Comparison of methods to reduce dose to swallowing-related structures in head and neck cancer.

INTRODUCTION: Emerging data suggest that reduction of dose to the larynx and pharyngeal constrictor may lower the risk of swallowing complications such as long-term gastrostomy dependence and aspiration. Organ avoidance becomes difficult when the primary tumor or involved nodes are present at the level of the larynx. MATERIALS AND METHODS: Fifteen patients with Stage III-IV squamous cell carcinoma of the head and neck with high-dose target volume at the level of the larynx (but not involving the glottic larynx) were planned with whole-field IMRT (WF-IMRT), as well as a low anterior neck field

Patient dose during videofluoroscopy swallowing studies in a Hong Kong public hospital.

A videofluoroscopy swallowing study (VFSS) is a major tool in diagnosing swallowing disorders. Like all other medical examinations that involve irradiation, patient dosage is a major concern. Cerebrovascular accident (CVA) and nasopharyngeal carcinoma (NPC) have been two of the most common indicating pathologies for VFSS studies. The goals of this study were to determine the mean dose area product (DAP) value and fluoroscopic time for VFSS examinations in Tuen Mun Hospital, to compare the result with the dose reference level of other similar studies, and to document the results with respect to

Swallowing disorder and inhibition of cough reflex induced by atropine sulfate in conscious dogs.

In this study, the effects of atropine sulfate (atropine) on swallowing and cough reflex were evaluated in the two experimental models in conscious dogs. To evaluate the effects of atropine on swallowing, 1 mL of marker (contrast medium) was injected into the pharynx under X-ray exposure to induce swallowing. Baclofen, used as a positive control, caused marker congestion in the upper esophagus. In our experimental model, atropine (0.02 and 0.1 mg/kg, i.v.) dose-dependently increased not only the number of marker congestions but also that of the swallows. In addition, atropine significantly sho…

Swallowing disturbance due to isolated vagus nerve involvement in systemic lupus erythematosus.

Neuropsychiatric lupus is a well-recognized complication of systemic lupus erythematosus (SLE) and encompasses a wide variety of neurological manifestations. Although isolated cranial nerve involvement is rare, it can cause significant morbidity and disability. This report describes a case with isolated vagus nerve involvement as the principal sign of central nervous system involvement. A 30-year-old female with SLE had one short course of lupus-associated psychosis in December 2001. In August 2002, the patient suffered from progressive swallowing difficulty, developed a transient episode of h

Air swallowing, belching, acid and non-acid reflux in patients with functional dyspepsia.

BACKGROUND: Frequent belching is a common symptom in patients with functional dyspepsia with a reported incidence up to 80%. We hypothesized … dyspepsia patients and 10 controls. Analysis of the impedance-pH signals included incidence of air swallows, belching, acid and non-acid reflux. RESULTS: The incidence of air swallows in functional dyspepsia patients were significantly higher compared with controls (153 +/- 15 vs. 79 +/- 10, P < 0.001), while the incidence of liquid-only swallows were no …

The prognostic value of abnormal findings on radiographic swallowing studies after total laryngectomy.

Pharyngocutaneous fistulae are a common complication after total laryngectomy. Our study evaluates the correlation of postoperative radiographic swallowing studies and clinical symptoms. We also propose a grading system to classify leaks radiographically. The records of 45 patients who underwent total laryngectomy were retrospectively reviewed. All patients had a radiographic swallowing study (RSS) on or around the tenth postoperative day. A grading system was developed to classify radiographic findings (grade 0-5). Twenty-two patients had an abnormal RSS (grade 2-5). Three patients (13.6%) ha

Swallowing-induced atrial tachyarrhythmias: prevalence, characteristics, and the results of the radiofrequency catheter ablation.

BACKGROUND: Detailed information on swallowing-induced tachyarrhythmias has been lacking. METHODS: The prevalence, characteristics, and results of the radiofrequency catheter ablation (RFCA) of swallowing-induced tachyarrhythmias were examined in 544 patients with symptomatic premature atrial contractions (PACs), paroxysmal atrial tachycardia (AT), and/or paroxysmal atrial fibrillation (AF). We also conducted a search of the medical literature on swallowing-induced tachyarrhythmias. Further, we presented an in-depth review of the literature and investigated the published data on swallowing-in

["The swallowed bullet": illustration of a rare missile trajectory in a pediatric gunshot neck injury].

