001, six patients were treated for complex soft-tissue defects following various orthopaedic procedures at the Department of Plastic Surgery in the Slotervaartziekenhuis in Amsterdam, a rheuma-orthopae

Teaching ear reconstruction using an alloplastic carving model.

BACKGROUND: Ear reconstruction is challenging surgery, often with poor outcomes. Our purpose was to develop a surgical training model for auricular reconstruction. METHODS: Silicone costal cartilage models were incorporated in a workshop-based instructional program. Trainees were randomly divided. Workshop group (WG) participated in an interactive session, carving frameworks under supervision. Nonworkshop group (NWG) did not participate. Standard Nagata templates were used. Two further frameworks were created, first with supervision then without. Groups were combined after the first carving b

Management of auricular hematoma and the cauliflower ear.

Acute auricular hematoma is common after blunt trauma to the side of the head. A network of vessels provides a rich blood supply to the ear, and the ear cartilage receives its nutrients from the overlying perichondrium. Prompt management of hematoma includes drainage and prevention of reaccumulation. If left untreated, an auricular hematoma can result in complications such as perichondritis, infection, and necrosis. Cauliflower ear may result from long-standing loss of blood supply to the ear cartilage and formation of neocartilage from disrupted perichondrium. Management of cauliflower ear in

Plastic bronchitis associated with adenovirus serotype 7 in children.

BACKGROUND: Plastic bronchitis is an uncommon but severe respiratory disease characterized by formation of casts in tracheobronchial tree. It can lead to airway obstruction and even respiratory failure. CASE PRESENTATION: Plastic bronchitis is mostly seen in both post-cardiac surgery patients, especially Fontan procedure, and infections including those caused by influenza viruses, Mycoplasma pneumoniae or tuberculosis. But it has rarely been reported to be associated with adenovirus infection. We report 2 cases of plastic bronchitis arising from adenovirus serotype 7 infection, manifested in

Revision surgery for malocclusion after open reduction of mandibular fracture: a case report.

Insufficient treatment of mandibular fractures can result in malocclusion, often necessitating revision surgery. Therefore, appropriate surgical treatment by oral and maxillofacial surgeons with adequate consideration of the occlusion is crucial. Moreover, in patients with bone defects between the fractured ends, bone grafting may be required. A 48-year-old man was referred to our department with malocclusion after initial surgery for mandibular fracture performed by plastic surgeons at another hospital. We performed revision surgery with bone grafting for a bone defect, and favorable postoper

Metoidioplasty.

Many patients who desire metoidioplasty want and need masculine appearing and functioning genitalia. A key part of masculinization involves moving tissue anteriorly. Standing micturition requires urethral lengthening. Vaginectomy, scrotoplasty, and perineal reconstruction are frequent adjunct procedures. Aesthetic and urinary complications are unfortunately common. With the various techniques and options available, shared decision making between surgeons and patients is vital to ensure the surgery needed is achievable with the surgeon's skill and the patient's anatomy.

A review of major reconstructive and gender-affirming surgical techniques in genital surgery and care for the transgender patient population.

The transgender community makes up a significant portion of the American population, yet there is a paucity of data available to surgeons and anesthesiologists. The purpose of this article is to provide information to surgeons, anesthesiologists, and other medical professionals. This narrative review explores the importance of medical information that healthcare professionals should be aware of ahead of gender-affirming surgeries. This review paper's purpose is to explore perioperative considerations for physicians before gender-affirming surgeries. Physicians should be aware of the hormone th

Orbital reconstruction.

PURPOSE OF REVIEW: Post-traumatic orbital reconstruction is a complex issue that involves both the soft tissue and bony injury. The current literature focuses primarily on bony reconstruction, with fewer investigators evaluating soft-tissue injuries. This article will review recent advances in orbital reconstruction, including presurgical planning software, intra-operative navigation, patient-specific implants, and intra-operative imaging. RECENT FINDINGS: Traditional techniques for diagnosis and orbital injuries continue to be refined; however, advances in computer-aided surgery are allowing

Pre-surgical nasoalveolar molding technique: a literature review.

Cleft lip and palate patients have many defects particularly nasal deformities. The nasoalveolar molding (NAM) technique is an adjunctive treatment, which not only corrects deviated alveolar segments but also addresses nasal deformity before cheiloplasty. NAM technique is claimed to facilitate primary surgical correction and to provide favorable esthetic outcomes. However, there is limited evidence to confirm the special benefits ofNAM technique since so far there have been no truly long term controlled clinical trials to evaluate outcomes of treatment based on pre-surgical NAM technique. NAM