BACKGROUND: The probability of a bullet deglutition is a remote cause of consultation. OBJECTIVES: To communicate a clinical case of gunshot wound's penetrating cervical trauma in a pediatric patient, with a no frequent trajectory. METHODS: We describe and illustrate a clinical case of penetrating neck gunshot wound in the left zone III. At the initial radiological examination, we observed an intraabdominal location of the bullet. The analysis of the clinical case demonstrated an oropharyngeal injury, and a "swallowed bullet", without other lesions. RESULTS: We observed a satisfactory evolu

Radiofrequency ablation of swallowing-induced atrial tachycardia: case report and review of literature.

We report the case of a 47-year-old white man who presented with 1-year history of palpitations while swallowing. Event monitor confirmed the episodes were atrial tachycardia. Medical management with a calcium channel blocker did not alleviate the patient's symptoms. Gastrointestinal endoscopy did not reveal any abnormality. Electrophysiologic study was performed to identify and map the tachycardia. Swallowing during the procedure was used to repeatedly induce the tachycardia. A focal right atrial tachycardia that was mapped to the lower posterior right atrium was successfully ablated. Follow-

Recovery of swallowing disorders in patients undergoing supracricoid laryngectomy with botulinum toxin therapy.

In recent decades, functional laryngeal surgery has become a widespread method of treating glottic and supraglottic neoplasms, since it ensures an oncological outcome comparable to that of radical surgery and functional results that are conducive to a good quality of life. The most common postoperative complaints for this type of surgery are swallowing disorders, which can thwart good surgical results, especially when severe. Five supracricoid laryngectomees with swallowing disorders unresolved by speech therapy were treated by percutaneous injection of botulinum toxin under electromyographic

[Partial intestinal obstruction after swallowing a self-expanding sponge for weight reduction].

HISTORY AND ADMISSION FINDINGS: A 21-year-old woman presented with predominantly right-sided cramp-like abdominal pain which had started the previous day. The abdomen was soft on palpation, the peristaltic sounds increased. Body temperature was normal. INVESTIGATIONS: Routine laboratory tests were unremarkable, including white cell count (4.5 G/l). and C-reactive protein. TREATMENT AND COURSE: She was admitted to hospital for observation. Because the symptoms persisted she underwent a right lower laparotomy because acute appendicitis was suspected. Intraperitoneal examination of the intestin

Videofluoroscopy of the swallowing act after partial supraglottic laryngectomy by CO(2) laser.

The evaluation of functional deglutition in patients after partial supraglottic laryngectomy by CO(2) laser was performed by videofluoroscopy of the swallowing act. We studied 20 patients in PA and LL projections, and the swallowing act, at 25 frames per s, was captured on videotape. Aspiration was found in six patients, four of whom had manifest clinical symptoms of aspiration. Four patients had post-deglutitive, one intra-deglutitive and one both intra- and post-deglutitive aspiration. Other swallowing disturbances or structural deformities were not detected. Dynamic radiological studies of

Vocal cord closure pressure during volitional swallow and other voluntary tasks.

Our goal was to determine and compare vocal cord (VC) closure pressure and its associated intratracheal pressure during several physiological events such as swallowing, coughing, straining, and phonation. We studied 11 healthy subjects (age 41 +/- 2 years) with no current or previous history of laryngeal or pulmonary diseases. VC closure pressure during the above-mentioned tasks was studied using a concurrent manometric and endoscopic technique. VC closure pressure during dry swallows averaged 298 +/- 23 mm Hg, while intratracheal pressures exhibited a biphasic pattern ranging from -4 +/- 0.5 …

Swallowing function after endoscopic resection of supraglottic carcinoma with the carbon dioxide laser.

From April 1998 to May 2000, 14 patients with supraglottic cancer underwent transoral laser surgery (T-stage T1-2: 11 patients, T3: 3 patients). In three patients, an epiglottectomy or hemi-epiglottectomy was performed. In 11 patients, further structures (false cords, the valleculae and the base of the tongue and/or parts of the arytenoid cartilage) had to be resected. Thirteen patients had to undergo neck dissection and post-operative irradiation. Tracheostomy was carried out prophylactically in two cases. Every patient received a nasogastric tube perioperatively. One week after surgery, an e
